ABSTRACT Introduction: Brief introduction: Ankle tendon and ligament sports injuries are common in football players. Objective: To continue to improve special strength training related to the characteristics of football after rehabilitation of injured ankle tendons and ligaments. Methods: Two master football sportsmen were rehabilitated by multi-point equal-length, short-arc and long-arc equal-speed training combined with balance ability exercises. Results: There were two long muscle L be maintain muscle tone plantar flexors force four times of 96 n/m, n/m 121, 140 n/m, 145 n/m than back flexors force of 63 n/m, 52 n/m, 60 n/m, 74 n/m tall. Plantar flexor fatigue was 57%, 30%, 29%, 12%, 28%, 18%, 20%, 21%. Conclusions: With the passing of time, the relative peak moment value of the right ankle plantar flexor muscle group of the two patients kept rising, the dorsiflexor muscle was basically flat, and the work fatigue index decreased step by step, indicating that the right ankle muscle strength level was significantly improved, the anti-fatigue ability was improved, and the rehabilitation treatment had a good effect. Retinal vessel segmentation is essential for the detection and diagnosis of eye diseases. However, it is difficult to accurately identify the vessel boundary due to the large variations of scale in the retinal vessels and the low contrast between the vessel and the background. Deep learning has a good effect on retinal vessel segmentation since it can capture representative and distinguishing features for retinal vessels. An improved U-Net algorithm for retinal vessel segmentation is proposed in this paper. To better identify vessel boundaries, the traditional convolutional operation CNN is replaced by a global convolutional network and boundary refinement in the coding part. To better divide the blood vessel and background, the improved position attention module and channel attention module are introduced in the jumping connection part. Multiscale input and multiscale dense feature pyramid cascade modules are used to better obtain feature information. In the decoding part, convolutional long and short memory networks and deep dilated convolution are used to extract features. In public datasets, DRIVE and CHASE_DB1, the accuracy reached 96.99% and 97.51%. The average performance of the proposed algorithm is better than that of existing algorithms.

There are many metabolic and hormonal factors related to polycystic ovary syndrome (PCOS) that can be affected by vitamin D3 supplementation. To find clinical trials, in vivo studies, and in vitro studies that met the review's inclusion and exclusion criteria, we searched many databases. PCOS women's ovulation and metabolic parameters were examined in relation to the effects of vitamin D3 treatment on PCOS risk variables such as seasonal changes in body mass index, and obesity. The current review included twenty-five articles. Vitamin D3(25-hydroxy vitamin D) levels were significantly lower in the PCOS group than in the control group, and lipid profile and androgen hormone levels were significantly higher in the PCOS group, resulting in increased cardiovascular events and exaggerated hirsutism. According to the majority of research, vitamin D3 plays a beneficial role in decreasing the pathophysiology of PCOS, notably in restoring ovulation, which ultimately improves fertility. Although other studies found no effect on lipid profile, there was a minor effect on reducing cardiovascular risks. The response of patients to vitamin D3 was influenced by the dose administered and the study's methodology. In conclusion, vitamin D3 had a good effect on the pathophysiology of PCOS in the majority of investigations.

Beams are part of the building structure that is important to consider when designing the structure. Some failures that occur in beams due to beam reinforcement are not installed such as planning/ design procedures, this problem can cause structural failure. Knowing the behavior of the beam structure due to the given load can help predict the strength of the structural beam and the comfort of the planned structure. To determine and predict the strength and comfort of the reinforced concrete beam structure due to the received load, experimental and simulation tests are carried out. VecTor2 simulation is used to predict shear, crack, and displacement forces in reinforced concrete beams when displacement loads are applied. The bond stress-slip effect (0.139) has a good effect on the strength and hysterical response of reinforced concrete beams. From the results of pushover testing and simulations, it is obtained that the ratio for load capacity ranges from 1.00-1.095. In addition, the installation of 135˚ hooks on stirrups shows that the crack behavior that occurs forms an angle of 45˚, this indicates that the bond between concrete and reinforcement is going well, this can be seen from the analogous behavior principle of reinforced concrete beams.

To improve the effect of urban agricultural garden landscape planning and design, this paper combines the agricultural Internet of Things technology to construct a smart garden planning and design system. Moreover, this paper selects the LEACH protocol that can support monitoring for a long time according to actual application needs, introduces the latest swarm intelligence optimization algorithm, the gray wolf algorithm, to optimize some of the problems in the LEACH protocol, and conducts simulation experiments on the improved algorithm. The simulation experiment results show that the improved algorithm has obvious advantages in cluster head selection, data transmission within the cluster, and route maintenance. After constructing a smart garden planning system based on the agricultural Internet of Things, the effect of the agricultural Internet of Things data processing in this paper is evaluated. Finally, this paper constructs a garden simulation system and analyzes the performance of the system. The results verify that the agricultural Internet of Things has a good effect in the planning and design of smart gardens.

Abstract BackgroundAcute severe organophosphorus pesticide poisoning (ASOPP) is a common severe emergency in developing countries. At present, the incidence of complications and mortality are still very high, and there are some difficulties in treatment. More and more studies have found that hemoperfusion combined with hemodialysis can improve the prognosis of patients based on conventional emergency treatment, but there is a lack of evidence-based medical evidence. Our meta-analysis is mainly to clarify the efficacy of hemoperfusion combined with hemodialysis on ASOPP.MethodsWe searched several databases. After formulating relevant inclusion and exclusion criteria, qualified studies were included and data were extracted. The outcome indicators were cure rate, efficacy, incidence of complications, time for cholinesterase level to return to normal, coma time, time of hospitalization and atropine dosage. Results were analyzed using risk ratios (RR), odds ratios (OR), weighted mean difference (WMD), standard mean difference (SMD), and 95% confidential interval (95% CI). The cochrane collaboration tool was used for the assessment of the risk of bias of every included studies. In terms of statistical methods, we used RevMan software (version 5.3) and Stata (version 14 and 16) for data analysis.ResultsWe included 35 RCTs, including 2650 participants. Compared with the emergency conventional treatment group, the use of hemoperfusion combined with hemodialysis to treat ASOPP patients based on conventional emergency treatment significantly increased the cure rate [RR=1.20, 95% CI (1.16, 1.25), p<0.00001], improved the efficacy [OR=2.76, 95% CI (2.07, 3.67), p=0.000], and reduced the incidence of complications [RR=0.26, 95% CI (0.20, 0.34), p<0.00001] and also reduced the patient's time for cholinesterase level to return to normal [WMD=-4.05, 95% CI (-4.40, -3.70), p<0.00001], coma time [SMD=-2.62, 95% CI (-3.08, -2.15), p<0.00001], time of hospitalization [WMD=-5.80, 95% CI (-6.57, -5.02), p<0.00001], and atropine dosage [WMD=-194.81, 95% CI (-223.55, -166.08), p<0.00001].ConclusionHemoperfusion combined with hemodialysis can apparently improve the prognosis of patients and has a good effect in the treatment of acute severe organophosphorus pesticide poisoning. In the future, more scientifically designed large-scale, high-quality, multi-center randomized controlled trials are needed to further illustrate the significance of this study.

Abstract Background: After severe trauma of lower limbs, bone、tendon or plate graft exposure is common.The traditional repair method is to use a variety of skin flap transplantation to cover the exposed part, but the wound often can not heal after operation, or the wound is cracked, ulcer, sinus, bone and steel plate are exposed again after wound healing.The reason for this result is that when the flap is covered, the space around the bone plate is not well closed, forming a dead cavity, blood and exudate accumulation, hematoma formation or infection, and finally the wound ruptures again. In addition, due to the swelling and contracture of the flap after operation, the suture tension between the flap and the receiving area becomes larger, the skin becomes thinner and broken, and then the wound is formed. In order to solve the above problems, we carried out the study of artificial true skin embedding combined with fascial sleeve flap transplantation in the treatment of chronic bone plate exposed wounds of lower limbs.Methods: In this paper, 11 cases of chronic wounds with bone exposure and skin necrosis after steel plate implantation were selected. First stage is the wound bed preparation including primary wound expansion,removal of necrotic tissue and incision of sinus wall, removal of deep necrotic bone and fibrotic scarred skin on the outer wall of steel plate to normal tissue on the outer edge of the wound, removal of precipitated peptone and purulent fur in the hole, periphery and bone space of the steel plate, and removal of tendon tissue with basal necrosis and disintegration of the wound. After vacuum sealing drainage (VSD) 1~2 weeks, the peritraumatic basal granulation tissue grew well and there was no necrotic tissue in the wound. In the second stage, the exposed bone was covered with artificial dermis, the steel plate hole or the periphery and the basal space were filled, and the exposed steel plate was completely embedded, and then the fascia sleeve flap was transplanted to cover the wound. The sural neurovascular flap was performed in 9 cases and the lateral superior malleolar artery perforator flap in 2 case. Results: The flap survived well in all 11 cases. During the follow-up of 6 months to the removal of the plate, there was no case of rupture, exposure and sinus formation.Conclusions: Artificial dermal covering combined with fascial sleeve flap transplantation can effectively avoid wound dehiscence or sinus formation caused by foreign body retention, infection and flap contracture,It has good effect in repairing chronic wounds with bone plate exposure after severe trauma of lower limbs.

Abstract Background and ObjectivesAsthma is a severe public health issue since it causes a progressive decline of pulmonary function and a lot of unnecessary deaths. Traditional Chinese medicine has played a great role in the prevention and treatment of asthma, Wenyang-Huayin (WYHY) formulation is one of the useful prescriptions in clinical, which the pharmacological mechanism has not yet been revealed. In this study, we explored its anti-airway inflammation and pulmonary protection effects in the treatment of asthma induced by ovalbumin (OVA), to provide support for its deeply clinical applications.MethodsAsthma model with the syndrome of accumulation of cold fluid in the lung in chronic bronchitis (a common type of Traditional Chinese Medicine (TCM) syndrome) was induced in SPF male Wistar rats by sensitizing and challenging with OVA, followed by drink, body cold and fatigue stimulation. WYHY was orally administered at 9.82142857 g·(kg·d)−1 body weight for 21 days, Rapamycin and the other two traditional Chinese medicines (Xiaoqinglong decoction and Astragalus granules) were used as positive controls. After intervention, the pulmonary function was measured by AniRes 2005 Animal pulmonary function Analysis system, the pathological features of pulmonary alveolus and bronchus were observed after Hematoxylin-Eosin (H&E) staining, and serum interleukin-13 (IL-13), transforming growth factor -β1 (TGF-β1), tumor necrosis factor-α (TNF-α), and interferon-γ (IFN-γ) levels were determined by enzyme linked immunosorbent assay (ELISA) assay.ResultsThe model rats showed heavy signs of airway inflammation and remodeling like chronic asthma, evidenced by the damaged pulmonary alveolus structure, increased inflammatory cell infiltration, declined pulmonary function, and abnormal changes of cytokines. All treatment methods could slow down the damage caused by molding factors, and WYHY might have an advantage in pulmonary protection effects than the others.ConcludeWYHY has a good effect on anti-airway inflammation and protecting pulmonary function in the treatment of OVA-Induced asthma rats, and its mechanism is worth of in-depth study.

Abstract A kind of plugging proppants was developed for solving the problem of ‘easy to lose sand’, resulting in difficult sand control in low pressure lost-circulation wells in South Sudan Oilfield. The proppants can cement together to form a consolidated body with certain strength (≥2 MPa) and permeability (≥1.3 μm2) in oil layers. The body can temporarily plug lost circulation channels. Meanwhile, a supporting sand-adding tool was developed to simplify the proppants squeezed process, and a sand-carrying fluid was optimized to improve sand suspension performance and protect pay zones in low pressure lost-circulation wells. These measures formed a sand control technology for low pressure lost-circulation wells. 12 wells have been successfully applied in South Sudan Oilfield and achieved good effect on increasing oil production by 15 t/d.

Background: Neonatal sepsis and meningitis is an important cause of neonatal mortality and morbidity especially in the developing countries. Bacteriological profile of meningitis and antibiotic sensitivity pattern may vary from one region to another. Aims and Objectives: We have planned this study to know the etiological agent of neonatal meningitis with its antibiotic sensitivity profile and to evaluate some other associated risk factors of meningitis. Materials and Methods: This observational, cross-sectional study was done for a period of 1 year in the SNCU and NICU of a district Medical College of West Bengal in neonates presented with clinical sepsis and meningitis. Sepsis screen, blood culture, cerebrospinal fluid (CSF) study, and culture sensitivity was done and recorded along with demographic data, clinical presentation, outcome, and other associated factors. Results: We found meningitis in 55 neonates out of 250 clinical sepsis. CSF culture was positive in 42 cases with Escherichia coli (30.9%), Klebsiella (26.1%), Staphylococcus aureus (16.6%), Acinetobacter (14.2%) and Coagulase negative Staphylococcus (CoNS 11.9%) as prevalent organism. E. coli and Klebsiella were mostly sensitive to Amikacin, Levofloxacin, and Colistin whereas less sensitive to Cefotaxime, Pipercilin-tazobactam or Meropenem and Acinetobacter showed good sensitivity only to Levofloxacin. Among the gram-positive organism, S. aureus and CoNS were only sensitive to Linezolid, Vancomycin, and Teicoplanin. Conclusion: This type of study should help to make a proper antibiotic policy for an institution so that the empirical first-line antibiotic can be started with good effect in cases of neonatal sepsis and meningitis before the arrival of culture sensitivity report.
