Microscopic and functional changes observed with dynamic optical coherence tomography for severe refractory atopic dermatitis treated with dupilumab

AimsTo investigate the structural changes of the superficial capillary plexuses (SCP) and deep capillary plexuses (DCP) using optical coherence tomography (OCT) angiography (OCTA) in patients with idiopathic macular hole (MH) after surgery, determine the factors related to changes of macular capillary plexuses and evaluate its association with postoperative visual outcomes.MethodsThirty-three patients with unilateral MH who were followed for ≥6 months after surgery were included. Ophthalmologic evaluations included best corrected visual acuity (BCVA) and spectral-domain OCT before surgery and 6 months postsurgery. En face OCTA images were obtained for both eyes at 6 months postsurgery, and the postoperative foveal avascular zone (FAZ) area and parafoveal vascular density were identified.ResultsCompared with fellow eyes, eyes after MH surgery had a smaller FAZ area in both SCP and DCP (p<0.05 for all). The FAZ area was positively correlated with postoperative foveal thickness of the whole, inner and outer layers (p<0.05 for all). In the parafoveal region, eyes after MH surgery had a tendency to have lower parafoveal vascular density, particularly in DCP (p=0.019). The parafoveal vascular density in DCP was positively correlated with retinal thickness of the whole, inner and outer layers (p<0.05 for all). Correlations between BCVA and FAZ area in both SCP and DCP were significant 6 months after MH surgery (p<0.05 for all).ConclusionAssessment of macular capillary plexuses using OCTA may be useful for monitoring retinal structural and functional changes in MH.

Purpose: to study the effect of the thickness and vascular density of the retina on its photosensitivity in diabetic macular edema. Material and methods. 9 patients (13 eyes) aged from 26 to 79 years (average age 62.1 ± 16 years) with a diagnosis of diabetic retinopathy (DR), diabetic macular edema (DME) were examined. In addition to the standard ophthalmological examination, a complex of specialized research methods was performed, including microperimetry, optical coherence tomography (OCT) and OCT with angiography (OCT-angio) of the posterior segment of the eye. At the same time, the influence of morphometric parameters on functional changes in the retina in the corresponding segments with a diameter of 6 mm was analyzed. Results. The study was a high inverse correlation relationship in the majority of cases between the parameters of the relevant sectors according microperimetry and OCT and OCT-angio in central zone of diameter 1, 3 and 6 mm and a direct correlation between central retinal thickness by OCT diagnosis and the so-called parameter K, denoting the number of points with sensitivity below 24 dB according microperimetry with a high degree of confidence (p<0.01, p<0.05). Conclusion. According to the results of our study, a direct correlation was found between the vascular density of the superficial and deep capillary plexuses and the parameters of photosensitivity in patients with DME. At the same time, the relationship between the thickness and light sensitivity of the retina is inverse. Abstract Chiasmal compression is a known cause of visual impairment, often leading to surgical decompression of the optic chiasm (OC). A prospective study was held at University Hospital in Hradec Králové to explore the benefits of optical coherence tomography (OCT) and visual evoked potentials (VEPs) in OC compression. 16 patients with OC compression, caused by different sellar pathologies, were included. The main inclusion criterion was the indication for decompressive surgery. Visual acuity (VA), visual field (VF), retinal nerve fibre layer (RNFL) and ganglion cell layer (GCL) thickness, and peak time and amplitude of pattern-reversal (P-VEPs) and motion-onset VEPs (M-VEPs) were measured pre- and postoperatively. The degree of OC compression was determined on preoperative magnetic resonance imaging. For M-VEPs, there was a significant postoperative shortening of the peak time (N160) (p < 0.05). P100 peak time and its amplitude did not change significantly. The M-VEPs N160 amplitude showed a close relationship to the VF improvement. Thinner preoperative RNFL does not present a statistically important limiting factor for better functional outcomes. The morphological status of the sellar region should be taken into consideration when one evaluates the chiasmal syndrome. M-VEPs enable detection of functional changes in the visual pathway better than P-VEPs.
