Etomidate Induced Seizure: Adverse Drug Event Case Report

Objective/purpose: To report a case of etomidate induced seizure in a patient that received 0.15 mg/kg of etomidate for a procedural sedation which resolved without intervention. Summary: A 68-year-old woman presented to the emergency department with a dislocated shoulder. Etomidate was given to the patient for procedural sedation to allow the physician to reduce the shoulder dislocation. Upon administration of the medication, the patient began seizing; this lasted for about 2 minutes. No medications were needed for cessation of the seizure. Conclusion: A causality assessment determined that the adverse effect of the epileptogenic action in etomidate was possible. Limited data is available in regard to the incidence rate of this adverse effect.

Abstract Introduction/Objective Nasopharyngeal (NP) swabs have been the traditional specimen source used for testing for respiratory viruses. However, at the start of the COVID-19 pandemic, several studies suggested that saliva could also be used as a specimen source for testing for SARS-CoV-2. Despite potential benefits, there was limited data on the characteristics of this specimen type and few commercial assays with FDA emergency use authorization allowed saliva as a specimen source. In order to explore the feasibility and validate using saliva as a specimen source for ambulatory and emergency department patients we designed a study to compare saliva to NP swabs for SARS-CoV-2 testing. Methods/Case Report Specimens were collected in the emergency department and ambulatory testing sites between May 6, 2020-July 7, 2020. Nasopharyngeal swabs were collected as part of routine clinical practice and patients were given written instructions to self-collect 1mL of saliva into a sterile specimen cup with or without a straw. SARS-CoV-2 testing was performed in parallel with both specimen types using the TaqPath COVID-19 Combo Kit (Thermo Fisher Waltham, MA). Saliva was diluted 1:1 in saline prior to testing. Specimens were transported to the lab at 4C and frozen at -80C prior to testing. Results (if a Case Study enter NA) Seventy-four patients had both an NP swab and saliva tested in this study. Thirty of the 74 patients (41%) were unable to produce the full 1mL of saliva requested, but all samples had sufficient volume for testing after dilution. There were 34 positive samples obtained with an 82% positive agreement between the NP swabs and saliva. In 6 cases, the NP swab was positive, and the paired saliva was negative. In 1 case, only the saliva was positive. The average Ct of the positive NP swabs with a paired negative saliva sample was 39.6. There was only a single invalid test for one of the saliva samples. Conclusion Saliva was a straightforward sample to collect and test for SARS-CoV-2. Challenges included obtaining sufficient sample and a less predictable matrix that required dilution to ensure proper pipeting. In this study, NP swabs were more sensitive for detection of SARS-CoV-2. Paired saliva was more often negative in patients shedding small amounts of SARS-CoV-2 based on a high Ct of the positive NP sample.

Intoxication by orphenadrine is uncommon. The clinical features consist of both central and peripheral anticholinergic effects. Ingestion of 2 to 3 g orphenadrine in an adult has been associated with fatality. A 46-year-old female was brought to our emergency department 1.5 hours after ingesting 40 tablets of 100 mg orphenadrine. She became stuporous 3 hours post-ingestion and developed generalized convulsions 1 hour later. Fifty-five hours post-ingestion, she had recovered and was found to have anterior shoulder dislocation. In addition, severe rhabdomyolysis and persistent nausea were observed. All of the above-noted toxic effects resolved with conservative treatment. Although orphenadrine intoxication can cause convulsions and hemodynamic instability, there is no available antidote and treatment remains supportive.

ABSTRACTBupropion is a newer generation antidepressant that is commonly used for treatment of depression and for smoking cessation. Seizures are a frequently reported adverse effect of bupropion in therapeutic oral doses; however, there are limited data about the consequences of nasal insufflation of bupropion. We report the case of a patient who presented to the emergency department (ED) with a recent history of generalized tonic–clonic seizures whose etiology was initially a diagnostic mystery. After an initial visit to another ED, the patient presented to our ED later that day with a recurrence of the seizures after crushing and nasally insufflating oral bupropion tablets. We review important implications of this case to emergency medicine, including the potential for abuse of bupropion, the difference between intranasal and oral administration, the changing trends in the etiology of drug-related seizures and the importance of examining the nares in patients with unexplained seizure and delirium.

Introduction: This paper compares the usage of inhaled methoxyflurane versus traditional procedural sedation and analgesia for manipulation and reduction of acute shoulder dislocation and acute elbow dislocation in the emergency department. Methods: This was a retrospective observational study of patients who presented with either acute shoulder dislocation or acute elbow dislocation to an adult tertiary emergency department between 1 April 2018 and 30 September 2019 and underwent manipulation and reduction with either methoxyflurane or procedural sedation and analgesia. Primary outcomes of patients’ length of stay in the emergency department and secondary outcomes of duration of procedure and success of reduction on first attempt for inhaled methoxyflurane were compared against those of procedural sedation and analgesia. Results: A total of 192 patients were included in this study; 74 patients underwent reduction with methoxyflurane while 118 patients (85 acute shoulder dislocation and 33 acute elbow dislocation) underwent reduction with procedural sedation and analgesia. The median length of stay in the emergency department was significantly shorter ( P<0.001) for the methoxyflurane group (99 minutes, interquartile range (IQR) 136.8 minutes) versus the procedural sedation and analgesia group (246.5 minutes, IQR 163 minutes). The median duration of procedure in the emergency department was also significantly shorter ( P<0.001) for the methoxyflurane group (16 minutes, IQR 17 minutes) versus the procedural sedation and analgesia group (32 minutes, IQR 40.3 minutes). There was no significant difference in reduction on first attempt between the two groups. Conclusion: The use of inhaled methoxyflurane in the manipulation and reduction of acute shoulder dislocation and acute elbow dislocation was associated with a shorter patient length of stay and a shorter duration of procedure, while no significant difference was observed in the success of reduction on first attempt when compared to procedural sedation and analgesia.

OBJECTIVE: To present a single case of ampicillin-induced recurrent cholestasis and a literature review. CASE SUMMARY: A 23-year-old man was hospitalized due to recurrent and self-limited cholestatic symptoms. He had used ampicillin before each cholestatic attack. He became well clinically and biochemically each time after cessation of the drug. One year after his recovery and discontinuance of ampicillin, the patient has had no recurrence of cholestasis. An objective causality assessment revealed that the adverse drug reaction was probable. DISCUSSION: Ampicillin-related hepatotoxicity is very rare, with injury being mainly hepatocellular. To our knowledge, there is only 1 case report in the literature referring to chronic cholestatic-type hepatotoxicity related to ampicillin. CONCLUSIONS: Ampicillin, which is one of the most widely used antibiotics, may cause recurrent cholestatic hepatitis. Clinicians should be aware of this adverse effect, and it should be kept in mind during diagnostic workup of liver injury.

Levetiracetam, a relatively new antiepileptic drug (AED), is used mainly as adjuvant and less as monotherapy of seizure. Though rare, Levetiracetam is reported to induce hallucination. To highlight the potential of this adverse drug event, we report a seizure-case that had auditory hallucination with Levetiracetam. A 32-year lady had 7-year history of unresponsive spells which increased in the last year, also occurred while asleep and were diagnosed as “generalized seizure” with video-EEG. With gradual optimization of Levetiracetam to 2250 mg, she continuously heard distressing sound of saw cutting wooden blocks. After 3-day continuous auditory hallucination, Levetiracetam had to be changed to sodium valproate.
