There are two important issues in the postoperative management of the urological elderly patient: complications and rehabilitation. Aging is associated with a loss of homeostatic reserve that may lead to postoperative complications due to drug interaction, malnutrition, infection, defects in wound healing, and poor psychological state. Furthermore rehabilitation, if not properly planned or implemented may influence not only the quality of life, but also the whole treatment results. The authors, in relation to pathogenetic aspects, describe what to do to prevent complications and to promote patient recovery. They conclude that both urologists and nurses must have specific geriatric training and that a psychiatrist, dietician, geriatrician and physiotherapist should be part of the team caring for the elderly surgical patient. Finally discharge from hospital must not be the end of therapy: in selected patients nursing management may continue at home reducing morbidity and mortality and helping better rehabilitation.

O aumento demasiado da população idosa tornou-se uma política de saúde pública, estima-se que até 2020 a perspectiva de vida alcance em torno de 75,5 anos, ou seja, a população será composta por 16,2 milhões de idosos. O objetivo é destacar a importância da atuação do enfermeiro na assistência aliada à segurança do paciente idoso. Trata-se de uma revisão da literatura de forma integrativa, nas bases de dados da SCIELO, LATINDEX, LILACS, REDALYC e sites governamentais (COFEN e COREN-SP). O aumento da população idosa é de ocorrência mundial e requer importância, todavia considera-se que a qualidade de vida pode ser alterada pelo déficit de autossuficiência, o enfermeiro assume papel importantíssimo na gerontologia atuando na promoção da qualidade de vida e possibilita um olhar diferenciado a esse grupo de pessoas. We report a case of buccal abscess caused by an impacted wisdom tooth in an extremely elderly person with malnutrition. The patient was a 94-year-old man, who complained that he had found it hard to open his mouth and that his cheek had been swollen for the previous 2 weeks. He had a shallow oral wound caused by an improperly fitting denture; however, the wound became infected. We performed incisional drainage of the abscess under local anesthesia. The swelling disappeared and he was able to open his mouth 55 mm. The elderly have a high risk of healing failure of injuries and it has been reported that infection in a host in a compromised state is severely intractable. This elderly patient was in a compromised state because of malnutrition. Cases such as this one will increase as the elderly population increases. Dentists need to consider the quality of life of patients with a longer life expectancy and should offer patients several treatment options before their general condition deteriorates.

The biomedical paradigm is so convincing from a biochemical point of view, and highly efficient in many cases of acute medical problems and emergencies, but unfortunately most patients do not get much better only treated with drugs; they need to do something about their lives themselves. It is highly important for the modern physician to understand the strengths and weaknesses of the modern biomedical paradigm, to understand when and when not to administer drugs to their patients. Often a symptom can be eliminated for a while with drugs, but this is not always good as the patient might need to learn to study the imbalances in life that cause the disturbances and symptoms. For the elderly patient, sometimes life can be extended in spite of the subjective fact that life has come to its end. Sometimes treatment with a drug can teach the patient that quality of life is the responsibility of the physician and not the patient. This learned attitude can give the patient problems later or make them less active in helping themselves (responsibility transfer in the wrong direction).This paper gives a number of examples where medical drugs really are the treatment of choice in general practice and some more doubtful examples of using of the biomedical paradigm.

Palliative care is aimed at people with diseases without perspective of cure or terminally, aiming to provide a better quality of life. This study aims to investigating the discourse of nurses about their understanding of palliative care to elderly patient with cancer and identify strategies used by nurses to promote palliative care to the elderly cancer patient. It is an exploratory research of a qualitative nature, carried out with thirteen nurses from a philanthropic institution in the city of João Pessoa, through a questionnaire. The empirical material was subjected to thematic content analysis, resulting in three categories: design of nurses to assist the elderly in Palliative Care: promoting comfort and minimizing the suffering, the importance of palliative care in humanized care to the elderly with cancer and strategies for the Promotion of Care of the Elderly with Cancer. Participants highlighted the palliative care as essential in the humanization of care, ensuring the dignity and quality of life among the elderly with cancer without possibilities of cure, adding such assistance, the family.
