Relevance.Ectopic (extrauterine) pregnancy (EP) occupies a leading place in the structure of urgent gynecological morbidity. This pathology poses a threat to the health and life of a woman, being one of the leading causes of maternal mortality during pregnancy during the first trimester. Among emergency gynecological operations, surgical interventions for EP make up about 50%, and in recent years, most operations have been performed by the laparoscopic method. However, up to now, the pathomorphological changes in the fallopian tubes in patients with EP remain poorly understood.Purpose of the study. To study the features of pathomorphological changes in the fallopian tubes in women with tubal pregnancy, operated on by the laparoscopic method, to substantiate the volume of surgery in patients with this pathology.Material and methods. Morphological examination was performed in 100 women operated on for tubal pregnancy using the laparoscopic method. A comprehensive morphological study of the fallopian tubes removed during the operation was carried out.Results and discussion. The implantation of the ovum in the fallopian tube led to significant changes in its macro- and microstructure, which were caused by the invasion of chorionic villi and involved all layers of the tube wall, differing only in the depth of penetration and prevalence. The anatomical features of the structure of the fallopian tubes contributed to the deep invasion of the ovum into the myosalpinx and subserous parts of the tube, creating the possibility of wall rupture in this area.Conclusions. 1. In all cases of ectopic pregnancy, implantation of the ovum was accompanied by invasion of cytotrophoblast and syncytiotrophoblast, and it involved all layers of the tube wall, differing only in depth and prevalence. The invasive properties of the cytotrophoblast lead to the development of pronounced degenerative changes in the tube wall, which leads to functional inferiority of the tube after the onset of tubal pregnancy in it.2. The chronic productive endomyosalpingitis diagnosed in most patients with ectopic pregnancy with deformation of the tube lumen against the background of changes in the tube wall caused by cytotrophoblastic invasion is an indication for tubectomy. Performing organ-preserving operations on an anatomically and functionally altered fallopian tube is impractical, since it is a high risk factor for recurrent tubal pregnancy in this tube.

Ectopic pregnancy is one of the most pressing prob-lems associated with reproductive problems all over the world. Unfortunately, so far, impaired ectopic pregnancy in some cases leads to maternal mortality. Suspicion of ectopic pregnancy requires a full clinical and diagnostic examination and hospitalization of the patient in the gynecological Department. In most cas-es, ectopic pregnancy is localized in the fallopian tube. Most often, according to the literature, patients with ectopic pregnancy are subjected to surgical treatment, which subsequently negatively affects the reproductive function of women, since the removal of ectopic pregnancy leads to both the removal of the tube and its injury, which increases the risk of repeat-ed ectopic pregnancy. The choice of the type of surgi-cal intervention on the fallopian tube in ectopic preg-nancy should be personalized and appropriate to the specific clinical situation. This paper analyzes various types of surgical interventions on the fallopian tubes in ectopic pregnancy.

The aim: To analyze contemporary practical means to improve diagnostics of primary fallopian tube cancer. Materials and methods: Authors analyzed specifics of clinical signs and anamnesis in 152 PFTC patients. Diagnostic capacity of cytological analysis of pathologic vaginal discharge, X-ray contrast methods of examination, sonography, tumor markers, and computed tomography was studied. Own results of PFTC diagnostics using different methods and world practice using MRI, PET-CT and laparoscopy were discussed. Results: Using own observations authors conclude that clinical analysis and complex use of the listed methods allows to mainly determine high risk group patients and set correct preoperative diagnosis in 35% and preliminary diagnosis in 20% of PFTC patients. Conclusions: Complex examination allows to recognize primary fallopian tube cancer on preoperative stage and to avoid inadequate surgical interventions in majority of PFTC patients.

SummaryA marked fibrinolytic activity could be demonstrated in the blood of 10 patients with polycythaemia. In these cases fibrinolysis was considered the result of a deficiency an antiplasmin. It could be demonstrated that this naturally occurring inhibitor of fibrinolysis is present in normal platelets but is lacking in the patient’s own thrombocytes. This factor is probably attached to the surface of the thrombocytes. It could be removed by repeatedly washing. Lack of antiplasmin is a dysfunction of platelets and a form of thrombopathy. It does not respond on treatment with ACA or prednisone. In primary polycythaemia busulfan (Myleran) was considered to be the treatment of choice, whereas in 1 case of secundary erythrocytosis (Fallot’s tetralogy) the reaction on unroasted peanuts was unmistakable. In 4 more patients an alcoholic extract of peanuts was given, and promising results were obtained in 3 of them. The importance of this “thrombopathic type’ of fibrinolysis has been pointed out especially in view of surgical interventions.
