Malignant melanoma is the most common primary malignant tumour of the iris, but represents a small proportion of all uveal melanomas. The authors describe a 34-year-old male with a pigmented lesion of the iris. The lesion remained stable for 7 years, but the patient re-presented after this time with sudden enlargement of the mass and hyphaema. Excisional biopsy confirmed cavitary melanoma of the iris. This is the first reported case of cavitation in a primary iris melanoma. The patient has not had any further adjuvant treatment and remains metastasis free at 5 years of follow-up.

Background: Conjunctival malignant melanoma is a rare ocular malignancy most commonly affecting mostly the elderly population. It is a pigmented lesion which canarise from primary acquired melanosis, de novo or from preexisting nevus. Case: A 63 year old male presented with a chief complaints rapidly increasing mass in the left eye for two months following trauma with a wooden stick. He had preexisting nevus in the same eye. His best corrected visual acuity was 6/12 in right eye and 1/60 left eye respectively. Slit lamp biomicroscopy examination showed 4 cm x 4 cm pigmented mass in the nasal bulbar and inferior palpebral conjunctiva causing mechanical ectropion of the lower lid with keratinization of palpebral conjunctiva. Incisionalbiopsy of conjunctiva showed malignant melanoma. On computed tomography, there was expansion of bony orbit. Considering all the findings, exenteration of the left orbitwas done. Histopathological report of exenterated mass was suggestive of malignantmelanoma of conjunctiva. Conclusion: Conjunctival melanoma is a rare malignant tumor of eye which has high metastasis rate and the treatment option is surgery with adjuvant therapy.

Primary oral malignant melanoma, very rare neoplasm of melanocytic origin, usually presents as a bluish black to tan-brown colored lesion Which is accounting for 0.2 to 8% of all melanomas, 1.6% of all head and neck malignancies, and 0.5% of all oral neoplasia. In general, the prognosis of oral melanoma is poor and worse than that of cutaneous melanoma. Here a case of oral malignant melanoma is presented, which was undetected during the first visit to a dental clinic. When a simple oral surgical treatment was carried out in that region, it resulted in the appearance of a massive pigmented lesion which was histopathologically diagnosed as malignant melanoma. This paper is presented to reemphasize the fact that any pigmented lesion in the oral cavity should be viewed with suspicion and proper investigation (biopsy) should be carried out to rule out any untoward experiences later.

Isolated pleural effusion is a very rare presentation of malignant melanoma. A 46 year-old male patient presented to us with complaints of shortness of breath during the previous month. A contrast enhanced computed tomography (CECT) imaging scan of his thorax showed right-sided pleural effusion with the absence of any mass lesion or mediastinal lymphadenopathy. Cytology of his pleural fluid showed pigmented cells suggestive of malignant melanoma. Staining of the pleural fluid cells with immunohistological markers for melanoma (HMB 45 and S 100) were positive. An examination of his skin did not reveal any pigmented lesion, nor was there a past history of malignant melanoma for this patient. The patient responded well to therapy for malignant melanoma and has remained asymptomatic for the last year. This patient was diagnosed with a rare case of primary malignant melanoma of the pleura presenting as isolated pleural effusion.

<p class="abstract">Malignant melanoma of the oral cavity is an exceedingly rare tumor representing 0.2 to 8% of all melanomas. Mucosal melanomas are extremely rare and aggressive neoplasms. Patient reporting to the clinician with a pigmented lesion should raise suspicion in the first visit itself and should be further investigated so as to detect this dreaded malignancy at an earlier stage and thus managed appropriately. We presented two such rare cases who reported at our centre treated with different modalities and had different responses to treatment. In first case report 65 year old male patient diagnosed with malignant melanoma of left upper alveolus underwent multiple modalities of treatment like surgery, chemotherapy, radiation therapy but unfortunately as he defaulted post-surgery and also due to COVID-19 lockdown restrictions he was treated in various centres and finally the result was inoperable residual gigantic mass resistant to chemotherapy and radiation therapy. In second case report, 82 year old male patient reported with malignant melanoma hard palate having good response to initial chemotherapy. He was planned on hypo-fractionated radiotherapy in view of his old age but he refused radiation treatment and is on oral temozolamide and thalidomide with stable disease and good quality of life since past 6 months.</p>
