Effects of Kampo Herbal Medicine on Plasma Melatonin Concentration in Patients

There are studies reporting that some natural products containing melatonin. Sleep disturbance and nocturnal polyuria are related to abnormal biorhythm. It is possible that Kampo medicine may influence biorhythm. We therefore examined the symptoms and plasma melatonin of patients treated with Rokumi-gan, Hachimi-jio-gan, Gosha-jinki-gan and Seishinrenshi-in. Furthermore, the concentration of four kampo medicines and crude drugs making up these formulations were investigated. Diurnal plasma melatonin was not affected by Rokumi-gan, but tended to be lowered by treatment with Hachimi-jio-gan, Gosha-jinki-gan and Seishinrenshi-in. Melatonin was present in several extracts from crude drugs, namely Rehmanniae radix, Moutan cortex, Cinnamomi cortex, Aconiti tuber, Plantaginis semen, Scutellariae radix and Lycii radicis cortex.

Effects of keishibukuryogan (KBG) on platelet aggregation were investigated. To ensure the specificity of KBG, tokishakuyakusan (TSS) and kamisyoyosan (KSS), which are known to have platelet aggregation-inhibiting effects, and rikkunshito (RKT) and shakuyakukanzoto (SKT), which are considered to be devoid of such effects, were used for comparison. The platelet aggregation of each test drug was measured by the screen filtration pressure method using whole blood of guinea pigs and expressed as a collagen-induced pressure rate (%) or a collagen concentration required for 50% increase in the pressure rate (PATI value). KBG suppressed the collagen-induced whole blood pressure rate increase and increased the PATI value, like TSS and KSS. Neither RKT nor SKT showed these effects. The Moutan cortex and Cinnamomi cortex, the constituent crude drugs of KBG, showed KBG-like pressure rate suppression and PATI-increasing effects. Furthermore, paeonol, a representative component of Moutan cortex, and aspirin which is known to have platelet aggregation-inhibiting activity (COX-1 inhibitor) also showed similar effects. These results suggest that the platelet aggregation-inhibiting activity of the constituent crude drugs Moutan cortex and Cinnamomi cortex is involved in the improving effects of KBG on impaired microcirculation and that paeonol plays a role in these effects.

To evaluate the influence of natural photoperiod on the daily rhythm of melatonin, we assessed melatonin plasma concentration at four different times of the year. Ten Sarda breed goats were led to the natural pasture during the day and were kept in a common stall during the night. Blood samples were collected by means of a cannula inserted into the jugular vein, every two hours for a 24-hour period during the vernal equinox, summer solstice, autumn equinox, and winter solstice. Our results showed the existence of clear seasonal variation in the daily rhythm of plasma melatonin in goats, with the highest MESOR value in winter followed by spring. Acrophase was always observed in the middle of the scotophase. Robustness of the rhythm was higher in the spring as compared to the winter. Our results showed a daily rhythm of plasma melatonin concentration strongly related to the seasons. These results could be useful to establish appropriately timed melatonin and/or photoperiod treatments for the management of reproductive physiology in goats.

Pulmonary inflammation is a hallmark of many respiratory diseases such as asthma, chronic obstructive pulmonary disease (COPD), and acute respiratory syndrome distress (ARDS). Most of these diseases are treated with anti-inflammatory therapy in order to prevent or to reduce the pulmonary inflammation. Herbal medicine-derived natural products have been used in folk medicine and scientific studies to evaluate the value of these compounds have grown in recent years. Many substances derived from plants have the biological effectsin vitroandin vivo, such as flavonoids, alkaloids, and terpenoids. Among the biological activities of natural products derived from plants can be pointed out the anti-inflammatory, antiviral, antiplatelet, antitumor anti-allergic activities, and antioxidant. Although many reports have evaluated the effects of these compounds in experimental models, studies evaluating clinical trials are scarce in the literature. This review aims to emphasize the effects of these different natural products in pulmonary diseases in experimental models and in humans and pointing out some possible mechanisms of action.

Plasma melatonin concentration of immature male elephant seals was determined by radioimmunoassay. Comparison of concentrations during two 24-h periods, one in midsummer and one in midwinter, showed that there was a marked circadian cycle in winter which was greatly modified during the long day length of summer. It is suggested that in summer there was sufficient ambient lighting during the night hours to depress the nocturnal rise in plasma melatonin. The complexity of pineal cycles in the natural environment is stressed, and in this regard the polar regions are of particular interest due to the extreme seasonal changes in day length there.
