No relationship is indicated between FHIT expression and clinicopathologic prognostic parameters in early stage cervical carcinoma

In this study we investigated FHIT (Fragile Histidine Triad) protein alterations in cervical carcinomas to assess the relation of this gene with cervical cancer. Eighty-eight patients with surgically treated FIGO (International Federation of Gynecology and Obstetrics) stage IB carcinomas of the cervix were included in this study. Clinicopathologic prognostic factors were compared with FHIT expression status. Disease-free and overall survival was evaluated according to prognostic factors and FHIT expression. The FHIT gene was found to be depressed in 53% (47/88) of the tumors. None of the clinicopathologic prognostic parameters showed a correlation with FHIT expression. Univariate survival analysis with the Kaplan-Meier method showed that only the age of the patient is significantly correlated with disease-free survival. Interestingly, when the same analysis was done for 5-year overall survival; diameter of the primary tumor, depth of invasion, occurrence of lymph node involvement, and number of metastatic lymph nodes were found to be statistically significant. Furthermore, multivariate analysis with Cox regression revealed that lymph node involvement was the only independent variable for 5-year overall survival. In the present study there was no statistical correlation between FHIT expression and clinicopathologic prognostic factors or survival figures of the patients. These findings may be explained with the carcinogenic role of FHIT in tumoral progression but not in the tumoral development that takes place after the carcinogenetic period.

Abstract Background : To assess the prognostic value of preoperative serum cyfra21-1 in male patients with urothelial carcinoma of bladder treated with radical cystectomy.Methods: Patients underwent radical cystectomy from 2009-2018 at our center were retrospectively analyzed and 267 male patients met our criterions. The median follow-up was 34 months. The serum level of cyfra21-1 was measured using enzyme linked immunosorbent assay. Patients were divided into two groups (cyfra21-1≤3.30ng/ml and cyfra21-1>3.30 ng/ml). Clinical significance of cyfra21-1 level was assessed.Results: Of the 267 patients, 110 (41.2%) had normal cyfra21-1, while 157 (58.8%) had elevated serum cyfra21-1. The prevalence of lymph node involvement, locally advanced stage (≥ pT3), tumor stages, tumor size and papillary were significantly higher in patients with elevated cyfra21-1 than in those with normal cyfra21-1. Patients with high cyfra21-1 showed worse Disease free survival and Overall survival than those with low cyfra21-1 ( P = 0.001 and 0.007, respectively). In multivariate analysis, High cyfra21-1, lymph node involvement, lymphovascular invasion and papillary were independent predictors of worse Disease free survival ( P = 0.036, <0.001, 0.002, 0.014 respectively). High cyfra21-1, lymph node involvement and lymphovascular invasion were also confirmed as independent predictors of worse Overall survival ( P = 0.038, 0.010, 0.005, respectively.)Conclusions: Elevated cyfra21-1 was associated with greater biological aggressiveness and worse prognosis than normal cyfra21-1.

Introduction: Pleomorphic lobular carcinoma (PLC) is a rare variant of invasive lobular carcinoma (ILC) that has a higher degree of cell atypia and pleomorphism, and has been associated with factors with a worse prognosis such as larger tumor size, and greater lymph node involvement, however, its real prognostic value is still not well defined. Objectives: To evaluate the clinical-pathological characteristics, overall survival, and disease-free survival in PLC and compare it to ILC. Method: All ILC and PLC cases submitted to surgery at Hospital Nossa Senhora das Graças from January 1st, 2008 to January 31st, 2018 were analyzed. The main clinical and pathological variables related to prognosis were analyzed, as well as the overall survival and disease-free survival for both groups. Results and Discussion: 77 cases of ILC and 35 of PLC were found. The PLC group presented factors of worse prognosis in relation to the ILC group, such as a higher histological grade (p<0.001), presence of lymphovascular invasion (p<0.001), greater axillary lymph node involvement (p=0.003), and higher Ki-67 rates (p<0.001). Contrary to what was expected, we did not find a greater expression of HER2 (p=0.095) in the PLC, and there was also no significant difference between the groups regarding disease-free survival and overall survival. Conclusions: PLC still cannot be considered as an independent variable with a worse prognosis and more studies are needed to better understand this entity.

e12561 Background: early systemic therapy has reduced recurrence and mortality from BC, especially among HER2+ subtype. Despite these advances, the difficulty of developing countries in incorporating new treatments, as well as the molecular and phenotypic profiles of HER2+ BC, are associated with the heterogeneity of treatment responses, leading to poor prognosis and outcomes. However, real-world data on prognostic factors for early HER2+ BC patients are scarce, especially in the Brazilian context. Therefore, this study aims to evaluate the influence of prognostic factors on disease-free survival (DFS) and overall survival (OS) in early HER2+ BC patients. Methods: this retrospective study identified early HER2+ BC (stage I to III with positive immunohistochemistry and in-situ hybridization) patients from Pérola Byington’s public hospital database (São Paulo, Brazil) diagnosed between January 2010 to March 2018. Patients were excluded if they were less than 18 years old, participated in clinical trials, presented metastatic disease de novo, concurrent malignancy, or inconsistent data. Multivariate Cox regression was used to evaluate prognostic factors for survival, and OS and DFS were estimated by Kaplan-Meier analyses. Results: of the 1,625 patients identified in the database, 1,142 women were included in the study. Of those, 40% were diagnosed with less than 50 years old. Among the included patients, 40.3% were HR-/HER2+ and 59.7% were HR+/HER2+. In addition, 19.4% of patients were diagnosed at stage I, 42.9% at stage II and 37.7% at stage III. A total of 1033 patients were included in the DFS analysis, with a probability of 71.8% DFS in 5-years. For OS analysis, a probability of 75.4% OS in 5-years was observed for 1139 patients included. The multivariate analysis showed that tumor staging, lymph node involvement and pathological complete response (pCR) were independent prognostic factors for both DFS and OS. Conclusions: the study in the Brazilian cohort corroborates the literature, showing that tumor staging, pCR, and lymph node involvement are key markers for DFS and OS and they should be considered when managing the early HER2+ BC patients. Objective: To analyze the clinical, pathological, and sociodemographic aspects between triple-negative breast cancer (TNBC) and non-TNBC in a Brazilian cohort and identify potential prognostic factors. Methods: This hospital-based retrospective cohort study included 447 women with breast cancer treated at referral centers in Southeastern Brazil. Overall and disease-free survival were compared; prognostic factors were evaluated. Results: Triple-negative breast cancer corresponded to 19.5% of breast cancer diagnosis and was more prevalent among nonwhite and less educated women. The patients with TNBC tended to present with stage III cancer, high p53 expression, lymphocytic infiltration, and multifocality and treated with radical surgery and chemotherapy. The 5-year overall and disease-free survival were 62.1% and 57.5% for TNBC and 80.8% and 75.3% for non-TNBC, respectively ( P < .001). The TNBC recurrence was associated with multicentricity, whereas lymph node involvement increased the risk of both recurrence and death. Non-TNBC worse clinical course was associated with nonwhite ethnicity, lower education level, lymph node involvement, and advanced stage. Conclusions: Triple-negative breast cancer exhibited a more aggressive behavior, earlier and more frequent recurrence, and worse survival compared with non-TNBC. While biological and social variables were associated with poorer prognosis in non-TNBC, only lymph node involvement and multicentricity were correlated with worse clinical outcomes in TNBC.

Objective: Inflammatory markers are of prognostic importance in many malignancies. This study aimed to examine the effects of neutrophil-lymphocyte ratio (NLR), platelet-lymphocyte ratio (PLR), and pathological parameters on survival in preoperative complete blood counts in patients with operated gastric cancer. Material and Methods: Between 2012 and 2017, 281 patients were analyzed after total/subtotal gastrectomy. According to the ROC curve, we determined the cut-off values for NLR as 2.5 and PLR as 158. Overall survival (OS) was calculated from surgery to the last interview or to death. Results: In univariate analysis age ≥ 55 (p = 0.028), non-adeno hystology (p = 0.003), lenfovascular invasion (LVI) positivity (p = 0.003), perineural invasion (PNI) positivity (p < 0.001), T 3-4 stage (p = 0.006), lymph node involvement (LN) 2-3 (p < 0.001), metastatic stage (p <  0.001), NLR ≥ 2.5 ( p<0.001) and PLR ≥ 158 (p < 0.001) were statistically significant for OS. In multivariate analysis age (HR 0.652, 95% CI: 0.475-0.895; p = 0.008), PNI positivity (HR 0.493, 95% CI: 0.337-0.720; p<0.001), more lymph node involvement (HR: 0.608, 95% CI:0.412-0.896, p = 0.012), metastatic stage (HR 0.377, 95% CI: 0.265-0.537; p < 0.001) and PLR ≥ 158  (HR: 0.610; 95% CI: 0.433-0.859; p = 0.005) were found to be independent prognostic factors affecting OS. Conclusion: Age ≥ 55, PNI positivity, more lymph node involvement, metastatic stage, and PLR ≥ 158  are independent prognostic factors for shorter overall survival. Given the high morbidity and mortality of gastric cancer, besides classical known prognostic factors, parameters such as preoperative PLR may have benefits for forecast the prognosis of gastric cancer.

Objectives:The Glasgow Prognostic Score (GPS) is known to reflect the degree of tumor-associated cachexia and inflammation and is associated with survival in various malignancies. We investigated the value of the GPS in patients with cervical cancer.Methods:We included 244 consecutive patients with cervical cancer in our study. The pretherapeutic GPS was calculated as follows: patients with elevated C-reactive protein serum levels (>10 mg/L) and hypoalbuminemia (<35 g/L) were allocated a score of 2, and patients with 1 or no abnormal value were allocated a score of 1 or 0, respectively. The association between GPS and survival was evaluated by univariate log-rank tests and multivariate Cox regression models. The GPS was correlated with clinicopathologic parameters as shown by performing χ2 tests.Results:In univariate analyses, GPS (P < 0.001, P < 0.001), International Federation of Gynecology and Obstetrics (FIGO) stage (P < 0.001, P < 0.001), and lymph node involvement (P < 0.001, P < 0.001), but not patients' age (P = 0.2, P = 0.07), histological grade (P = 0.08, P = 0.1), and histological type (P = 0.8, P = 0.9), were associated with disease-free and overall survival, respectively. In a multivariate analysis GPS (P = 0.03, P = 0.04), FIGO stage (P = 0.006, P = 0.006), and lymph node involvement (P = 0.003, P = 0.002), but not patients' age (P = 0.5, P = 0.5), histological grade (P = 0.7, P = 0.6), and histological type (P = 0.4, P = 0.6) were associated with disease-free and overall survival, respectively. The GPS was associated with FIGO stage (P < 0.001) and histological grade (P = 0.02).Conclusions:The GPS can be used as an inflammation-based predictor for survival in patients with cervical cancer.
