AbstractA 9-year-old girl patient presented with left-sided weakness and joint contractures developing over a period of 18 months. She was known to be suffering from β-thalassemia major and was on regular blood transfusions. Eighteen months ago, she had suffered from an episode of ischemic cerebrovascular accident affecting the right side of her brain. Magnetic resonance angiogram revealed vaso-occlusive disease affecting mainly the anterior cerebral circulation, resembling Moyamoya disease. She was advised to carry out regular physiotherapy but her parents discontinued it, which resulted in the gradual development of joint contractures and muscle wasting.

The “most able Cartesian philosopher,” Jean Denis (1635–1704), undertook a series of blood transfusions in 1667 and 1668 in Paris for therapeutic purposes, especially to cure madness, using the blood of animals. A range of actors and institutions opposed the controversial experiments, and the high law court of Paris condemned the practice in 1668. This article examines the attitudes toward animals and animal blood on both sides of the transfusionist debate and the resulting insistence on the “beast within” human nature that found a renewed expression at the beginning of the Classical Age.

Blood transfusions are associated with adverse physiologic effects and increased cost, and therefore reduction of blood product use during surgery is a desirable goal for all patients. Cardiac surgery is a major consumer of donor blood products, especially when cardiopulmonary bypass (CPB) is used, because hematocrit drops precipitously during CPB due to blood loss and blood cell dilution. Advanced age, low preoperative red blood cell volume (preoperative anemia or small body size), preoperative antiplatelet or antithrombotic drugs, complex or re-operative procedures or emergency operations, and patient comorbidities were identified as important transfusion risk indicators in a report recently published by the Society of Cardiovascular Anesthesiologists. This report also identified several pre- and intraoperative interventions that may help reduce blood transfusions, including off-pump procedures, preoperative autologous blood donation, normovolemic hemodilution, and routine cell saver use.A multimodal approach to blood conservation, with highrisk patients receiving all available interventions, may help preserve vital organ perfusion and reduce blood product utilization. In addition, because positive intravenous fluid balance is a significant factor affecting hemodilution during cardiac surgery, especially when CPB is used, strategies aimed at limiting intraoperative fluid balance positiveness may also lead to reduced blood product utilization.This review discusses currently available techniques that can be used intraoperatively in an attempt to avoid or minimize fluid balance positiveness, to preserve the patient's own red blood cells, and to decrease blood product utilization during cardiac surgery.

2006-ban jelent meg az Európai Rákkutatási és Kezelési Szervezet korábbi megállapításokat korszerűsítő ajánlása, amelyben a rákos betegekben kialakuló anémia kezelésére adott vértranszfúziót, illetve az ezt követő erythropoetin-kezeléseket értékelik. Az ajánlásban a vértranszfúziót a 9 g% hemoglobinszintre csökkenő anémia esetén javasolják. Eddig a kemoterápia következtében kialakult anémia vértranszfúziós kezelésére egyértelműen meghatározott hemoglobin-határértékszint Magyarországon nem szerepelt. Cél: A szerzők az osztályukon 2005-ben kezelt petefészekrákos betegeknek adott vértranszfúziók gyakoriságát vizsgálták. Nemzetközi, illetve hazai egyértelmű ajánlás hiányában a vizsgálati időszakban a vértranszfúziókat – a beteg klinikai állapotát is figyelembe véve – a 10 g%-ot elérő anémia esetén alkalmazták. Anyag és módszer: Az Országos Onkológiai Intézet Nőgyógyászati Osztályán 190 hám eredetű, petefészekrákos betegben történt kemoterápia. Ha a hemoglobinszint 10 g% alá csökkent, választott vörösvértest-transzfúziót végeztek, majd a betegek többségében (51/64 = 79,6%), erythropoetin-kezelés történt. Eredmény: A 190 közül 64 betegnél (64/190 = 34%) történt vérátömlesztés a kemoterápia kapcsán kialakult anémia miatt, s az utóbbiaknak csaknem felében (34/64 = 53%) 1-nél több alkalommal végeztek vértranszfúziót. A betegek 86%-ában a vértranszfúzióra G2-súlyosságú anémia miatt került sor. Az ismételten szükséges vértranszfúziókat a leggyakrabban a carboplatin-gemcitabin- (16/16) kezelések után alkalmazták. Következtetés: A petefészekrákokban adott kemoterápiák a betegek harmadában okoznak 10 g%-nál súlyosabb fokú anémiát. A vérszegénység kezelésében a vértranszfúzión kívül gondolni kell az erythropoetin-készítmények alkalmazására.
