Abstract Background Calcitonin gene-related peptide (CGRP) (receptor) antibodies (erenumab, fremanezumab and galcanezumab) are increasingly used in prophylactic treatment of migraine. In the approval studies, severely affected patients with migraine and chronic daily headache without any headache free days were excluded. Thus, less is known about the effectiveness of CGRP antibody treatment in this cohort. Methods Clinical routine data of 32 patients with migraine and daily headache were analysed after three months of treatment with a CGRP antibody (16 erenumab, 7 galcanezumab, 9 fremanezumab), including changes of monthly headache days (MHD) monthly migraine days (MMD) and monthly acute medication intake (AMD) as well as migraine characteristics. Statistical analysis was performed with the Wilcoxon-Test. Migraine characteristics were analysed descriptively. Results The number of MHD was significantly reduced (mean reduction (standard error), p-value): (-4.2 (1.3), p = 0.009) as well as MMD (-4.3 (1.6), p = 0.033). Four patients (13 %) reached a 50 % reduction regarding MHD and 8 patients (25 %) regarding MMD, migraine duration and intensity improved under therapy. Conclusions Despite the low responder rate, CGRP antibodies can be effective at least in a few cases of severely affected patients with drug resistant migraine and chronic daily headache. <p>Learning activities may cause excessive stress IN children. 82.8% of the causes of stress in children in Indonesian are from their very tight routine. Data of the National Commission for Children Protection show an average of 200 cases per month which increases 28%. Brain gym is used as one of the activities to address the issue of stress in children and to improve memory. This research is aimed at identifying the effect of brain gym on stress levels in school-age children of fifth grade at SD Negeri Pokoh 1 Wedomartani Ngemplak Sleman Yogyakarta. This research is a quasi-experimental research with one-group pre and post-test design. The subjects of the research were all fifth grade students at SD Negeri Pokoh 1 Wedomartani Ngemplak Sleman Yogyakarta in 2014, numbering 36 students. Sampling employed a total sampling and data were analyzed using the wilcoxon test. The results showed that the stress level of children before the brain gym given was mostly in the moderate category (50.0%) and after the brain gym given was mostly in the normal category (75.0%). The statistical test using the wilcoxon test generated p-value of 0.000 &lt;0.05. Conclusion, there was a significant effect of brain gym on stress levels before and after treatment in the fifth grade students at SD Negeri Pokoh 1 Wedomartani Ngemplak Sleman Yogyakarta.</p>

ZusammenfassungIn diesem Artikel wird eine aktuelle Übersicht über die Prinzipien der modernen Schmerztherapie gegeben. Folgende Neuerungen der letzten Jahre sind von besonderer Bedeutung. Bei der Therapie chronischer Schmerzen erzielt die Kombination medikamentöser und nicht medikamentöser Maßnahmen die besten Erfolge. Der Einsatz von Opioiden kann unter Beachtung der evidenzbasierten Leitlinien auch bei Nichttumorschmerzen erfolgen. Die neuen Opioide sind nebenwirkungsärmer geworden, insbesondere im gastrointestinalen Bereich. Neuerdings steht ein spezifisches Naltrexon-Analogon zur Behandlung der opioidinduzierten Obstipation zur Verfügung. Neue Substanzen gegen neuropathische Schmerzen sind Oxcarbazepin, Pregabalin, Duloxetin und Lacosamid. Akupunktur bei chronischen Schmerzen ist besser als Warteliste. Für die Diagnostik von Kopfschmerzen ist 2004 eine neue Klassifikation der International Headache Society erschienen. Vor allem das Konzept des Chronic Daily Headache ist dabei weiterentwickelt worden. In der Attackenbehandlung der Migräne erzielt der frühzeitige Einsatz von Triptanen den besten Therapieerfolg. Eine Kombination von Triptanen und NSAR ist in Einzelfällen insbesondere beim Wiederkehrkopfschmerz sinnvoll. Substanzen der ersten Wahl in der Migräneprophylaxe sind die Betablocker Propranolol und Metoprolol, Flunarizin, Valproat und Topiramat.
