Rapid adoption of information systems in an organization with poor management: the case of fund-holding in UK primary health care

ABSTRACT Objective: to analyze the clinical information systems used in the management of tuberculosis in Primary Health Care. Method: descriptive, quantitative cross-sectional study with 100 health professionals with data collected through a questionnaire to assess local institutional capacity for the model of attention to chronic conditions, as adapted for tuberculosis care. The analysis was performed through descriptive and inferential statistics. Results: Nurses and the Community Health Agents were classified as having fair capacity with a mean of 6.4 and 6.3, respectively. The city was classified as having fair capacity, with a mean of 6.0 and standard deviation of 1.5. Family Health Units had higher capacity than Basic Health Units and Mixed Units, although not statistically relevant. Clinical records and data on tuberculosis patients, items of the clinical information systems, had a higher classification than the other items, classified as having fair capacity, with a mean of 7.3 and standard deviation of 1.6, and the registry of TB patients had a mean of 6.6 and standard deviation of 2.0. Conclusion: clinical information systems are present in the city, mainly in clinical records and patient data, and they have the contribution of professionals linked with tuberculosis patients.

ABSTRACT Objective: TO analyze the presence and extent of the Primary Health Care attributes in the breastfeeding process based on the experience of the health professionals in the services certified by the Amamenta Brasil Network in comparison with non-certified services. Method: cross-sectional and descriptive study with a quantitative approach, using the PCATool-Brazil - version for professionals, which was applied to 53 health professionals, physicians and nurses, from a city in the State of São Paulo, Brazil. Descriptive and inferential statistical analysis was applied, using Student’s t-test and the Mann-Whitney test. Results: the services certified by the Amamenta Brasil Network scored higher on Longitudinality(6.7)and Coordination-information systems (8.3).The professionals who were trained by the Amamenta Brasil Network scored higher on Coordination-information systems(8.4)and on Comprehensiveness - available services (7.0).These two groups also gave more positive answers to the other attributes. Conclusion: the services certified by and the professionals who were trained by the Amamenta Brasil Network demonstrated a higher degree of orientation towards Primary Health Care. The data appoint that the better performance on the attributes is related, among other factors, to the professionals’ being better qualified to develop practices that value the promotion, protection and support of breastfeeding, in accordance with the principles of Primary Health Care.

Objetivo: Avaliar a influência das ações de alimentação dos Sistemas de Informação utilizados na Atenção Primária a Saúde (APS) sobre os cuidados de enfermagem ao indivíduo ou comunidade. Método: Pesquisa quantitativa, com enfermeiros da Atenção Primária. Utilizaram-se os testes ANOVA one-way, Kruskal-Wallis e correlação de Spearman com significância de 5% e confiança de 95%. Resultados: A maioria dos enfermeiros era mulher (94,5%), 34,4 anos de idade, 8,8 anos de formação, sendo 7,6 anos atuando na atenção primária, especialista e concursada. Observou-se associação negativa (p-valor = 0,008) entre tempo destinado às atividades de alimentação dos sistemas e o tempo de atenção aos pacientes. Conclusão: O estudo aponta a influência das ações gerenciais voltadas aos sistemas de informação sobre a atenção prestada aos indivíduos/comunidade.
