Ventral Cleft of the Larynx in an Adult

Congenital clefts of the larynx are rare and usually found dorsally. This case report describes a patient with a partial anterior nonfusion, or ventral cleft, of the thyroid cartilage. This was first noted on computed tomography of the larynx used for delineation of carcinoma and was confirmed by horizontal whole mount histologic sections of the resected larynx. The thyroid cartilage suggested arrested fusion of the laminae in the middle to late embryonic period. This patient had an adult form of this rarely reported anomaly and the first, to our knowledge, detected with CT scan.

Segmental omental infarction (SOI) is a rare cause of acute abdominal pain. Depending on the site of infarction, it mimics conditions like appendicitis, cholecystitis, and diverticulitis. Before the widespread use of Computed Tomography (CT), the diagnosis was usually made intraoperatively. SOI produces characteristic radiological appearances on CT scan; hence, correct diagnosis using this form of imaging may prevent unnecessary surgery. We present the case of a young woman who was treated conservatively after accurate radiological diagnosis.

Introduction: Superior mesenteric artery (SMA) syndrome is a rare cause of upper gastrointestinal obstruction. Diagnosis is confirmed via computed tomography (CT) scan showing acute angulation at the origin of superior mesenteric artery compressing on the duodenum causing proximal dilatation of the second part of duodenum.

AbstractA 67-year-old woman with rheumatoid arthritis was hospitalized because of dysphagia and severe nodulosis. Over a two-year period the patient had been treated with methotrexate. A computed tomography (CT) scan of the neck showed a 2 × 2 cm large tumour behind the top left lateral thyroid cartilage. A biopsy taken during direct laryngoscopy showed it was a rheumatic nodule. Treatment with colchicine reduced the patient's dysphagia. As methotrexate is used increasingly in the treatment of rheumatoid arthritis and as this particular drug causes rheumatic nodules in five to 10 per cent of the patients, it must be foreseen that the incidence of nodules in the upper airways will increase.

A facial medial dermoid cyst in a 7-year-old male child is presented along with its Computed tomography (CT) imaging scans. The CT scan images depict clear nasal bone indentation with no erosion. Dermoid cysts are a subset of benign heterotopic neoplasms termed choristomas and periorbital dermoid cysts account for up to 9-10% of paediatric of head and neck dermoid cysts. The periorbital dermoid may occur in two variants as per the plane of location: anterior (superficial) or deep. The presented case in this report is of anterior variety at frontonasal suture.

Introduction. About 5%?10% of mediastinal tumors in adults are teratomas and about 85% of them are benign. Case report. We report a case of extragonadal malignant teratoma in a 39-year old man. The computed tomography (CT) scan of the chest revealed the soft-tissue density mass in the middle lobe of the lung. The posterolateral thoracotomy was performed and a mediastinal tumor of 25 cm was completely resected. Histopathological findings confirmed malignant teratoma. One month after the resection the positron emission tomography- computed tomography (PET/CT) scan of the whole body was performed and showed progression of the disease. Patient developed signs of the superior vena cava obstruction and received radiotherapy of the mediastinum and the metastatic lesion of the lumbal vertebra. After radiotherapy, the patient developed paraplegia and urinary incontinence and received the best supportive treatment. Conclusion. Primary extragonadal germ cell tumors have poor prognosis due to their relative chemoresistance and frequent findings of advanced disease after establishing the diagnosis.

We present a large hydatid cyst located midline and posterior to prostatic gland and seminal vesicles in an asymptomatic patient. Computed tomography (CT) scan revealed a large retrovesical mass and the transrectal ultrasonography idetified a degenerated hydatid cyst with “ball of wool’’ appearance. Although extremely rare, hydatid disease should be kept in mind in the differential diagnosis of pelvic particularly retrovesical midline masses.

This paper reports the case of a patient who presented limited buccal opening, though, without apparent symptomatology. The patient was partially toothless and exodontia of the remaining teeth was indicated. Clinical examination and clinical interview were performed as well as panoramic radiograph, transcranial radiograph of the temporomandibular joints (TMJ) and, later, computed tomography (CT), emphasizing the importance of complementary exams in the diagnostic phase. Analysis of the CT scan confirmed the structural alteration in the condylar and coronoid processes of the jaw, explaining the limited buccal opening. The remaining teeth were extracted and complete upper and lower dentures were fabricated, reestablishing the occlusal relationship and the patient's stomatognathic functions. Surgical treatment of the coronoid process was discarded.

A 13-month-old Rottweiler was presented with a lameness on the left front leg. The CT scan revealed a large fragmented coronoid process in the elbow joint which was not demonstrable on regular radiographs.

Introduction. Inflammatory myopathies are a rare group of diseases characterized by proximal weakness. Incidence ranges from 7.98/million/year and prevalence at 14/100,000. The utility of [18F] fluorodeoxyglucose (FDG) positron emission tomography (PET)/computed tomography (CT) scan is increasing for the complementary diagnosis of myopathies. Case Report. An 84-year-old male was admitted with a history of difficulty rising from a chair and a fall. Laboratory results showed increased creatine kinase levels of more than 50 times the normal reference values. Electromyography (EMG) showed myopathic changes, and FDG-PET/CT scan showed increased FDG uptake in bilateral quadriceps. A biopsy was performed revealing lymphocytic predominant infiltrates and myonecrosis. Prednisone and intravenous immunoglobulin (IVIG) were administered with strength improvement. The patient was discharged for further follow-up. Discussion. FDG-PET/CT in inflammatory diseases has proven useful as muscle fibers have increased FDG uptake. In some cases, FDG-PET/CT is also useful in determining associated neoplastic diseases.
