Return to Competition following Clavicle Fractures in Professional Road Cyclists

Purpose: The COVID-19 outbreak has challenged professional athletes’ training and competition routines in a way not seen before. This report aims to inform about the changes in training volume and intensity distribution and their effects on functional performance due to a 7-week home-confinement period in professional road cyclists from a Union Cycliste Internationale Pro Team. Methods: A total of 18 male professional cyclists (mean [SD] age = 24.9 [2.8] y, body mass = 66.5 [5.6] kg, maximal aerobic power = 449 [39] W; 6.8 [0.6] W/kg) were monitored during the 10 weeks before the lockdown (outdoor cycling) and the 7-week lockdown (indoor cycling turbo trainer). Data from the mean maximal power output (in watts per kilogram) produced during the best 5-minute and best 20-minute records and the training intensity distributions (weekly volumes at power-based training zones) were collected from WKO5 software. Results: Total training volume decreased 33.9% during the lockdown (P < .01). Weekly volumes by standardized zones (Z1 to Z6) declined between 25.8% and 52.2% (effect size from 0.83 to 1.57), except for Z2 (P = .38). There were large reductions in best 5-minute and best 20-minute performance (effect size > 1.36; P < .001) with losses between 1% and 19% in all the cyclists. Conclusions: Total indoor volumes of 12 hours per week, with 6 hours per week at low intensity (Z1 and Z2) and 2 hours per week at high intensity over the threshold (Z5 and Z6), were insufficient to maintain performance in elite road cyclists during the COVID-19 lockdown. Such performance declines should be considered to enable a safe and effective return to competition.

AbstractBackground: The purpose of this study was to compare body balance in road and off-road cyclists, immediately before and after the racing season.Material/Methods: Twenty individuals participated in the study and they were divided into two groups: specialists in road-cycling (n = 10) and in off-road cycling (n = 10). Immediately before and after the five-month racing season stabilographic trials were carried out (at rest and after progressive exercise). In assessing body balance the distance and velocity of the centre shifts (in the anterior-posterior and left-right direction) were analysed. The tests were performed with the cyclists’ eyes open, eyes closed, and in feedback.Results: After the racing season, in the off-road cyclists’ group, distance and velocity of the centre of pressure shifts increased after a progressive exercise.Conclusions: In the off-road cyclists’ group the balance of the body in the sagittal plane deteriorated after the racing season. Moreover, after the racing season off-road cyclists were characterized by a worse balance of the body, compared to road cyclists

Background: Although clavicle fractures are a common sports injury, there are limited studies on the incidence and causes of clavicle fractures among winter sports athletes. Purpose: To evaluate the characteristics and injury mechanisms associated with clavicle fractures among patients evaluated at a Colorado ski resort. Study Design: Descriptive epidemiology study. Methods: This was a retrospective descriptive analysis of patients with clavicle fractures at the Denver Health Winter Park Medical Center during the 2012-2013 to 2016-2017 ski seasons. Chart review was performed on the patient cohort to confirm clavicle fracture diagnosis and to evaluate factors associated with clavicle fracture. Results: A total of 393 clavicle fractures (6.2% of total clinic visits) occurred during the study period, corresponding to an overall clavicle fracture incidence of 8.4 per 100,000 participant-visits. The mean patient age was 26.4 years (range, 5-73 years). The majority were middle-third fractures (85.5%), occurring mainly in men (87.3%). More than half of the fractures were comminuted (54.5%) and occurred in snowboarders (55.0%). The most common mechanism of injury was a fall onto snow while skiing or snowboarding (92.4%). Women sustained more clavicle fractures while skiing compared with snowboarding (82.0% vs 18.0%; P < .001), while men sustained more fractures while snowboarding compared with skiing (60.3% vs 39.7%; P < .001). Conclusion: Clavicle fractures are relatively common, but there are scant incidence data for clavicle fractures in mountain sports. Consistent with prior studies, clavicle fractures were more common in younger patients and men. The most common anatomic fracture location was the midclavicle. A greater proportion of clavicle fractures among men were sustained during snowboarding and among women during skiing.
