Analysis of 100 Cases of Paget's Disease of the Breast

Of 100 cases of Paget's disease of the breast admitted to the National Cancer Institute of Milan from 1940 to 1974, 91 were statistically evaluated. They were divided in two groups according to presence or absence of a palpable nodule. The results of surgical treatment in terms of 5 and 10 year survival rates were 59 and 44 %, respectively, with a median survival of 9 years. For the two separate subgroups, those with a palpable nodule were 38 and 22 % for 5 and 10 years, respectively, while those without a palpable nodule were 92 and 82 % for 5 and 10 years, respectively. For the two groups the median survival was 3.6 and 16.4 years, respectively. The extent of surgery should be dependent on the presence or absence of palpable nodules under the nipple. For the two groups (with and without) extended radical mastectomy and the Patey-Dawson mastectomy are recommended.

Paget’s disease of the breast is a rare type of cancer of the nipple–areola complex and that is often associated with an underlying in situ or invasive carcinoma. It is often misdiagnosed as eczema of breast and treatment is delayed. Here we present a case where a 30 year old female presented with itching ulceration and destruction of her left nipple. She was treated initially by local physicians by applying local ointments but as her condition did not improve she was admitted to department of surgery Shaheed Suhrawardy Medical College hospital where she was diagnosed as Paget’s disease with infiltrating ductal cell carcinoma. Description of the technique of super-radical mastectomy with removal of axillary, internal mammary, supraclavicular and mediastinal lymph nodes developed at the National Cancer Institute of Milan and previously reported (1). The thoracic defect due to thoracectomy with removal of the clavicle, the medial portion of the first and second ribs and the corresponding half of the sternum is now avoided by sternal replacement and suture. Thus radicality is maintained and cosmetic impairment is confined to breast removal.

Paget’s disease of the nipple is a rare disease with a rather bizarre presentation which makes the diagnosis easily missed. Three cases are presented here with different modes of presentation and histological findings. A biopsy provided the definitive diagnosis in the three patients and the treatments offered was based on the histological findings - modified radical mastectomy and simple mastectomy for invasive carcinoma and carcinoma in-situ respectively.

Background: Perianal Pa get's disease is an uncommon intraepithelial neoplasm that involves areas affluent with apocrine glands, most frequently in the female external genitalia, but may also affect the perianal region. Objective: We present the pathologic and clinical management of a case of Paget's disease involving the perianal region, in which diagnosis was delayed for 8 years. Conclusions: The importance of a high index of suspicion for early diagnosis is emphasized, which should be followed by aggressive surgical treatment with wide excision and local reconstruction.
