The period since the war has seen a vast increase in the amount of orthodontic treatment provided for children in the United Kingdom. This is currently given by practitioners within the general dental service, orthodontists in the former local authority dental service and by consultants and their teams in the hospitals. The training programme for consultants has been evolved but there is a need for appropriate training programmes for other orthodontists. While we have reason to be proud of the present situation, there is room for improvement.

This paper has been written to stimulate discussion on the future development of orthodontic treatment and teaching in the United Kingdom. It is the author's opinion that we are standing at a crossroads and ought, therefore, to choose between two pathways. In the paper the present situation in relation to teaching and treatment is summarized, and the two alternatives discussed.

Introduction Most evaluations of field epidemiology training programmes (FETP) are limited to process measures, but stakeholders may need evidence of impact. Objective To ascertain if the United Kingdom (UK) FETP met its objectives to: (i) strengthen capacity and provision of national epidemiology services, (ii) develop a network of highly skilled field epidemiologists with a shared sense of purpose working to common standards and (iii) raise the profile of field epidemiology through embedding it into everyday health protection practice. Methods The evaluation consisted of: (i) focus groups with training site staff, (ii) individual interviews with stakeholders and (iii) an online survey of FETP fellows and graduates. Findings were synthesised and triangulated across the three evaluation components to identify cross-cutting themes and subthemes. Findings Eight focus groups were undertaken with 38 staff, ten stakeholders were interviewed and 28 (76%) graduates and fellows responded to the survey. Three themes emerged: confidence, application and rigour. FETP was perceived to have contributed to the development, directly and indirectly, of a skilled workforce in field epidemiology, increasing stakeholders’ confidence in the service. Graduates applied their learning in practice, collaborating with a wide range of disciplines. Fellows and graduates demonstrated rigour by introducing innovations, supporting service improvements and helping supervisors maintain their skills and share good practice. Conclusion The UK FETP appears to have met its three key objectives, and also had wider organisational impact. FETPs should systematically and prospectively collect information on how they have influenced changes to field epidemiology practice.

Traffic separation schemes and other routing measures have now been established in the coastal waters of many countries and new schemes are being introduced each year. Traffic separation was originally intended to reduce the risk of collision between ships proceeding in opposite directions but this paper explains how routing measures are now being used mainly for coastal protection. Improvements in navigational aids may lead to more extensive routing schemes in the future with increasing restriction on the movement of shipping.The first traffic separation schemes adopted by IMCO (now IMO) in 1965 and 1968 were based on proposals made by the Institutes of Navigation of France, the Federal German Republic and the United Kingdom. In the report submitted to the Organisation by the Institutes in 1964 it was stated that ‘the object of any form of routing is to ease the congestion and lessen the likelihood of end-on encounters by separating opposing streams of traffic …’.

Objectives: The aim of this study was to describe generally the development and present situation with health technology assessment (HTA) in the United Kingdom.Methods: The methods used are a review of important materials that have described the development process and present situation, supplemented by some personal experiences.Results: The United Kingdom has been characterized historically as a country with a strong interest in evidence in health care, both clinical trials for efficacy and cost-effectiveness analyses. However, this evidence was not well-linked to the needs of the National Health Services (NHS) before formation of the NHS R&D Programme in 1991, The R&D Programme brought substantial resources into HTA and related activities, with the central aim of improving health care in Britain and increasing value for money. However, policy makers as well as staff of the R&D Programme were dissatisfied with the use of the HTA results in clinical and administrative practice. Therefore, the National Institute of Clinical Excellence (NICE) was formed in 1999. NICE issues guidance intended to influence practical decision making in health care at the national and local levels, based on efficacy information and, in some cases, economic analyses. NICE is now also seeking ways to maximize impacts on practice.Conclusions: The UK experience shows that information on clinical and cost-effectiveness may not be enough to change practice, at least in the short-run. Still, one may conclude that the United Kingdom now has one of the few most important and influential HTA programs in the world.

Several studies, in recent years, have demonstrated what has become known as the ‘quantitative deficit’ in UK sociology. This deficit is primarily manifested through negative student attitudes towards quantitative methods, a lack of ability in that area and a paucity of quantitative research and publication in the discipline that utilises quantitative methods. While we acknowledge the existence of that deficit, we argue in this article, and present some initial evidence in support of this argument, that the issue is not simply just about a ‘crisis of number’ but the kind of sociology taught and practised in the United Kingdom. We suggest here that there are two broad categories of sociology that do not necessarily divide along quantitative–qualitative lines, which we term ‘analytic’ and ‘critique’. Much of UK sociology takes a ‘critique’ approach, which may well be a quite legitimate way to do sociology, but is not a sufficient basis on which quantitative sociology can be done and has implications for the future of the discipline.
