In this publication we report a case of atypical, aggressive clinical course of arachnoid cyst in 19-year old female patient, which caused raised intracranial pressure and disruption of bony structures of the middle cranial fossa and the orbit. It also describes peculiarities of operative management and results of surgical treatment of this patient.

Three−negative breast cancer is characterized by aggressive clinical course, early metastasis and poor prognosis, leading to more active oncosurgical tactics and systemic treatment. Due to the aggressive clinical course of this oncopathology with increasing risk of locoregional and distant metastases, the most common surgical treatment tactics are modified variants of radical mastectomy with wide dissection of lymph nodes in areas of potential metastasis. Radicalism of the operation is the cause of the increase in the incidence of wound and lymphoenosis, the development of which depends on many factors, the determination of which is important for the development of methods of prevention of complications. In recent years, in the early stages of the disease, a radical breast resection after a course of adjuvant radiotherapy has been considered as an alternative. To study the immediate post−surgery results of three−negative breast cancer, depending on the initial clinical and pathological parameters and features of therapeutic tactics, a study was conducted in which 66 patients participated. Patients underwent radical mastectomy and breast resection, as well as lymph node dissection. Nine patients underwent aloplastic reconstruction. The results of the analysis showed that most often after radical surgeries, irrespective of their volume, there are lymphovenous complications, the development of which is affected by an increase in body mass index and lesions of lymph nodes. Aloplastic breast reconstruction contributes to a reduced incidence of lymphoid complications, and neoadjuvant chemotherapy is associated with an increased incidence of wound complications. These factors should be considered when planning the surgery protocols of patients with three negative breast cancer. The clinical picture of the combined course of irritable bowel syndrome (IBS) and neurocirculatory dystonia (NCD) in young patients has been described in the article. Features of abdominal pain syndrome and disorders of defecation, depending on the type of NCD (hypertonic or hypotonic) were shown. The conditions under which the exacerbation of IBS occurred in these patients were reviewed. The role of the family doctor in monitoring of the comorbid conditions was shown.

Renal medullary carcinoma (RMC) is a rare, yet aggressive malignancy of the kidney that is found predominantly in young patients with African descent and sickle cell hemoglobinopathies and most specifically sickle cell trait. Due to its aggressive nature, most cases have metastasis or local invasion at the time of diagnosis. Prognosis is extremely poor with survival less than 1 year after diagnosis. Here we present a case of metastatic RMC in a 29-year-old African female. Despite chemotherapy with cisplatin, gemcitabine, and paclitaxel, and initial shrinkage of the tumor, the patient died 5 months after diagnosis.

The treatment of ulcerative colitis (UC) has changed over the last decade. It is extremely important to optimize the therapies which are available nowadays and commonly used in daily clinical practice, as well as to stimulate the search for more powerful drugs for the induction and maintenance of sustained and durable remission, thus preventing further complications. Therefore, it is mandatory to identify the patients' prognostic variables associated with an aggressive clinical course and to test the most potent therapies accordingly.To date, the conventional therapeutic approach based on corticosteroids, salicylates (sulfasalazine, 5-aminosalicylic acid) or immunosuppressive agents is commonly used as a first step to induce and to maintain remission. However, in recent years, knowledge of new pathogenetic mechanisms of ulcerative colitis have allowed us to find new therapeutic targets leading to the development of new treatments that directly target proinflammatory mediators, such as TNF-alpha, cytokines, membrane migration agents, cellular therapies.The aim of this review is to provide the most significant data regarding the therapeutic role of drugs in UC and to give an overview of biological and experimental drugs that will become available in the near future. In particular, we will analyse the role of these drugs in the treatment of acute flare and maintenance of UC, as well as its importance in mucosal healing and in treating patients at a high risk of relapse.
