Feeding and Eating Disorders

This chapter discusses feeding and eating disorders, which are characterized by disturbed eating behavior and excessive concern about body weight and shape. There is substantial comorbidity of anorexia nervosa and bulimia nervosa. Many patients with bulimia nervosa have other mental health problems related to impulse control as well, such as substance use disorders. Patients with binge-eating disorder experience periods of consumption of objectively large amounts of food accompanied by a feeling of loss of control. Anorexia nervosa is one of the most lethal psychiatric illnesses, as it is associated with mortality rates as high as 5% per decade of illness.

Anorexia nervosa is an eating disorder with peak onset in adolescence, which carries the highest mortality rate of all psychiatric illnesses. It is commonly comorbid with other physical and mental health problems, yet training on management of people with eating disorders and working knowledge of clinicians working with underweight adolescents is inconsistent. This review of anorexia nervosa in adolescents provides an overview of the presentation, aetiology and treatment of this disorder, with a particular focus on the assessment and management of physical health risks, including refeeding syndrome.

Eating disorders involve maladaptive eating patterns accompanied by a wide range of physical complications likely to require extensive treatment. In addition, “eating disorders” frequently occur with other mental disorders, such as depression, substance abuse, and anxiety disorders. The earlier these disorders are diagnosed and treated, the better the chances are for full recovery” (NIMH, 2011). As of 2013, lifetime prevalence rates for anorexia nervosa, bulimia nervosa, and binge eating disorder are 0.9%, 1.5%, and 3.5% among females, and 0.3%, 0.5%, and 2.0% among males respectively (Hudson, Hiripi, Pope, & Kessler, 2007). Early diagnosis is imperative; the National Institute of Mental Health estimates that the mortality rate for anorexia is 0.56% per year, one of the highest mortality rates of any mental illness, including depression (NIMH, 2006). More recent research (Crow et al., 2009) indicates mortality rates as high as 4.0% for anorexia nervosa, 3.9% for bulimia nervosa and 5.2% for eating disorders not otherwise specified. Current research and treatment options are discussed.

Feeding and eating disorders are defined by persistent disturbance of eating (or behaviors related to eating) with subsequent changes in consumption or absorption of nutrition that are detrimental to physical health and social functioning. The following eating disorders are described in the DSM-5: anorexia nervosa, bulimia nervosa, binge eating disorder, pica, rumination disorder, avoidant/restrictive food intake disorder (ARFID), other specified feeding or eating disorder (OSFED), and unspecified feeding or eating disorder (USFED). ARFID, OSFED, USFED, rumination disorder, and binge eating disorder are new additions to the manual and are first described in the DSM-5. The DSM-5 also provides severity specifiers—mild, moderate, severe, and extreme—for the diagnoses of bulimia nervosa and anorexia nervosa. This review describes the eating disorders enumerated in the DSM-5 and provides information regarding their genesis and course. Clients of social workers have elevated suicide risks. One of those types of clients are people with eating disorders. Suicidality is associated with attitudes towards suicide. Although the suicidality of people with eating disorders is well-studied, their attitudes towards suicide have received less attention. The object of this research is to study the attitudes among people with eating disorders – 126 participants took place in this research: 63 people with eating disorders (anorexia nervosa, bulimia nervosa, binge eating disorder, other eating disorders) and 63 people without eating disorders. All the participants with eating disorders at the time of the research were receiving treatment at the Center for Eating Disorders (VšĮ Vilnius Mental Health center). The majority of participants were women (92%), with an average age of 25 years. All the respondents filled in an Attitudes towards suicide questionnaire (ATTS) and its supplement. The results showed that participants with eating disorders perceived suicide as less predictable and associated it with loneliness more than participants without eating disorders. Attitudes of people with anorexia nervosa and with bulimia nervosa towards suicide did not differ significantly. Participants with anorexia nervosa perceived suicide as less predictable, showed more nonintrusive attitudes towards suicide and associated it with loneliness more than participants without eating disorders. Attitudes towards suicide among people with bulimia nervosa and people without eating disorders did not differ significantly. The results could be explained by considering the characteristics of people with eating disorders and the methodology of this research. Recommendations for social work practice were formed based on the findings.

This chapter addresses the pharmacotherapy of the eating disorders (EDs). Many persons with EDs receive pharmacotherapy, but pharmacotherapy research for EDs has lagged behind that for other major mental disorders. This chapter first provides a brief rationale for using medications in the treatment of EDs. It then reviews the data supporting the effectiveness of specific medications or medication classes in treating patients with anorexia nervosa (AN), bulimia nervosa (BN), binge eating disorder (BED), and other potentially important EDs, such as night eating syndrome (NES). It concludes by summarizing these data and suggesting future areas for research in the pharmacotherapy of EDs.

The epidemiology of eating disorders holds important clues for understanding factors that may contribute to their etiology. In addition, epidemiological findings speak to the public health significance of these deleterious syndromes. Information on course and outcome are important for clinicians to understand the prognosis associated with different disorders of eating and for treatment planning. This chapter reviews information on the epidemiology and course of anorexia nervosa, bulimia nervosa, and two forms of eating disorder not otherwise specified, binge eating disorder and purging disorder.

Many persons with eating disorders (EDs) receive pharmacotherapy, but pharmacotherapy research for EDs has lagged behind that for other major mental disorders. In this chapter, we first provide a brief rationale for using medications in the treatment of EDs. We then review the data supporting the effectiveness of specific medications or medication classes in treating patients with anorexia nervosa (AN), bulimia nervosa, binge eating disorder (BED), and other potentially important EDs, such as night eating syndrome (NES) and sleep-related eating disorder (SRED). We conclude by summarizing these data and suggesting future areas for research in the pharmacotherapy of EDs.
