SummaryA study was carried out to compare the sensitivity as well as cost of three different methods of psychiatric case detection. It was found that the simplest method, which involved interviewing about 3 per cent of the adult population, with a questionnaire taking only five minutes to complete, picked up as many adult epileptics and nearly as many psychotics as the inquiry with all the adults in the population using a sophisticated structured interview schedule. A method of medium complexity, in which the short five-minute questionnaire was given to one adult member of each family, detected in addition to all adult epileptics and psychotics, and many juvenile epileptics and mentally retarded.The cost of the simplest method was one-ninth and that of the method of medium complexity, one-fifth of the cost of the most sophisticated method.The method of medium complexity is recommended for use in the rural psychiatry programme of the developing countries.

The research work was carried out with the objective to estimate the cost and returns in cultivation of paddy (basmati) in IPM-INM and CPM farms in Haryana. Information was extracted from 120 farmers by using well-structured interview schedule through survey method. Firstly four districts was purposively selected then one block was selected randomly from each district and from each block, 20 IPM-INM farmers and 10 CPM farmers were surveyed. The overall findings revealed that the total cost of cultivation for IPM-INM technology in paddy (` 110574ha-1) was somewhat greater than CPM paddy (` 106456 ha-1), the per quintal cost of production of IPM-INM paddy (` 2197) which was lower as compared to its CPM (` 2300), mainly due to higher productivity of IPM-INM (50.33 q/ha) paddy than the CPM (46.29 q/ha). The gross returns from IPM-INM and CPM farm was estimated to ` 138854 and ` 127927 per hectare respectively. As indicated by B:C ratio over variable cost of 2.73:1 for IPM-INM and 2.45:1 for CPM. Therefore, the cultivation of paddy was found to be more profitable with the adoption of IPM-INM technology, which is sustainable in every aspect.

The Bihar state produces more than 80 per cent of Makhana globally. Despite having monopoly production, the net profit earned by Makhana producers is relatively small as the cost of cultivation is high. In order to analyse the cost of cultivation and to identify major cost incurring operations, the present study was conducted in Purnea, Darbangha and Saharsa districts of Bihar. A random sample of 120 Makhana growers and 60 processors was randomly selected from study area. Respondents’ personal interviews were conducted with the help of semi structured interview schedule to collect the data. Findings of the study revealed that majority of the Makhana cultivators belongs to Mallah community. The average age of Makhana growers was 47 years (SD=10.3). The majority of Makhana growers (65.83%) belonged to the age group of 45-64 years. Further it was observed that 100 per cent of the sampled growers’ and processors’ households were headed by males. The average family size of Makhana grower was 9. The majority of the Makhana growers were living in joint family. It was found that, majority (76.66%) of the Makhana growers were having small and marginal holdings of which 34.16 per cent were small farmer and 42.5 per cent were marginal farmer. It was also observed that the average income of the Makhana growers was Rs. 224925 per year. Further it was found that cost of Makhana cultivation per acre was Rs. 48248/acre, The major cost component of Makhana cultivation was found to be the harvesting activity which contributed around 60 per cent of total cultivation cost. The revenue earned by sales of Makhana was Rs. 59299 per care. From each acre of Makhana cultivated farmers earned Rs. 11051. The B:C Ratio of Makhana production activity was found to be 1:1.22. Therefore it was concluded that, there exists ample scope for reduction of cost of cultivation through mechanization of harvesting activity.

Background and design— Australia has the highest rates of skin cancer in the world, and the incidence is estimated to be doubling every 10 years. Despite advances in the early detection and treatment of melanoma about 800 people still die nationally of the disease each year. A possible strategy for further reducing the mortality from melanoma is an organised programme of population screening for unsuspected lesions in asymptomatic people. Arguments against introducing melanoma screening have been based on cost and the lack of reliable data on the efficacy of any screening tests. To date, however, there has been no systematic economic assessment of the cost effectiveness of melanoma screening. The purpose of this research was to determine whether screening may be potentially cost effective and, therefore, warrants further investigation. A computer was used to simulate the effects of a hypothetical melanoma screening programme that was in operation for 20 years, using cohorts of Australians aged 50 at the start of the programme. Based on this simulation, cost—effectiveness estimates of melanoma screening were calculated. Results— Under the standard assumptions used in the model, and setting the sensitivity of the screening test (visual inspection of the skin) at 60%, cost effectiveness ranged from Aust$6853 per life year saved for men if screening was undertaken five yearly to $12137 if screening was two yearly. For women, it ranged from $11 102 for five yearly screening to $20 877 for two yearly screening. Conclusion— The analysis suggests that a melanoma screening programme could be cost effective, particularly if five yearly screening is implemented by family practitioners for men over the age of 50.

Objective To compare the cost-effectiveness of sialendoscopy with gland excision for the management of submandibular gland sialolithiasis. Study Design Cost-effectiveness analysis. Setting Outpatient surgery centers. Methods A Markov decision model compared the cost-effectiveness of sialendoscopy versus gland excision for managing submandibular gland sialolithiasis. Surgical outcome probabilities were found in the primary literature. The quality of life of patients was represented by health utilities, and costs were estimated from a third-party payer’s perspective. The effectiveness of each intervention was measured in quality-adjusted life-years (QALYs). The incremental costs and effectiveness of each intervention were compared, and a willingness-to-pay ratio of $150,000 per QALY was considered cost-effective. One-way, multivariate, and probabilistic sensitivity analyses were performed to challenge model conclusions. Results Over 10 years, sialendoscopy yielded 9.00 QALYs at an average cost of $8306, while gland excision produced 8.94 QALYs at an average cost of $6103. The ICER for sialendoscopy was $36,717 per QALY gained, making sialendoscopy cost-effective by our best estimates. The model was sensitive to the probability of success and the cost of sialendoscopy. Sialendoscopy must meet a probability-of-success threshold of 0.61 (61%) and cost ≤$11,996 to remain cost-effective. A Monte Carlo simulation revealed sialendoscopy to be cost-effective 60% of the time. Conclusion Sialendoscopy appears to be a cost-effective management strategy for sialolithiasis of the submandibular gland when certain thresholds are maintained. Further studies elucidating the clinical factors that determine successful sialendoscopy may be aided by these thresholds as well as future comparisons of novel technology.

Objective Evidence backing the effectiveness of mobile health technology is growing, and behavior change communication applications (apps) are fast becoming a useful platform for behavioral health programs. However, data to support the cost-effectiveness of these interventions are limited. Suggestions for overcoming the low output of economic data include addressing the methodological challenges for conducting cost-effectiveness analysis of behavior change app programs. This study is a systematic review of cost-effectiveness analyses of behavior change communication apps and a documentation of the reported challenges for investigating their cost-effectiveness. Materials and methods Four academic databases: Medline (Ovid), CINAHL, EMBASE and Google Scholar, were searched. Eligibility criteria included original articles that use a cost-effectiveness evaluation method, published between 2008 and 2018, and in the English language. Results Out of the 60 potentially eligible studies, 6 used cost-effectiveness analysis method and met the inclusion criteria. Conclusion The evidence to support the cost-effectiveness of behavior change communication apps is insufficient, with all studies reporting significant study challenges for estimating program costs and outcomes. The main challenges included limited or lack of cost data, inappropriate cost measures, difficulty with identifying and quantifying app effectiveness, representing app effects as Quality-adjusted Life Years, and aggregating cost and effects into a single quantitative measure like Incremental Cost Effectiveness Ratio. These challenges highlight the need for comprehensive economic evaluation methods that balance app data quality issues with practical concerns. This would likely improve the usefulness of cost-effectiveness data for decisions on adoption, implementation, scalability, sustainability, and the benefits of broader healthcare investments.

ABSTRACT Objective The objective of the present study was to evaluate 88 adolescent crack users referred to hospitalization and to follow them up after discharge to investigate relapse and factors associated with treatment. Methods Cohort (30 and 90 days after discharge) from a psychiatric hospital and a rehab clinic for treatment for chemical dependency in Porto Alegre between 2011 and 2012. Instruments: Semi-structured interview, conducted to evaluate the sociodemographic profile of the sample and describe the pattern of psychoactive substance use; Crack Use Relapse Scale/CURS; Questionnaire Tracking Users to Crack/QTUC; K-SADS-PL. Results In the first follow-up period (30 days after discharge), 65.9% of participants had relapsed. In the second follow-up period (90 days after discharge), 86.4% of participants had relapsed. Conclusion This is one of the first studies that show the extremely high prevalence of early relapse in adolescent crack users after discharge, questioning the cost/benefit of inpatient treatment for this population. Moreover, these results corroborate studies which suggested, young psychostimulants users might need tailored intensive outpatient treatment with contingency management and other behavioral strategies, in order to increase compliance and reduce drug or crime relapse, but this specific therapeutic modality is still scarce and must be developed in Brazil.
