From a clinical point of view, the most common presentations of cutaneous metastatic disease are papules and nodules. However, a wide morphological spectrum of lesions has been described, including erythematous patches or plaques, inflammatory erysipelas-like lesions, diffuse sclerodermiform lesions with induration of the skin, telangiectatic papulovesicles, purpuric plaques mimicking vasculitis, and alopecia areata like scalp lesions. The so-called zosteriform pattern has been described to be in few cases and to the best of our knowledge has never been described associated with a metastasis of a nasopharyngeal carcinoma. This case highlights the relevance of including cutaneous metastases in the differential diagnosis of patients with nonhealing herpes zoster-like lesions, especially in those with underlying neoplasm recently diagnosed.

The corkscrew aorta is a variant of the normal anatomical course of the aorta. This rare condition is characterized by a marked tortuosity of the aorta. In our experience it concerns the tract of subrenal aorta, that is an unusual condition, since there are no other cases in the literature. It is characterized by the presence of at least two kinking, and a coiling interposed among them. It is diagnosed by Angio-CT and its response is incidental, being from an asymptomatic clinical point of view.

This paper describes a clinical case of secondary syphilis, which was manifested exclusively by syphilitic alopecia.We describe the details of the clinical picture, as well as a differential diagnosis of syphilitic alopecia and alopecia areata on the basis of trichoscopy data.For a patient with scalp lesions typical of syphilis (focal form of alopecia), the trichoscopic picture was represented by hairs broken at different heights from 1 to 5 mm, “empty” follicles (“yellow” dots), anisotrichosis due to the loss of terminal hair. The eyebrow trichoscopy showed “yellow dots”, vellus hairs, hair thinning due to the loss of bristly hair. A comparison of the trichoscopic pictures for alopecia areata and syphilitic areata have revealed the absence in the latter of a number of signs typical of the former, such as pencil-point, ex clamation-point, zigzag, monilethrix and tulip hairs.Patients with a clinical picture of focal, diffuse or mix ed alopecia should be tested for syphilis using serological methods.

AbstractObjectives:To present a rare case of rectal adenocarcinoma metastasising to the external auditory canal, in a patient in whom computed tomography staging of the abdomen, chest and pelvis was clear for metastatic disease.Methods:Case report and review of the literature.Conclusions:Metastatic tumours to the external auditory canal are rare, with rectal adenocarcinoma being one of the rarest. However, it is important that the external auditory canal is regarded as a potential site for metastasis, even when computed tomography staging is clear. Metastatic tumour should be included in the differential diagnosis of a patient presenting with an ear mass.

Esophageal cancer represents a neoplasm that is thought to have both an increased incidence and prevalence in the following years. Although much progress has been made in the detection and the early treatment of esophageal cancer, the prognosis is still limited, and mortality remains very high. The most common histological types of esophageal cancer are squamous cell carcinoma, and, respectively, adenocarcinoma. Lately, there has been an accelerated increase in the incidence of adenocarcinoma, in the context of increased prevalence of gastro-esophageal reflux disease and obesity, but also of the current alimentary diet, especially in developed countries. The esophagus has its anatomical features. Moreover, it is located topographicallyin a complex cervico-thoraco-abdominal area, unique for a viscus. From the clinical point of view, the onset of symptoms of esophageal cancer is insidious. For these reasons, special attention should be pain in the early detection and differentiation of this neoplasm from other pathologies. These pathologies are very varied and may comprise other esophageal diseases, neighborhood pathologies such as cervical, thoracic, abdominal, systemic pathologies (immunologic, infectious) or other pathologies such as the oro-maxillo-facial, oculo-orbital, vascular, muscular, and cutaneous ones. Leiomyoma is a benign mesenchymal tumor that develops from smooth muscle cells. It can present in various histological variants. Leiomyoma with bizarre nuclei is an infrequent variant of uterine smooth muscle neoplasm. It is characterized by focally or diffusely distributed bizarre cells on the background of a typical leiomyoma. These bizarre cells are large, multinucleated, or multilobulated and have an eosinophilic cytoplasm. Even though leiomyomas with bizarre nuclei display benign clinical behavior, their differential diagnosis from leiomyosarcoma can sometimes be difficult. Leiomyoma has been described most commonly in the uterus. There is no case of leiomyoma originating from paratubal cysts described in the literature. In this article, we present a rare case of leiomyoma with bizarre nuclei originating from a paratubal cyst.
