Age of tobacco use initiation and association with current use and nicotine dependence among US middle and high school students, 2014–2016

IntroductionTobacco use mostly begins in adolescence and young adulthood. Earlier age of initiation of cigarette smoking is associated with greater nicotine dependence and sustained tobacco use. However, data are limited on the age of initiation of non-cigarette tobacco products, and the association between using these products and nicotine dependence and progression to established use.MethodsCombined 2014–2016 National Youth Tobacco Survey data, a nationally representative cross-sectional survey of US students in grades 6–12 yielded 19 580 respondents who reported ever using any of five tobacco products: electronic cigarettes, cigarettes, cigars, smokeless tobacco and hookah. Analyses assessed age of reported first use of each product among ever-users, overall and by sex and race/ethnicity. Current daily use, past 30-day use, feelings of craving tobacco and time to first tobacco use after waking were assessed by age of first use.ResultsAmong ever-users, weighted median age for first use was 12.6 years for cigarettes, 13.8 years for cigars, 13.4 years for smokeless tobacco, 14.1 years for hookah and 14.1 years for e-cigarettes. First trying these tobacco products at age ≤13 years was associated with greater current use of the respective product and nicotine dependence compared with initiating use at age >13 years.ConclusionsFirst tobacco use at age ≤13 years is associated with current daily and past 30-day use of non-cigarette tobacco products, and with the development of nicotine dependence among youth ever-users. Proven tobacco prevention interventions that reach early adolescents are important to reduce overall youth tobacco use.

The aim of the study was to assess the situation related to tobacco consumption among adolescents aged 13 – 15 in the Kyrgyz Republic.Materials and Methods. The study used a two-stage selection of schoolchildren in grades 7 – 9 (teenagers 13 – 15 years old) to conduct the survey. At the 1st stage, information was collected about all schools of the republic, where children of the target group study. the schools in which the survey was subsequently conducted were selected by the statistical sampling method according to the number of students in grades 7 – 9. The grades whose students participated in the questionnaire were set at stage 2 by random sampling in each school The survey includes 43 questions from the Standard main Questionnaire of the Global Youth Tobacco Survey (GYTS) and 30 supplementary questions (73 in total) with multiple choice of answers. The statistical processing was performed using the SUDAAN software package (US Centers for Disease Control and Prevention); a 95% confidence interval was calculated to calculate weighted prevalence estimates and standard errors (SE).Results. The survey found that 6.0% of schoolchildren (9.5% of boys and 2.4% of girls) currently use various types of tobacco; 4.4% of schoolchildren currently use tobacco (6.8% of boys and 2.0% of girls); 2.4% of schoolchildren smoke cigarettes; 2.4% use smokeless tobacco, 2.8% use other tobacco products (hookah); 2.8% of schoolchildren smoke electronic cigarettes (3.9% of boys and 1.7% of girls). Tobacco use most often begins before age 7 or after age 12. 16.8% of schoolchildren are exposed to tobacco smoke at home, of which 14.2% are in closed places and 14.8% in open public places. 87.2% of schoolchildren purchase cigarettes in a shop, kiosk, from a street vendor, and in the market. During the visit to the point of sale 26.7% noticed tobacco advertising and promotions; 49.7% saw anti-tobacco messages in the mass media. Definitely think that smoking tobacco by other people is bad for them, 71.2% of schoolchildren.Conclusion. A high percentage of teenagers consuming e-cigarettes, hookah, and smokeless tobacco products has been noted, although > 50% of them are aware of the dangers of tobacco use. Every 6th student is a passive smoker. The vast majority of schoolchildren aged 13 – 15 who try to quit smoking do not receive professional help in quitting tobacco use, and are exposed to protobacco media marketing campaigns.

Purpose: To understand the effect of pro-tobacco marketing on electronic cigarette and combustible cigarette dual use among US middle and high school students under 18 years of age. Design: Data were derived from the 2018 National Youth Tobacco Survey, an annual self-administered school-based cross-sectional survey. Setting: The survey was administered in public and private middle and high schools across the United States. Participants: The probability sample size was 15 238 middle and high school students with complete responses who were under 18 years of age during the study period. Measures: The study measured self-reported exposure to online combustible and electronic cigarette advertisements, dual use of combustible and electronic cigarettes during the past 30 days, exposure to the Real Cost antitobacco campaign advertisements, and other sociodemographic factors (eg, race/ethnicity, gender, and grade). Analysis: Logistic regressions were used to measure pro-tobacco marketing exposure and dual use as a function of pro-tobacco marketing exposure. Results: Descriptive analyses show that 59.0% of respondents were exposed to pro-tobacco online marketing, and 2.9% were dual users. Dual users (odds ratio [OR] = 1.73) and high school students (OR = 1.43) were more likely to report exposure to online pro-tobacco marketing. Conclusions: Findings indicate that a gap in electronic cigarette pro-tobacco marketing regulatory oversight may exist. Further policy action may be warranted to protect the public health of minors and other vulnerable populations who are most susceptible to pro-tobacco marketing.

Smoke-free ordinances and policies protect youth from exposure to secondhand smoke (SHS) and cigarette use. This study investigated whether smoke-free ordinances also protect youth from the use of other tobacco products. We compared the prevalence of SHS exposure, cigarette smoking, cigar smoking, smokeless tobacco use, and e-cigarette use among high school students living in a municipality with or without a smoke-free ordinance and in homes with and without smoke-free policies. Data were analyzed using the 2017 Mississippi Youth Tobacco Survey (n = 1923). Smoke-free ordinances were found to be associated with lower prevalence of SHS exposure (41.9% vs. 51.5%), cigarette smoking (5.1% vs. 11.4%), and cigar smoking (7.2% vs. 10.9%). There were no differences in smokeless tobacco use (6.6% vs. 6.5%) or e-cigarette use (11.2% vs 12.1%). Smoke-free homes were associated with lower prevalence of SHS exposure (38.0% vs 74.6%), cigarette smoking (4.8% vs. 17.6%), cigar smoking (6.4% vs. 16.4%), smokeless tobacco use (4.9% vs. 13.2%), and e-cigarette use (9.6% vs. 19.5%), p < 0.05 for all comparisons. The results suggest that smoke-free ordinances and policies protect against exposure to tobacco smoke and use of combustible tobacco products, but smoke-free ordinances do not protect from smokeless tobacco and e-cigarette use. Tobacco-free, rather than smoke-free, ordinances might offer more protection.

Introduction: In Appalachia, youth tobacco use rates exceed the national average, and youth in these rural areas are increasingly aware of new and emerging tobacco products, such as e-cigarettes. Given that e-cigarettes are a commonly used tobacco product by youth, additional inquiry is needed to better understand both youth product perceptions and use patterns, especially in communities with entrenched acceptance of tobacco. Study Aim and Hypothesis: Our aim is to examine the prevalence of conventional tobacco and e-cigarette use among Appalachian youth and to examine associations between use and tobacco harm perceptions. We hypothesized that conventional tobacco users would be less likely than non-users to perceive e-cigarette use as harmful and that e-cigarette users would be less likely than non-users to perceive e-cigarette use as harmful. Methods: A survey of middle and high school students (n=1,008) was conducted in rural Appalachia. The primary outcome in this analysis, tobacco use, was categorized as never user, conventional tobacco only user (cigarettes or smokeless), or e-cigarette only user. Descriptive characteristics were compared among the three use groups. Relationships between harm perceptions and tobacco product use were assessed using multinomial logistic regression models adjusted for age, race, gender, and grade level. Results: Nearly one-fourth (23%) of the participants had tried e-cigarettes. Compared to never users, conventional users were more likely to indicate that e-cigarettes did not cause: (1) health problems (OR: 1.65; CI: 1.075-2.521) or (2) addiction (OR: 1.63; CI: 1.064-2.496). Youth who indicated e-cigarettes do not cause health problems were more likely to be e-cigarette users than non-e-cigarette users (OR: 2.04, CI: 1.460-2.838), and youth who indicated smoking does not cause addiction were more likely to be conventional users than never or e-cigarette only users (OR: 1.96; CI: 1.181-3.262). Conclusions: In summary, youth who use conventional tobacco products or e-cigarettes are less likely to perceive e-cigarettes as harmful. These findings reinforce the need for vigilance in restricting youth access to tobacco products, increased emphasis on health campaigns that clarify scientific uncertainty surrounding tobacco safety, and regulation of e-cigarette marketing that targets youth.

While tobacco use nationwide is declining, smokeless tobacco (SLT) use remains steady, particularly among men in rural areas. Despite the harms of SLT, researchers know little about what initiates and sustains this use. In this study, we argue that SLT persistence is encouraged by its salience as an emblem of rural manhood. Based on interviews with 64 male and 19 female rural high school students we find that SLT symbolizes rural masculinity, and that many boys “spit and dip” to perform their status as rural men. We specify several peer, family, and community mechanisms that undergird this process. Finally, we discuss implications for men’s health research and intervention efforts.

Abstract BackgroundTobacco use among adolescents has long term adverse health consequences, especially during adulthood. Currently, little is known about tobacco use behaviour among adolescents in Comoros. Our study aims to estimate the prevalence and identify key factors associated with tobacco use among adolescents in Comoros using the 2015 Comoros Global Youth Tobacco Survey data.MethodsNational cross-sectional survey data of 2,810 eligible school-going adolescents aged between 11-17 years were analysed. Complex sample logistic regression analyses to determine the correlates of current cigarette smoking and current use of non-cigarette tobacco products.ResultsThe overall prevalence of current cigarette smoking was 14.3% [males (18.5%), females (9.9%)]. The prevalence of current use of non-cigarette tobacco products was 5.8% [males (6.7%), females (4.9%)]. Being male (AOR=2.23;95%CI:1.38-3.59), exposure secondhand smoke within (AOR=3.869;95%CI:2.835-5.280)) and outside their home (AOR= 1.514; 95%CI: 1.100-2.084) and exposure to tobacco industry promotion (AOR=2.986; 95%CI:2.246-3.969) were predictors of current tobacco use among adolescents. Similarly, Exposure to tobacco industry promotion (AOR=.2.669;95%CI:1.610-4.425) were associated with non-cigarette tobacco use. Adolescents exposed to anti-smoking education in schools were less likely to use non-cigarette tobacco (AOR=0.523;95% CI:0.311-0.881) than those not exposed to anti-smoking education in schools.ConclusionOne in seven school-going adolescents smoke cigarettes, and approximately one in 20 school-going adolescents use non-cigarette tobacco products in Comoros. Exposure to secondhand smoke within and outside the home and exposure to tobacco industry promotion were associated with tobacco use in school-going adolescents in Comoros. Our findings suggest the need for adolescent-friendly gender-sensitive tobacco interventions, including strengthening existing tobacco control laws to prevent and reduce tobacco use among school-going adolescents in Comoros.

The battle against tobacco usage is being fought on all fronts. On December 19, 2019, a measure to raise the minimum age to buy tobacco products to 21 from 18 was passed by the United States Congress and signed by President Donald Trump. This instated banning the sale of all tobacco products and electronic cigarettes to anyone in the US under the age of 21. This follows the raising of the age to buy tobacco in California to 21 in 2016. According to the California Tobacco Control Program: in 2016, roughly 10% of high-school students were smoking cigarettes, but by 2018, only 2%. The percentage of retailers selling tobacco to underaged youth dropped dramatically. These data show that the CA Tobacco 21 law was effective in decreasing the obtainability and usage of tobacco by youth. We expect that US Tobacco 21 will be similarly effective in reducing tobacco use by youth leading to less tobacco addiction in the US.
