Eliminating Medicaid Adult Dental Coverage In California Led To Increased Dental Emergency Visits And Associated Costs

ABSTRACT Background This study aims to evaluate dental emergency visits in a public hospital as the number of dental emergency visits has recently increased. Materials and methods This was a retrospective observational study. This study was approved by the local ethical committee and data were recorded from the dental emergency service record book between January 1, 2012, and December 31, 2013. Results In total, 1,477 (4.01%) patients were admitted to emergency dentist services. Mean age was 24.59 ± 11.07 (min–max: 2–76) and 978 (66.2%) patients were between 19 and 35 years old. The most common diagnosis was pulpitis or periapical abscess (n = 1033, 69.9%) and the most common practice was the prescription of antibiotics and/or analgesics (n = 885, 59.9%). Conclusion The latest increase in the number of dental emergency visits is a burden, and emergency physicians must be capable of differentiating the real emergencies from common uncomplicated complaints. Dental caries/Tooth decay is progressive destruction of tooth structure, leading to tooth ache. Tooth ache is the most common type of orofacial pain and when severe it is considered a dental emergency. Pain killers will be the first option to get rid of pain but use of these pain killers have many adverse effects and also its over prescription leads to kidney disorders, liver disorders etc. and in Ayurveda for the management of pain in Krimidanta it is mentioned that, Krimighna (anti-microbial), Vatagna (which relieves Vata) and Ushnaveerya Dravyas (drugs with hot potency) are to be used which can relieve the toothache and discolouration. Hence here an attempt was made to find out an alternate medicine which is effective, has no contraindications and with no side effects. Here the patient was treated with Dantapichu (soaked cotton swab) with Katuparni Ksheera (milk of Argemone Mexicana Linn.) as a local application. After the treatment, a significant response in various symptoms such as tooth pain, tingling sensation and swelling was observed.

A retrospective study was performed to verify if the number of admissions for urgent dental care in the Urgent Dental Care Service of San Paolo Hospital in Milan (Italy) was directly related to the different phases of the COVID-19 pandemic. Different periods were analyzed: 25 March–5 April 2019 (pre-COVID); 23 March–3 April 2020 (lockdown); 8 June–19 June 2020 (reopening); and November 9–November 20 (second wave). Raw data regarding admissions, diagnoses, and treatments were extracted. Descriptive and bivariate analyses were performed. The survey included 901 admissions, 285 in pre-COVID, 93 during lockdown, 353 in reopening, and 170 in the second wave. In each time period, statistically significant differences were found in the prevalence of each kind of diagnoses (χ2(3) = 20.33 p = 0.01 for endodontic emergencies, χ2(3) = 29.05 p < 0.01 for cellulitis/phlegmon, χ2(3) = 28.55 p < 0.01 for periodontal emergencies, Fisher’s Exact Test p < 0.01 for trauma, and χ2(3) = 59.94 p < 0.01 for all other kinds of diagnosis). A remarkable increase in consultations (+186.36%) and other treatments (+90.63%) occurred during reopening. Tooth extraction was the most frequently delivered treatment, but suffered the largest reduction during lockdown (−79.82%). The COVID-19 pandemic has highly affected dental activity in north Italy, underling the weaknesses of a private dental system in a pandemic scenario.
