Abstract Introduction Although splenectomy is still considered as the most effective curative treatment for primary immune thrombocytopenia (ITP), its use has significantly declined in the last decade, especially since the emergence of thrombopoietin receptor agonist (TPO-RAs) and anti-CD20 monoclonal antibodies 1-3. The main objective of our study was to evaluate if splenectomy was still as effective in the modern era, particularly for patients who failed to respond to TPO-RAs and rituximab. One of the secondary objectives was to assess, among patients who did not respond to or relapse after splenectomy, the pattern of response to subsequent intervention with treatments used before splenectomy and particularly TPO-RAs. Methods This multicentre retrospective observational study involved adults who underwent surgical splenectomy for primary ITP in France from 2011 (authorization of TPO-RAs in France) to 2020. To be included in the study, patients had to fulfil the following criteria : age ³18 years, primary ITP diagnosis defined according to the usual international criteria 2. Patients with abnormal spleen histology (other than reactional lymphoid hyperplasia, white-pulp hypoplasia or red pulp hyperplasia) or yet definite secondary ITP were excluded. Response was defined according to international criteria 4. Sustained response was defined as the absence of ITP relapse at last visit. We performed univariable and multivariable logistic regression procedures to calculates the odds ratio associated with a sustained response. Results In total,185 patients, 98 (53 %) women, with median age at splenectomy of 43.3 [interquartile range 27.6-64.3] years, were included in 18 French university and general hospitals from the French reference center network. Most of the patients were splenectomised at the chronic phase of ITP (n=150, 81.1%) and only two patients had undergone surgery within 3 months after ITP onset. Of note, 100 (54.1%) and 135 (73.0%) patients received TPO-RAs and/or rituximab prior to the splenectomy, respectively. The median time of follow-up after splenectomy was 39.2 months [16.5-63.0]. Overall, 144 (77.8%) of patients had an initial response and 23 patients (12.4%) relapsed during follow-up leading to an overall rate of sustained response of 65.4%, similar to the one observed in the pre-TPO-RA's era 1. Characteristics of patients according to the period during which occurred the splenectomy is available in Table 1. Among the 14 patients who failed to respond to both eltrombopag and romiplostim prior to splenectomy a sustained response after splenectomy was observed in 7 (50%). Among the 13 patients who had failed after both TPO-RAs and rituximab, we observed a sustained response in 6 (46%). In the multivariate analysis, an older age (60-75 years: OR 0,39 [0,17-0,86], p=0.02; >75 years: OR 0,28 [0,10-0,75], p=0.013) and a history of more than 4 treatment lines for ITP before splenectomy (OR 0.25 [0.08-0.66], p=0.010) were significantly associated with a lack of sustained response after splenectomy. TPO-RAs were used for 57/64 (89.1%) patients who failed to respond to splenectomy. Among them, 21 were treated with one TPO-RA (i.e. eltrombopag or romiplostim) which was previously used before splenectomy without any efficacy and a response was observed in 13 (62%) of them. Conclusions In conclusion, splenectomy seems to be still a relevant option for treating adult primary ITP not responding to TPO-RAs and rituximab. Abstract Immune thrombocytopenia (ITP) is a rare condition characterized by thrombocytopenia and variable bleeding severity. After acute management with steriods and intravenous immunoglobulin (IVIG), the next line therapies include medical treatments: immunosuppressive therapy such as rituximab, mycophenolate mofetil (MMF), thrombopoietin receptor agonists or surgical treatments. The choice of therapy depends on the time course of the disease, severity, patient choice and comorbidities. The evidence for comparative efficacy between different treatment modalities is lacking in the literature, in particular the immunosuppressive therapies MMF and rituximab. The UK ITP registry is a national registry of primary ITP, capturing demographics, clinical features, treatments and comorbidities. As of July 2021, it has 4402 participants from 90 acute NHS Trusts across the UK. We analyzed data from the UK adult ITP registry to assess responses to rituximab and MMF therapy. All patients entered into the registry from 1st January 2010 were included in this analysis. Internationally recognized definitions of response based on platelet count were used: partial response (PR) platelets >30 x10 9/L and double baseline, complete response (CR) platelets >100 x10 9/L; No response - absence of CR and PR. In addition, we reviewed use of rescue therapy and use of additional therapies following treatment. A total of 844 patients were included in the analysis, of these 486 had received rituximab and 358 had received MMF since 2010. The median follow up before treatment was 230.5 weeks for rituximab and 195 weeks for MMF. At the time of commencing rituximab (and within 4 weeks of starting), 26.1% patients were receiving prednisolone, 10% IVIG, 3% dexamethasone, 3.2% MMF, 2.95% romiplostim and 2.36% eltrombopag. At the time of commencing MMF (and within 4 weeks of starting), 39.2% patients were receiving prednisolone, 12.74 IVIG, 3.86% dexamethasone, 3.6 rituximab, 5.6% romiplostim and 3.86% eltrombopag. The total number of patients achieving platelet response at each time response are listed in table 1. Of those receiving rituximab with baseline platelets <30 x10 9/L at starting therapy, 40% had at least PR by 4 weeks, 50% at 12 weeks and 30% at 12 months. For MMF, these percentages were 30%, 30% and 18% respectively, however, the numbers of patients in the MMF group were lower. The median duration of response was 44 weeks for rituximab and 27 weeks for MMF. Where new therapies were required, the median number of weeks until therapy was switched was 18 weeks for rituximab and 15 weeks for MMF. 77% patients and 80.7% patients who received rituximab and MMF respectively changed treatment during their follow up. Overall 284 (58.4%) patients received rescue medication during follow up, of those that received MMF 234 (65.36%) received rescue medication. The main rescue therapies were prednisolone (589 episodes in rituximab group and 520 in MMF group) and IVIG (482 in rituximab group and 321 in MMF group). In conclusion, this is one of the first studies, using real world data and long term follow up, to compare outcomes after immunosuppression. Both rituximab and MMF are often administered with additional therapies at the outset, predominantly steroids and IVIG, but also other therapies such as the thrombopoietin receptor agonists. The use of rescue therapies during follow up, suggesting treatment failure or a need to augment the response, is common with most patients in both groups ultimately having treatment change during follow up. Extension of this study to include other therapies and identifying groups most likely to respond will support optimization of treatment for patients with ITP. Figure 1 Figure 1. Abstract Introduction: Immune thrombocytopenia (ITP) is an autoimmune disease due to peripheral destruction but also impaired central production of platelets. Autoimmune reaction directed against megakaryocytes (MKs) has been described, and may explain morphological abnormalities of MKs observed in some patients with primary ITP. Thrombopoietin receptor agonists (TPO-RAs) are indicated as second-line treatments for ITP, but no predictive factors of response used in clinical routine practice has been demonstrated. The utility of systematic bone marrow smears (BMS) at ITP diagnosis is discussed. Howerer, it is usually recommended before second-line treatments. Two studies have suggested an association between MK abnormalities and response to corticosteroids in primary ITP, but none have investigated this association for TPO-RAs. This study aimed to investigate the association between MK abnormalities and response to TPO-RAs in adult patients with primary ITP. Methods: The source of population was the CARMEN registry. The CARMEN (Cytopénies Auto-immunes: Registre Midi-PyréneEN) registry is aimed at the prospective follow-up of all incident ITP adults in the French Midi-Pyrénées region (South-West of France, 3 million inhabitants) since June 2013. Each investigator follows all adult patients (aged ≥18 years) with incident ITP in routine visit or hospital stay. ITP was defined by international definition (platelet count <100 x 10 9/L and exclusion of other causes of thrombocytopenia). The study population consisted in all patients included in the CARMEN registry between June 2013 and March 2018 with primary ITP, treated by TPO-RA and with a BMS before initiating TPO-RA. We excluded the patients with a number of MKs <10 MK on the BMS. Morphological abnormalities were established based on literature and defined by consensus among 3 expert cytologists (AR, JBR and VDM). All MKs present on each smear were analyzed. MKs were categorized by the presence of dysplasia (monolobed MK and/or separated nuclei and/or microMKs), and according to their stage of maturation (basophilic, granular and thrombocytogenic). All patients' medical charts were reviewed by two experts in ITP (OW and GM) to determine the response to TPO-RAs. Response was defined by a platelet count between 30 and 100 G/L with at least a doubling of basal platelet count according to the international definition. In case of subsequent exposure to both TPORAs in a single patient, response was defined by response to at least one TPO-RA in the main analysis. We performed a subgroup analysis by TPORAs. Results: During the study period, 451 patients with incident ITP were included in CARMEN-registry. Among them, 105 had been treated by TPO-RAs, including 65 with BMS before the exposure to TPORA. We then excluded 20 patients with secondary ITP and 7 with less than 10 MKs on the BMS. We finally included 38 patients in the analysis. Median age at diagnosis was 71 years (interquartile range - IQR: 31 - 94) and 34.2% were women. Thirty-three patients were treated with eltrombopag, 17 with romiplostim including 13 who were exposed to both TPORAs. Thirty-four (89.4%) achieved response. The median number of MKs analyzed per patient was 137 (IQR: 50 - 265). All results are presented in Table 1. In the main analysis, there was no significant difference in the median percentage of dysplastic MKs in responders (4.0%, 95% confidence interval - CI: 2.3 - 6.4) and non-responders (4.5%, 95% CI: 0.7 - 7.1). There was a trend for a higher proportion of granular MKs (4.5%, 95% CI: 3 - 6) and basophilic MKs (30.1%, 95% CI: 21.9 - 39.1) in non-responders comparing to responders (granular: 2.0%, 95% CI: 0 - 4.1; basophilic: 21.3%, 95% CI: 11.4 - 40.7). Results were similar in the subgroup of patients treated with eltrombopag (data not shown; the low number of patients treated with romiplostim precluded any analysis). Conclusion: In this study, neither MK abnormalities nor the pattern of MK maturation stages were significantly associated with response to TPO-RAs. These results do not support a systematic bone marrow smear in patients with primary ITP to look for morphological predictive factors of response to TPO-RA. Figure 1 Figure 1. Abstract Background Immune Thrombocytopenia (ITP) is a bleeding disorder due to a combination of increased platelet destruction and reduced production, often secondary to anti-platelet/megakaryocyte antibodies. The presence of antibodies to glycoproteins (GP) IIb/IIIa (integrin αIIbβ3) and GPIb/IX, detected in majority of ITP patients, may correspond to different responses to treatment, i.e., anti-GPIb is associated with more severe disease, and less responsive to intravenous immunoglobulins and steroids. Thrombopoietin Receptor Agonists (TPO-RA) increase platelet production by stimulation of megakaryopoesis. Predictors of response to TPO-RA and influence of antibody profile on response are currently unknown. In our previous study we investigated Absolute Immature Platelet Fraction (A-IPF) prior to TPO-RA treatment and did not find a correlation between A-IPF, anti-GP antibodies, and platelet counts. The aims of this study were to further investigate: 1. The role of anti-GP antibodies in response to TPO-RA; 2. Effect of patients' antibodies on megakaryocyte (MK) viability, maturation, apoptosis and formation of proplatelets (in vitro); 3. The influence of patients' clinical characteristics on response to TPO-RA. Materials and Methods 91 patients with persistent or chronic ITP, were treated at Weill Medical College of Cornell University until January 2015 with TPO-RAs: 52 patients received eltrombopag, 22 romiplostim and 17 avatrombopag. Serum samples of 84 patients were analyzed for the presence of anti-GP by MAIPA assay as previously described. Patients with baseline platelet counts less than <30x109/L were defined as responders to TPO-RA if the average of their six median monthly platelet counts was ≥50x109/L and doubled from average baseline counts (prior to TPO-RA). Patients with baseline platelet counts 30-50x109/L were responders if the average platelet count was ≥75x109/L. MKs were derived from human umbilical cord blood stem cells as previously described. Cells were grown using SFEM medium, adding on day 0 of culture 50 ng/ml recombinant TPO and aliquots of serum of ITP patients or healthy controls. The percentages of immature (CD41+/CD42-), mature (CD41+/CD42+), viable and apoptotic MKs were analyzed by flow cytometry on day 12. Apoptosis was analyzed by measuring Mitochondrial Outer Membane Potential (MOMP) and Phosphatidyl Serine (PS) externalization. MKs were considered apoptotic if they had positive staining for PS externalization, viable if positive for MOMP, and dead if positive for 7-Aminoactinomycin D (7AAD). Proplatelet formation by MKs was analyzed by microscopy. Statistical analysis using unpaired T-test and Pearson correlation test were performed. Results Ninety-one patients were included, 40 male (44%) and 51 female (56%), with a median age of 37.4 years (range 2-87). Median duration of ITP before TPO-RA treatment was 8 years (range 0.3-45). The 18/91 (19.8%) non-responders to TPO-RA were not different from the 73/91 responders in age, gender, number of prior treatments, duration of ITP, and past splenectomy. The presence of either or both anti-GP antibodies was correlated with average lower platelet counts on TPO-RA: 82.3 x109/L versus 123x109/L in patients without detected antibodies ("neither") (p=0.003). However, the response to TPO-RA was not influenced by the type of antibody: in patients with anti-GPIb the average platelet count was 76.1x109/L, and with anti-GPIIb/IIIa 80.7x109/L (Figure 1). In culture, excess dead MKs were found in anti-GPIb group and antiGPIb&antiGPIIb/IIIa group compared to "neither" group (p=0.0013 and p=0.027 respectively) and comparing antiGPIb&antiGPIIb/IIIa to control (p=0.0025). We did not observe changes in the degree of MK apoptisis or in MK maturation in the presence of serum antibodies. In cultures treated with serum of patients having anti-GPIb, less proplatelets were detected comparing to control (p=0.044) or to "neither" (p=0.0039). We conclude that patients with anti-GP antibodies respond less to TPO-RA, however there is no difference in response to TPO-RA between patients having anti-GPIb and anti-GPIIb/IIIa, unlike responses to other treatment modalities (e.g., steroids or immunoglobulins). TPO-RA could be a preferable treatment option in ITP patients having anti-GPIb. Figure 1. Abstract Abstract 392 Introduction Primary immune thrombocytopenia (ITP) is a common autoimmune bleeding disorder characterized by low platelet counts and an increased risk of bleeding. Recent consensus recommendations on ITP management (Provan, Blood 2010) emphasize the need for individualized treatment strategies based on patient preference; however little is known about which treatments patients prefer and why. Although splenectomy is most likely to induce a durable remission, uptake of splenectomy by patients and physicians is variable and a general tendency towards splenectomy avoidance has recently been observed. The objective of this study was to better understand patient preference and the factors affecting patients' decision for or against splenectomy. Methods We designed an exploratory qualitative interview study. Criterion sampling was used to identify eligible patients 18 years of age or older who were diagnosed with relapsed (lasting 3 – 12 months) or chronic (lasting longer than 12 months) primary ITP and who had been offered splenectomy as a treatment option by their physician, until data saturation was achieved. One to one, semi-structured interviews were conducted using an open-ended interview guide designed to investigate factors impacting splenectomy decision-making. Interview transcripts were coded independently in triplicate and interrater agreement was high. Major themes were identified from the data using a team analytic approach and audit trail. Results. Data saturation was achieved after 15 patients were interviewed; 6 were for splenectomy, 7 were against, and 2 were undecided. Patients were between the ages of 19 and 82 [median 43 years; interquartile range (IQR), 31 – 61] and 9 (60%) were female. Median duration of ITP was 49 months (IQR 13 – 113); patients had received a median of 2 prior treatments (IQR 2 – 3) and median platelet count at the time of the interview was 72 × 109/L (IQR 29 – 106). Four major themes were identified from the data about influences on treatment preferences: 1) patients' understanding of the ITP disease process; 2) patients' perception of the impact of ITP on their quality of life; 3) patients' understanding of the risks and benefits of treatments offered by their physician; and 4) patients' perception of splenectomy as a last resort. Patients were likely to accept splenectomy if their disease was perceived as having a negative impact on their quality of life. In general, patients had limited understanding of the cause of ITP and often misinterpreted the meaning of quoted probabilities of success with splenectomy. Conclusion Increased awareness of influences on patient treatment preferences will help physicians guide ITP patients through the complex decision-making process regarding splenectomy and can inform the design of decision-aids. Abstract Abstract 4667 Introduction: Primary immune thrombocytopenia (ITP) is an autoimmune disorder characterized by a low platelet count and an increased risk of bleeding. Hemostatic treatment modalities, bypassing the need for platelet transfusion, would be desirable for control of serious acute bleeds in patients with ITP. This study aimed at (i) performing a thorough characterization of the coagulopathy of ITP, (ii) investigate new ways to obtain acute correction of the coagulopathy performing in vitro experiments with recombinant factor VIIa (rFVIIa, NovoSeven®), fibrinogen (RiaSTAP®), and the combination of rFVIIa and fibrinogen, and finally (iii) investigate the correlation of the hemostatic response to the baseline platelet counts of the ITP patients. We challenged the hypothesis that rFVIIa combined with fibrinogen concentrate can correct whole blood (WB) clot formation in patients suffering from ITP even at very low platelet counts. Methods: Blood from 12 ITP patients and 15 healthy controls was drawn into 3.2% citrate containing corn trypsin inhibitor 18.3μg mL−1 to inhibit artificial contact activation. The WB (mean platelet count 22 × 109L−1 (range 0–42)) was spiked in vitro with buffer (control), fresh donor platelets (+40×109 L−1), rFVIIa (1 or 4μg mL−1), or fibrinogen (1 or 3mg mL−1) as well as the combination of rFVIIa and fibrinogen. Dynamic WB coagulation profiles were recorded by ROTEM® Thromboelastometry using activation with tissue factor (0.03pM) and re-calcification. Parameters of clot initiation (clotting time, CT), clot propagation (maximum velocity, MaxVel) as well as clot termination (maximum clot firmness, MCF) were evaluated. Thrombin generation in “platelet-rich” ITP plasma was evaluated using calibrated automated thrombography. Overall differences between groups were evaluated by paired and unpaired t-tests as appropriate. Simple linear regression analyses were performed using the differences observed after addition of the various interventions (intervention – baseline) as the dependent variable (y) and the platelet count as the independent variable (x). The slope was used to evaluate dependency of the hemostatic response on the platelet count, whereas the intercept was used to evaluate the hemostatic response at very low platelet counts. A p-value less than 0.05 was considered statistically significant. Results: Compared with healthy controls the WB coagulation profiles of the ITP patients were characterized by a prolonged CT (mean: 1490 vs. 941s, p<0.001) as well as a markedly reduced MaxVel (3.4 vs. 9.7mm×100s−1, p<0.001) and MCF (38.2 vs. 49.4mm, p=0.01). Fibrinogen showed no positive hemostatic effect. Recombinant FVIIa reduced the CT (744s, p<0.001) and increased the MaxVel (6.28mm×100s−1, p<0.001) whereas no change was observed in the MCF. Thrombin generation in “platelet-rich” plasma supported the findings in WB. The improvement in CT following addition of rFVIIa was independent of the platelet count (p-values > 0.45) and the intercept showed a significant improvement at very low platelet counts (1μg mL−1: −643s, p<0.001; 4μg mL−1: −811s, p<0.001). In contrast, the increase in MaxVel after addition of rFVIIa was highly dependent on the platelet count (1μg mL−1: R2 = 0.81, p < 0.001; 4μg mL−1: R2 = 0.86, p < 0.001) and the intercept was not significant (1μg mL−1: 0.05mm×100s−1 p=0.87; 4μg mL−1: 0.54 mm×100s−1 p=0.15). The combination of fibrinogen and rFVIIa revealed a synergistic effect also showing an increased MCF (50.7mm) in addition to a reduced CT (794s) and improved MaxVel (7.9 mm×100s−1) displaying larger effects than following fresh donor platelet substitution (CT 1164s; MaxVel 6.96mm×100s−1; MCF 49.6mm). Furthermore, rFVIIa together with fibrinogen also showed a significant response at very low platelet counts in all parameters (Intercept: CT −788s, MaxVel 3.3mm×100s−1, MCF 13.9mm, p-values<0.004) Conclusions: Data suggest that rFVIIa combined with fibrinogen can correct the coagulopathy of ITP even at very low platelet counts, and may be an alternative to platelet transfusion. Clinical trials are needed to further investigate if this new treatment modality holds the potential to serve as an effective acute treatment option in ITP. Abstract Background Rituximab, a monoclonal anti-CD20 antibody is commonly used to treat immune thrombocytopenia (ITP). Results of randomized controlled trials (RCTs) evaluating the efficacy of rituximab are conflicting. We conducted a systematic review and meta-analysis of RCTs to determine a more precise estimate of the effect of rituximab on platelet count response in adults with ITP. Methods We searched MEDLINE (from 1946), EMBASE (from 1980), and the Cochrane database using the MeSH terms antibodies, monoclonal, and purpura thrombocytopenia idiopathic and the textwords rituximab, rituxan, mabthera, and immune thrombocytopenic purpura. In duplicate, two reviewers independently assessed study eligibility, abstracted data and assessed each study for methodological quality. Results We identified 4 RCTs (n=360) that met our eligibility criteria. Each trial compared rituximab to placebo combined with other ITP treatments, including dexamethasone, or standard of care. Each trial enrolled non-splenectomized patients only. The likelihood of achieving a platelet count >100 x109/L at 6 months was greater with rituximab than placebo (relative risk [RR] 1.38, 95% CI 1.08-1.76). More patients receiving rituximab achieved a platelet count greater than 50 x109/L at 6 months (RR 1.46, 95% CI 1.18-1.80) compared to placebo. Rituximab was not associated with a reduction in the risk of any bleeding (RR 1.49, 95% CI 0.55-4.04) or an increase in the risk of infection (RR 1.33, 95% CI 0.74-2.38). Conclusions Rituximab is associated with a modest increase in the likelihood of achieving a platelet count greater than >100 x109/L at 6 months compared to placebo. No significant reduction in bleeding or increased risk of infection was observed at 6 months. Randomized trials were generally small, with relatively short follow-up. Large pragmatic multicenter comparative trials are needed to examine durability of response over a longer period of follow-up. Abstract Introduction: Immune ThrombocytoPaenia (ITP) in children consists of an isolated thrombocytopaenia (platelet count less than 100 x 109/L) in the absence of any other identifiable cause (Rodeghiero 2009) and is a diagnosis of exclusion. The majority of children diagnosed with ITP remit in the first few months with approximately 30% going on to develop chronic ITP (Thrombocytopaenia persisting > 12 months) (Imbach 2006). The majority of children with chronic ITP are managed supportively and do not require treatment. However, clinically significant bleeding may occur spontaneously or when challenged (e.g. infection, surgery, trauma). For patients who require frequent breakthrough treatment or in whom lifestyle restrictions are unacceptable, the treatment options are limited and the side effects often unacceptable to both families and physicians Initial study reports suggest that thrombopoietin receptor agonists (TPO-RA) are effective and safe in paediatric patients, however longer term follow up data is unavailable Here we present follow-up data on the efficacy and safety on the use of TPO-RA in 34 paediatric patients with Chronic ITP across 5 tertiary referral centres with up to 5 years follow up. Methods: A retrospective case note review was performed for paediatric patients with Chronic ITP who were treated with off-licence TPO-RA in 5 tertiary referral centres. Patients were initially treated on clinical study (PETIT n=25, PETIT2 n=2, or off study n=7). Results: 34 patients (Female n=14, Male n=20) were identified who were treated with TPO-RA between 2009 and 2014. Median age when starting TPO-RA was 10 years of age (Range 2 to 17 years). 70% of patients were White, 12% Asian, 9% Arabic, 6% Hispanic and 3% Mixed ethnicity. Prior to starting TPO-RA all 34 patients had received at least one prior ITP therapy, with 41% receiving ≥3 previous ITP therapies (n=14). 71% had received steroids (n=24), 68% IVIG (n=23), 50% Rituximab (n=17), 27% Anti-D (n=9), 3% steroid sparing agents (n=1) and 3% had undergone splenectomy (n=1). 21% were on concurrent platelet rising therapy when starting TPO-RA (n=7). 97% started on TPO-RA for bleeding and/or thrombocytopaenia (n=33) and 3% started on TPO-RA for surgery (n=1). During this period patients received treatment with Eltrombopag (n=28), Romiplostim (n=5) or both (n=1). Median duration of treatment with Eltrombopag was 30 months (range 6 to 55 months) and Romiplostim was 13 months (range 1 to 32 months). 32 patients were treated for ≥6 months, 28 for ≥1 year, 20 for ≥2 years and 4 for ≥4 years. Median maintenance dose for Eltrombopag was 0.94mg/kg and Romiplostim 7.92micrograms/kg). 53% of patients are still currently taking a TPO-RA (n=18). 1 patient who failed to respond to Eltrombopag, responded to Romiplostim. 50% of patients had a trial off treatment (n=17), with 4 patients remaining off treatment (Eltrombopag n=3, Romiplostim n=1). 4 patients stopped treatment due to the fear of potential side effects, 3 patients stopped treatment due to compliance issues, 4 patients stopped due to a lack of response to treatment and treatment was withdrawn in 1 patient due to subclinical (<grade 1) lens Opacities. 8 patients underwent bone marrow biopsy before and during/after treatment with TPO-RA. Of these 5 showed an increased reticulin grade after starting TPO-RA (Maximally Grade 1 to 2). Conclusion: Our experience demonstrates that both Eltrombopag and Romiplostim are an effective treatment option in paediatric patients with chronic ITP. Both Eltrombopag and Romiplostim were well tolerated with exposures over 5 and 2 years respectively. 4 patients remain off treatment after a treatment ‘holiday’. Therefore withdrawal of treatment should be considered in all patients who are stable on treatment. No new safety concerns were identified but we would advocate continued surveillance. Abstract Abstract 2406 Poster Board II-383 Introduction Primary immune thrombocytopenia (ITP) is an autoimmune disease characterized by elevated reticuloendothelial destruction and suboptimal megakaryocytic production of platelets. Although a common second-line treatment eliciting a response among two-thirds of patients with primary ITP, splenectomy remains controversial owing to its long-term failure rate and risks of surgical sequelae and opportunistic infection. The pattern of platelet sequestration may be a possible predictor of response to splenectomy in patients with primary ITP. Objectives To evaluate the utility of autologous 111In-labeled sequestration studies in predicting the short- and long-term outcomes of splenectomy in patients with primary ITP who have failed at least two treatment modalities or who require permanent administration of high doses of corticosteroids. Methodology The investigation comprised patients diagnosed with primary ITP undergoing an autologous 111In-labeled platelet sequestration study prior to possible splenectomy at Barts & The London NHS Trust between March 1994 and May 2007. Patients were required to have platelet counts of 5-100×109/L; no antibody therapies were permitted four weeks prior to the study. 111In platelet-labeling was performed in accordance with recommendations of The International Committee for Standardization in Hematology (J Nucl Med 1988). At 0.5, 3, 24, and 48 hours post-injection, blood samples (5 mL) were taken and surface counting performed, with the gamma camera set to register 111In-photon peaks at 171 and 245 KeV. Surface counting, corrected for background and decay, was performed over the heart, liver, and spleen using the geometric mean of anterior and posterior views. Regions of interest were drawn over these organs using the 0.5 hour acquisition and transferred to subsequent acquisitions to ensure analysis of identical anatomical areas. A graph of platelet-associated radioactivity versus time was constructed to derive the platelet half-clearance time (t50%), which was used in the algorithm, 1.44× t50%, to compute mean platelet lifespan (MPLS). To assess the degree of splenic sequestration, the relative increase in the spleen:liver (S:L) ratio from 0.5 hours to 80% platelet destruction was calculated. An S:L ratio increase, X > 2.0 was classified as a purely splenic pattern, 1.4 < X ≤ 2.0 as predominantly splenic, 0.8 < X ≤ 1.4 as mixed, and X ≤ 0.8 as hepatic (Najean et al., Brit J Haem 1992). At least one-year following the studies, hematologists were questioned as to whether patients underwent splenectomy and asked to provide 1-3 month and 6-12 month post-surgery platelet counts as well as platelet count and ITP-specific treatment status at most recent follow-up if they did. Surgical failure was defined as a platelet count less than 100×109/L or reliance on an ITP-specific treatment [most recent follow-up only]). The odds ratio of failure for patients with hepatic and mixed patterns versus purely splenic and predominantly splenic patterns were calculated at 1-3 and 6-12 months post-surgery and most recent follow-up using logistic regression. Covariates included age at surgery, gender, and post-surgical follow-up time (most recent follow-up only). Results Between March 1994 and May 2007, 232 patients with primary ITP took part in autologous 111In-labeled platelet sequestration studies, of which 82 (35.3%) patients, 74 adults (≥ 16 years at surgery) and 8 children, underwent splenectomy. Among the splenectomized cohort, median age at surgery was 29.3 years (range: 7.8-67.5 years). Purely and predominantly splenic patterns of sequestration were noted in 64 (78.0%) patients while mixed and hepatic patterns were seen in 18 (22.0%) patients. As illustrated in Table 1, patients constituting the latter group exhibited statistically significant, increasing odds ratio of failure at each follow-up interval. Conclusion This investigation provides evidence of an increased risk of failure from splenectomy in primary ITP patients with mixed and hepatic patterns of platelet sequestration. Background Either allogeneic hematopoietic stem cell transplantation (SCT) from HLA-matched sibling donor or immunosuppressive therapy (IST) has been recommended as one of the standard treatments for severe aplastic anemia (SAA). Regarding only 30% of chance finding HLA‐matched sibling donor, SCT from an alternative donor including unrelated (URD) or haplo-identical related donor (HAPLO) is considered to be a treatment option after failure to IST in patients who lack of a HLA-matched sibling donor. The aim of this study was to compare the outcomes of URD SCT and HAPLO SCT for SAA patients. Method Consecutive 152 adult patients with SAA who received first SCT between March 2002 and May 2018 were included: 73 of HLA-well-matched (8/8) URD (WM-URD), 34 of HLA-mismatched URD (MM-URD), and 45 of HAPLO. With the intention to have a follow-up period at least 1 year, data were analyzed at May 2019. A conditioning regimen with total body irradiation (TBI) and cyclophosphamide was used for URD-SCT, whereas that with TBI and fludarabine was administered for HAPLO-SCT (Lee et al, BBMT 2011;17:101, Park et al, BBMT 2017;23:1498, Lee et al, Am J Hematol 2018;93:1368). The combination of tacrolimus and methotrexate were used as graft-versus-host disease (GVHD) prophylaxis. Results The median follow-up was 53.4 (range, 0.2-174.1) months. The median age of URD and HAPLO cohort was 30 (range 18-59) and 34 (range 18-59) years, respectively. Except for one and three patients who failed respective a neutrophil and platelet engraftment, other patients achieved neutrophil and platelet engraftments with median 11 and 15 days for WM-URD, 13 and 16.5 days for MM-URD, and 12 and 14 days for HAPLO, respectively. The five-years overall survival (OS), failure-free survival (FFS), and cumulative incidences (CIs) of graft-failure and transplant-related mortality were similar among three groups: 88.3%, 85.5%, 2.7%, and 11.7% for WM-URD; 81.7%, 81.7%, 0%, and 18.3% for MM-URD, and 86.3%, 84.1%, 6.7%, and 9.2% for HAPLO. The 180-days CI of grade II-IV acute GVHD in WM-URD, MM-URD and HAPLO were 35.6%, 52.9%, and 28.9%, respectively; and moderate to severe chronic GVHD were 28.7%, 38.7% and 11.8% in respective cohort. The CI of grade II-IV acute GVHD and moderate to severe chronic GVHD were significantly higher in MM-URD than those in HAPLO (both, p=0.026). ATG is the only factor affecting both grade II-IV acute GVHD (Hazard ratio 0.511, p=0.01) and moderate to severe chronic GVHD (Hazard ratio 0.378, p=0.003) in multivariate analysis. Other complications including CMV DNAemia, hemorrhagic cystitis, invasive fungal disease, secondary malignancy, and sinusoidal obstruction syndrome were similar among three groups. Survival outcomes of a subgroup of ≥ 2 allele MM-URD (n=16) extracted form MM-URD were inferior that of other donor types (n=136): 75.0% vs. 86.9% (p=0.163) for 5-year OS and 75.0% vs. 84.7% (p=0.272) for 5-year FFS. Conclusion This study shows that there were no significant differences between alternative donor sources in the absence of suitable matched sibling donor. Host/donor features and urgency of transplant should drive physician towards the best choice among alternative donor sources for SAA patients treated with SCT. However, selection of ≥ 2 allele MM-URD should not be recommended due to high incidence of GVHD and inferior outcomes. Introduction: About 2 to 10% of patients (pts) diagnosed with Chronic Lymphocytic Leukemia (CLL) develop diffuse large B-cell lymphoma (DLBCL, so-called Richter transformation (RT)) over long-term follow-up. The outcomes of pts with RT are variable and poorly understood and there is no consensus on the best therapeutic approach. The aim of this study was to analyze the clinical characteristics, outcomes and factors predictive of survival in a large series of RT from the French Innovative Leukemia Organization (FILO). Methods: Biopsy-confirmed RT (limited to DLBCL and excluding Hodgkin lymphoma) diagnosed from 2001 to 2018 were identified from eight FILO centers. Clinical and biological characteristics of CLL and RT at diagnosis, including cytogenetics, clonal relation with the pre-existing CLL, Epstein-Barr virus (EBV) status, cell of origin (COO) analyzed by immunohistochemistry and RT score (Tsimberidou AM et al, J Clin Oncol, 2006) were analyzed as well as treatment and outcomes. Overall survivals (OS) were defined as time from CLL and RT diagnosis to death from any cause and analyzed using the Kaplan-Meier method. Statistical analyses were performed with SAS version 9.4. Results: A total of 70 CLL pts who developed RT were identified. The median age at CLL diagnosis was 62 years old (range 35-82), and 50 (71.4 %) were male. The median time to transformation was 5.5 years (range 0 to 22 years), with 12 simultaneous diagnosis of CLL and RT. Prior to RT, 20 (29%) pts had not been treated for CLL, 50 received one (n=21) or more (n= 29) line of treatment ; 6 pts had received a novel agent (ibrutinib, idelalisib or venetoclax). The median age at RT diagnosis was 68 years old (range 42-88). All biopsies were centrally reviewed; 38/58 pts (66%) had elevated LDH (>1.5N) ; 35/65 pts (54 %) had bulky disease (≥ 5 cm); 10/54 (18.5%) pts had del(17p) or TP53 mutation ; 9/42 pts (21%) had a complex karyotype (at least 3 abnormalities). The CLL and RT were clonally related in 27/27 (100%) tested pts. COO by Hans algorithm was non germinal center B cell-like (GCB) in 26/28 pts (93%). EBV was positive or detected in 5/40 (12.5%) pts. The median of Ki67 positivity was 70% (range 30% to 100%). The RT score (based at RT diagnosis on ECOG performance status 2-4, LDH >1.5 x normal, platelets<100 x 109/L, tumor size >5 cm and >1 prior therapy for CLL) was : low risk in 17 pts (31%), low-intermediate risk in 10 pts (19%), high-intermediate risk in 14 pts (25%) and high risk in 14 pts (25%). The most common first-line treatment of RT was immunochemotherapy (n=57, 87%) including R-CHOP-like regimen (n=48, 73%). Autologous or allogeneic transplantation was performed for 7 pts (11%). Response to first-line treatment was complete or partial response in 26 pts (40%), and stable disease or progression in 39 pts (60%). After a median follow-up of 8 years, 51/64 pts (80%) have died. The main causes of death were progressive DLBCL (n=36, 71%), infection (n=8, 16%) or progressive CLL (n=2, 4%). The median OS of the cohort from CLL and RT diagnosis (Figure 1) were 7.8 years and 9.5 months, respectively. In univariate analysis, patients with TP53 disruption at CLL stage, low platelets count, elevated LDH, elevated beta2-microglobulin, high ECOG score, high RT score, EBV positivity and absence of response to first-line RT treatment had worse OS. The ECOG score, platelets count and TP53 disruption remain significant in multivariate Cox-regression. Last, we compared the clinical and biological parameters of two Richter groups defined as: (i) short-term survivors (<12 months, n = 34) and (ii) long-term survivors (>48 months, n = 18). Long survival was significantly associated with elevated platelets count, low LDH, low ECOG, low RT score and response to RT first-line treatment. Discussion: The clinical outcomes of RT patients is poor and novel treatment options are needed. However, a group of long-term survivors was identified, characterized by elevated platelets count, low LDH, low ECOG, low RT score and response to immunochemotherapy.
