Idiopathic CD4 lymphocytopenia (ICL) is characterised by a low CD4 +lymphocyte count in the absence of HIV or other underlying aetiologies. We report a case of a 17-year-old girl with ICL with autoimmune hepatitis who developed isolated renal mucormycosis, which, to our knowledge, is the first reported case described in literature. Combination therapy with antifungals and surgical resection was done, and the patient improved. This case report illustrates the importance of timely multidisciplinary approach to recognise this highly fatal disease at an early stage.

The authors present a review of the literature on arteriovenous shunting, which is a relatively new theory. It is described in the patient with diabetes and coexisting neuropathy. The authors present a case report of a patient with arteriovenous shunting and diabetic neuropathic ulcerations.

Osteopetrosis congenita is invariably a fatal disease that is diagnosed within the first decade of the child's life. Although some steps have been taken to overcome this disease by bone marrow transplantation, further research is needed to determine the long-term effectiveness of this treatment. Osteopetrosis tarda is often detected as an incidental finding. Many cases are asymptomatic, yet many patients do suffer from general bone pain, infections, and repeated incidents of pathologic fractures. Such cases may present within the bones of the foot. Treatment is directed at addressing the pathology that may have resulted from the disease state. Osteopetrosis tarda, although rare, can affect the foot. Steps toward treatment of the primary disease entity are still being taken; however, recognition and treatment of the pathology is the primary goal in the adequate treatment of these patients.

Haemorrhagic nephritis and enteritis of geese (HNEG) is a fatal disease caused by goose haemorrhagic polyomavirus (GHPV). The aim of our study was to investigate a field outbreak of HNEG by pathological methods and real-time PCR assay using light upon extension (LUX PCR) with special regard to the possibility of vertical transmission. This is the first time that presence of GHPV was confirmed in goslings that died within 4 days after hatching showing typical symptoms of HNEG, which indicates vertical transmissibility as the shortest incubation period of HNEG is 6 days. The way of viral transmission is a key issue and thus the disease control measurements and HNEG epizootiology may be revised based on the findings of this study.

Fatal asthma is a rare and potentially fatal disease. This disease requires suitable treatment to achieve rehabilitation, especially when accompanied by other complications, such as a silent chest and severe bronchial spasm. A 36-year-old man presented with a 10-year history of asthma that broke out into persistent asthma attacks and cardiac arrest, and was accompanied by a silent chest for 18 hours. He recovered and was discharged without any sequelae after being treated by a ventilator, hormones, epinephrine, analgesics, sedation, and muscle relaxants. Comprehensive treatment with a ventilator, hormones, epinephrine, analgesics, sedation, and muscle relaxants has a good effect on fatal asthma combined with a silent chest.
