Depression Screening in the School Setting: Identification of the Depressed Adolescent

Adolescent depression is a silent epidemic in this country. Untreated depression has detrimental effects on physical health, psychosocial well-being, and academic productivity. It is important for school nurses to be able to recognize depression and refer students promptly for treatment. This article and its associated learning module will provide school nurses with updated information on adolescent depression, discuss barriers in depression screening, use of the PHQ-9 (Patient Health Questionaire-9 Item) as an evidence-based depression screening tool in the educational setting, and the important role of school nurses in depression screening. It is anticipated that by increasing awareness and knowledge about adolescent depression and providing training in the use of an evidence-based screening tool, school nurses will have greater confidence in identifying and referring students in need. BACKGROUND Disease screening identifies a disease in an individual/community at an early stage to prevent or treat the condition effectively. The current COVID-19 pandemic has restricted hospital visits for screening and other healthcare services resulting in the disruption of screening for diseases such as cancer, diabetes and CVD. Smartphone technologies, coupled with built-in sensors and wireless technologies, enable the smartphone to function as a device for disease screening and monitoring with negligible additional costs. OBJECTIVE This review aimed to evaluate the use of smartphone applications (apps) in the disease screening and acceptability of this technology in the medical and healthcare sectors. METHODS We followed a systematic review process to assess the scope for the app in the disease screening process. Four databases (Medline complete, Web of Science, Embase, and Proquest) were searched. Articles published in English and examining the use of the app in disease screening were included. Primary outcomes for the research articles and their statistically significant Results showed that app-based screening group had significant (OR:1.7, 95% CI: 1.2–2.4) eye care utilisation compared to their traditional screening counterparts. A good correlation between clinical Snellen and smartphone visual acuity measurements (ρ=.91) is observed. For depression screening, the ROC curve is .8012, indicating that mental-health ratings are comparable to Patient Health Questionnaire-9 (PHQ-9) results, and could be used as a depression screening tool in practice. Although the findings of cognitive impairment suggest that the digital-version readings are similar to the standard paper-version readings, the participants preferred devices with larger screen (e.g. tablet). Also, the smartphone-compatible oximeter is a weak predictor to detect central sleep apnoea in stable heart failure participants. value, where applicable are presented and discussed. Critical considerations when designing, developing, and deploying smartphone solutions is laid forth to provide equitable healthcare solutions without barriers. Furthermore, the findings might increase the research prospects to evaluate smartphone solutions as valid and reliable screening solutions.

Depression is a frequent sequela of stroke that has been associated with poor recovery and rehabilitation response. Clinical depression may occur within 3 months post stroke and can last for several years if left untreated. Utilization of a depression screen helps identify patients at risk for post-stroke depression. It is important to recognize and treat depression symptoms early to improve patient outcomes. In performing a gap analysis in preparation for our survey for Comprehensive Stroke Center Certification, it became apparent this psychological aspect of patients was not being met. A systemic literature review was performed in search of a depression screening tool that was easy to use and addressed the needs of the patient. The tool chosen is the validated 2-item Patient Health Questionnaire depression module (PHQ-2). This depression screen is also utilized in our rehabilitation center which enhances the communication between our facilities. After educating all the neuroscience nursing staff and stroke physicians, the tool became part of the patient assessment. The nurse screens each stroke patient on admission to the division or transfer from the intensive care unit. If the patient scores 3 or greater, the physician is notified so appropriate follow-up and treatment can occur. If a patient is unable to be assessed, the physician is notified so other depression assessment measures can be considered. The utilization of a depression screening tool along with staff education has increased our staff’s awareness of the potential devastating effects depression can have on stroke recovery.

In 1987, when the U.S. Centers for Disease Control and Prevention recommended the use of universal precautions in response to the HIV/AIDS epidemic, the demand for medical gloves dramatically increased. Unfortunately, the manufacturing techniques for the most widely-used gloves—natural rubber latex—also changed, in order to expedite production. These changes resulted in an increase in latex allergies, because excessive residual proteins that cause allergies remained on the gloves and became airborne with the gloves’ high level of powder. This allergy concern is especially relevant in the school setting where school nurses are not only concerned about their own well-being, but the well-being of the children in their care. Today, latex gloves are still the most widely used of any medical gloves, but they have much lower levels of protein and are available in low-or no-powder. This article examines whether the current latex gloves indeed reduce the allergenicity of the gloves and how allergy and barrier protection data can assist school nurses in making informed glove choices.

Understanding the components that make up a research study facilitates critical analysis and application in school nursing practice. This article describes a straightforward method for analyzing research studies, enabling school nurses to understand and apply current research in their daily practice.

Understanding the unique needs of school nurses is crucial for developing strategies to increase use of evidence-based health care practices in schools. This article describes the development and evaluation of an instrument designed to identify the needs of school nurses and variables associated with use of evidence-based practice in the school setting, that is, the School Nurse Evidence Based Practice Questionnaire (SN-EBP). After questions were drafted, reviewed, pilot-tested, and revised, data were collected from 386 school nurses for psychometric testing. The initial version consisted of 91 content items in five sections. Principal components analysis with promax rotation identified 17 factors in the five sections of the questionnaire, with Cronbach’s alpha coefficients for the sections ranging from .62 to .89. The questionnaire showed good discrimination between known groups on key factors. The SN-EBP was reduced to 69 content items and demonstrated sufficient reliability and validity to warrant further use. It is the only scale available for use in identifying needs and variables associated with use of evidence-based practice in this population.

Primary and acute care settings are the focus of a concerted effort to implement evidence-based practice (EBP) in health care; yet, little attention has been given to use of EBP among school nurses. The aims of this study were to (a) describe current use of EBP among school nurses attending a national school nurse conference, (b) describe demographic, individual, and organizational factors associated with EBP use, and (c) identify resources needed to enhance EBP use. A survey designed for this study was distributed during a national school nurse conference. Descriptive statistics, correlations, and independent t tests were used to analyze respondent data regarding the current level of EBP use, awareness, skills, and information sources. Respondents also identified resources needed to increase use of EBP, which included networking opportunities, predeveloped EBP guidelines, and education on outcome evaluation. The results from this study will be used to develop strategies to increase the use of EBP in the school setting.
