Metastatic Multicentric Neurofibrosarcoma of the Lumbosacral Plexus in a Cow

A metastatic multicentric neurofibrosarcoma of the lumbosacral plexus in an adult cow is described. The left lumbosacral plexus was obliterated by a mass which extended through the intervertebral foramen into the spinal canal and between the dorsal arches of the fifth and sixth lumbar vertebrae. A closely associated (possibly contiguous) mass extended into and separated the left sacroiliac joint. Multiple similar masses involved peripheral nerves and skeletal muscles of the pelvis, pelvic limbs, and abdominal wall. Metastatic lesions were scattered throughout the lungs. The lumbosacral lesion and all other masses consisted of interwoven bundles of loosely cohesive, elongated cells separated by variable collagenous matrix. Many neoplastic cells were positive for S-100 protein. Ultrastructurally, fibroblastic cells were mixed with scattered cells possessing schwannian characteristics.

Primary malignant melanoma of the bladder is very rare. Rather than being a primary lesion, malignant melanomas of the bladder are more commonly metastatic lesions. The histopathological appearance largely does not differ from that of melanoma at other body sites. It is often difficult to discriminate whether a bladder melanoma is primary or metastatic. Therefore, a careful review of histological features and performing necessary immunohistochemical staining procedures for S-100 protein and HMB-45 are very important in achieving a correct diagnosis. We report a case of hypomelanotic malignant melanoma of the bladder. Despite the variety of therapies available for primary melanomas of the bladder, the prognosis is still poor.

ABSTRACT This work aimed to describe the origin, distribution, and ramifications of the ischiadicus nerve in the giant anteater and to provide anatomical data which could explain not only the evolutionary aspects but also provide important information for other related works. For the present study, four specimens were used, prepared by perfusion of 10% formaldehyde solution via the femoral artery, for conservation and dissection. The origin of the right and left ischiadicus nerves in the giant anteater from the ventral ramification of the third lumbar (L3) and the first (S1), second (S2), and third (S3) sacral spinal nerves. These nerves were symmetrical in all animals studied. The distribution and ramification occurred to the superficial, middle, and deep gluteal, gemelli, piriform, quadratus femoris, tensor fasciae latae, caudal crural abductor, cranial and caudal parts of the biceps femoris, adductor, semitendinous, and cranial and caudal parts of the semimembranous muscles. Based on the origins of the ischiadicus nerves, there is a caudal migration in the nerve location in animals in a more recent position on the evolutionary scale due to reconfiguration of the lumbosacral plexus, resulting from the increase in a number of lumbar vertebrae. There is no complete homology of the muscle innervation.

AimsIn our previous study, striking Nek9 staining was observed in peripheral nerves for the first time. Therefore, in the current study, we aimed to detect Nek9 expression in peripheral nerve sheath tumours, melanocytic tumours and their mimics.MethodsThe expression of Nek9 was analysed in 234 mesenchymal tumours including schwannoma, neurofibroma, malignant peripheral nerve sheath tumour (MPNST), melanoma and their mimics adopting immunohistochemistry. In addition, S-100 and SOX10 were detected in all tumours.ResultsThe results revealed an intense and diffuse staining of Nek9 in all schwannomas (30/30) and melanomas (20/20). The neurofibromas (86%, 19/22) and MPNSTs (76%, 18/21) showed a high frequency of positive Nek9 staining. Nek9 showed a comparable sensitivity to S-100, and better sensitivity and less specificity than that of SOX10. Among the histological mimics, Nek9 was only strongly and diffusely expressed in rhabdomyosarcomas (RSs) (97%,37/38) while negatively stained in most of the other tumours. It was noted that Nek9 immunoresponse was more diffuse than that of MyoD1 and myogenin in RS.ConclusionsIn summary, Nek9 has a good sensitivity in the diagnosis of tumours with Schwannian, melanocytic and skeletal muscle differentiations. The immunohistochemical analysis of Nek9 expression may be helpful in the diagnosis and differential diagnosis of the aforementioned tumours.

Schwannoma is a benign neurogenic tumor originating from Schwann cells. These produce the myelin sheath that covers peripheral nerves that are often affected. This latter localization is extremely rare, and only a few case reports can be found in the medical literature. Studies have shown that approximately 0.5% to 5% of schwannomas are retroperitoneal, constituting 0.2% of adrenal incidental tumors. These usually present as incidental findings, nonsecreting adrenal masses in asymptomatic patients. Diagnosis of a schwannoma is based on detection of spindle cells with Antoni A and Antoni B regions in histological sections and positive staining for S-100 protein by immunohistochemical analysis. We report a case of an incidentally identified during an abdominal ultrasound examination with schwannoma localized in the left adrenal gland.
