Objective: The relationship between Hepatitis Delta infection and Helicobacter infection in patients with non-cirrhotic hepatitis B infection was retrospectively investigated. Material and Methods: Stool samples of 117 patients included with Delta hepatitis infection in the study At total 36 of them were tested for H. Pylori infection. To detect  H. Pylori, stool samples were tested using a commercial stool H. Pylori antigen assay. Results: Of these, 13 (19%) patients had H. Pylori seropositivity in the Hepatitis B infection group and 23 (48%) patients tested positive for H. Pylori infection in hepatitis delta infection group. There was a statistically significant difference between groups regarding H. Pylori seropositivity by the faecal test (p= 0.001). Conclusion: This study provides new knowledge on H. Pylori infection and reflects the need for evidence-based and comorbid dieases-oriented guidelines in the field of gastroenterology.

Objective: Although laparoscopic colon cancer surgeries have increased in recent years, their oncological competence is questioned. In our study, we aimed to evaluate oncological competence by comparing laparoscopic and open surgery. Material and Methods: The study was planned retrospectively. A total of 94 patients were included in the study, 42 of whom underwent laparoscopy, and 52 patients underwent open surgery. Both groups were compared in terms of demographic characteristics, staging, number of benign/malignant lymph nodes, histological findings and complications. Result: The final pathology report of all patients was adenocarcinoma. The median number of dissected lymph nodes was 20.9 in the open group (8-34) and 19.46 in the laparoscopy group (7-31) (p=0.639). The median number of dissected malignant lymph nodes was 1 (0-13) in the open surgery group and 3.1 (0-8) in the laparoscopy group (p=0.216). The laparoscopy group exhibited a longer operation time (281.2±54.2 and 221.0±51.5 min, respectively; P=0.036) than the open surgery group, but a shorter intensive care unit(ICU) discharge, quicker initiation oral feeding, and shorter length of hospital stay (4.0±0.9 vs. 5.7±2.0 days, respectively; P<0.001). Discussion: Laparoscopic surgery elicits many benefits such as less wound infection, lower requirement for blood transfusion, shorter hospitalization, quicker initiation of oral feeding and mobilization. Our study has shown that laparoscopic surgery provides quite adequate lymph node dissection when compared with oncological surgery, which is viewed with suspicion in the light of these benefits of laparoscopy.

Objective: Emergency surgical interventions due to colorectal cancer (CRC) obstruction are risk factors for poor prognosis. This study aims to compare emergency and elective surgeries for colorectal tumours performed in a single center. Material and Methods: CRC patients operated on between November 2014 and November 2019 were included in the study. Patients were divided into two groups; Patients operated under elective conditions, and patients operated under the emergency diagnosis of ileus or acute abdomen. Results: A total of 103 CRC patients were included in the study. Forty-five (43.7%) were operated in emergency situations, and 58 (56.3%) electively. 45.6% of the emergency cases were found to be Stage 3B and 4 (p=0.009). Bleeding and constipation were more common in elective cases, whereas in emergency cases, applications related to ileus and perforation were quite frequent (p<0.001). It was found that 62.3% of the tumors in emergency cases were seen in sigmoid and rectosigmoid regions (p=0.015). There was no anastomosis in 60.0% of emergency cases (p<0.001). Conclusion: In the hospital area where the study was applied, compared to other countries, more patients with CRC underwent emergency surgery for intestinal obstruction. Therefore, necessary measures must be taken to prevent further increases in these rates.

Objective: Anti-Mullerian hormone has been indicated as a novel biomarker for ovarian reserve assessment. This study aimed to determine the comparative advantage of serum levels of AMH, FSH, LH, E2, and  LH/FSH ratio among women with varying menstrual cycles and duration of menstruation.   Material and Method: A total of 90 subjects, which consisted of sixty subjects and thirty healthy subjects as control, were recruited. Blood samples were collected on day 3 of the menstrual cycle and evaluated for ovarian markers using the ELISA technique. All data were analyzed using SPSS version 23.0. Results: AMH and LH/FSH levels were lower in women with varying menstrual cycles than in the control group. FSH, LH, and E2 levels were significantly higher in women with varying menstrual cycles and flow duration than in the control group(P<0.05).  AMH was negatively correlated with age (r= -0.72), BMI( r= -0.4), FSH(r= - 0.3), LH(r = -0.2) (p<0.05) and E2(r= -0.2, p>0.05). Also age was positively correlated with FSH(r=0.5,p<0.05), E2(r=0.3, p<0.05) and BMI(r=0.4, p<0.05). The level of AMH was not significant with cycle length and days of flow(p>0.05). This implies that AMH can be measured independently of the cycle phase. This show that AMH was lower in women with varying menstrual cycle with an increase in gonadotrophin and E2. The strong negative relationship between age and AMH implies that age is determining factor of ovarian reserve.  Conclusion: AMH combined with age and FSH may improve ovarian reserve evaluation, making AMH a better marker

Objective: Serological tests for dental workers have been suggested by different international agencies to ensure the safety of dental practitioners and, subsequently, their patients. In our organization, the percentage of dental workers who underwent serological tests was low (26%). Material and Methods: An intervention was designed using three sequential PDSA cycles to test changes proposed by team members. The percentage of dental workers who underwent these tests was used as the measure. Results: During the project period, the percentage of dental workers who underwent serological tests within nine months increased from 24% to 87%. Amongst the three interventions, the final one exhibited the most prominent change leading to major improvement.  Conclusion: Serological tests are essential investigational data used to ensure the safety of dental workers, which subsequently also enhances patient safety. Further interventions are highly recommended to maximize the number of dental workers who undergo serological investigation.

Objective: This is a case report presenting two elderly patients; one with mesenteric ischemia and the second with gallstone ileus, in which their operative management has resulted in short bowel syndrome (SBS). Case: This pathology required prolonged post-operative care and monitoring with the management of different related complications. Conclusion: This case report will cover the pathophysiology, medical and operative management in addition to the acute and chronic complications of SBS

Objective: Androgen deprivation therapy (ADT) is used alone or in combination with docetaxel or androgen inhibitors in the initial treatment of metastatic prostate cancer (PC) (mPC). Enzalutamide is an androgen receptor inhibitor that is used orally and plays a role in different steps of the androgen receptor (AR) signal pathway. The aim of this study is to determine the real life data of patients using enzalutamide for metastatic PC. Material and Methods: The 118 patients from a totally 6 centers using enzalutamide treatment were included in this retrospective analysis. Clinical information of patients was recorded from patient files or automation records. Results: Median OS was 71 months, and median PFS was five months (4,1 – 5,9 months). There was no association of Gleason score with OS and PFS (p = 0.5 and p = 0.4, respectively). Although those who were metastatic at the time of diagnosis lived longer than those who developed metastases later, the difference was not statistically significant (p = 0.9). Likewise, there was no relationship between the time of metastasis development and PFS (p = 0.2). There was no difference in OS and PFS between patients with visceral metastasis and those without (p = 0.3, p = 0.5, respectively). Conclusion: Enzalutamide is an effective and safe agent in accordance with the literature in the patient group included in this study, although some patients may have an unresponsiveness to enzalutamide or develop progression under the enzalutamide treatment. More studies are needed to understand which patient group can benefit more from enzalutamide.

Objective: Parkinson Disease (PD) is known the second most frequent neurodegenerative age-related disorder after Alzheimer’s disease. Although over the six million people worldwide suffer from PD, the main cause of the disease remains are unknown. Speech and language impairments have emerged in most patients with PD during the course of the disease. However, clinical profiles or characteristics that might differentiate individuals with PD who are predisposed to speech and language deficits are generally overlooked. Moreover, factors that expedite language disability have still been remained elusive. It is thought that the awareness of speech and language impairments in PD can significantly help to maintain language abilities as the disease progresses and also may contribute to improving communication skills with patients. For this reason, the present study aims to constitute a comprehensive frame for the speech and language characteristics of individuals with PD

Objective: There are some studies about the significance of the peripheral blood  parameters in breast cancer. However, there is very few studies about prognostic importance of peripheral blood parameters in human epidermal growth factor-2 receptor (Her-2) positive breast cancer. We aimed to evaluate whether prognostic significance of peripheral blood parameters in patients Her-2 positive metastatic breast cancer with treated Pertuzumab, Trastuzumab, Docetaxel (PTD) Material and Methods: We included 56 patients with Her-2 positive metastatic breast cancer patients who were treated with PTD. We recorded patients' clinical,demographic features and we obtained peripheral blood parameters such as neutrophil-lymphocyte ratio (NLR), red blood cell distribution (RDW), mean platelet volume (MPV), lymphocyte, neutrophil after the  sixth cycle of the treatment and before the treatment. We separated the patients into two groups depending on the progression status. Progression-free survival was analyzed by Kaplan-Meier statistical analysis. Results: Patients mean age was 50.7. Progression was detected in 34 patients. When we explored and compared hemogram parameters in the groups before the treatment, there wasn’t statistically any significant difference between these parameters such as neutrophil, lymphocyte, neutrophil to lymphocyte ratio, mean platelet volume, red blood cell width. In the progressive group;,while pretreatment NLR was 3.83, it was detected 2.72 after six cycle treatment and difference was meaningful ( p: 0.043). The pretreamtent MPV was 8.63, and It was 8.15 after six cycle treatment, and difference between these counts was statistically important (p: 0.006). PFS was 18.0 months in the study group. Conclusion: Peripheral blood parameters were not statistically significant in both group comparisons. In the progression group, the difference between NLR and MPV count was statistically significant after the sixth cycle of the treatment and before the treatment.

Objective: The reverse transcription-polymerase chain reaction (RT-PCR) analyses method is the most important diagnostic method in the diagnosis of SARS-CoV-2 virus infection. In this research, we  aimed to investigate the positivity of SARS-CoV-2 by RT-PCR from distal part of the endotracheal tube (DPET) samples, which have not been investigated in any study yet. Materials and Methods: A total of 48 patients with a diagnosis of COVID-19 hospitalized in the intensive care unit receiving mechanical ventilation and whose conditions resulted in death or extubation were included in the study. The distal 6 cm part of the orotracheal intubation tube was removed from the patient (including the cuff). DPET samples were mixed with viral transport medium and vortexed; then, it was centrifuged at 4500g for 4 minutes. RNA isolation was performed by taking 400 µl from the supernatant and then SARS-CoV-2 RT-PCR was studied. Results: In 15 patients (31.25 %) the swab samples were PCR positive, 42 patients (87.5 %) had positive computed tomography finding and 48 patients (100 %) had positive clinical findings. Among the patients whose oropharynx (OP)/nasopharynx (NP) combined swab sample was positive for RT-PCR, the rate of RT-PCR positivity detected in DPET samples was 26.7%. While OP/NP combined swab sample was negative, DPET RT-PCR positivity rate was found to be 9.09%. Conclusions: Patients with positive DPET RT-PCR are detected when the swab is negative. These findings suggest that DPET can be used as a good lower respiratory sample without the risk of particle spread and transmission to healthcare personnel.
