An unusual lesion mimicking adrenal malignancy

Merkel cell carcinoma is a rare, often fatal, neuroendocrine cutaneous malignancy with a highly variable clinical presentation. Due to the rapid progression of the disease, early detection and treatment is vital to survival. Here, we report the case of a 45-year-old woman whose podiatrist noticed an unusual lesion on her lower left leg and referred her to a dermatologist for work-up. A diagnostic excisional biopsy confirmed the diagnosis and was followed by treatment that included wide local excision of the primary lesion with subsequent chemotherapy and radiation. At the time of diagnosis, sentinel lymph node biopsy was positive. Due to the increasing prevalence of Merkel cell carcinoma in the past decade and its propensity to present on the lower extremity, podiatric physicians need to be aware of the clinical presentation and treatment guidelines for this elusive disease.

An intraoral mass of eight months duration in a six year-old girl was diagnosed as papillary endothelial hyperplasia. Histologically, the tissue was characterized by the unusual endothelial cell proliferation, which is significant, as papillary endothelial hyperplasia resembles angiosarcoma and possible over-treatment thereafter. Clarification of this unusual lesion based on histological findings with supportive histochemical staining and immunohistochemistry in the light of clinico-pathological correlation is discussed. This lesion warrants better documentation of the clinical behavior with regular monitoring.

✓ A giant arteriovenous fistula in a newborn infant was treated by surgical occlusion of the feeding vessels at 20 days of life. Congestive heart failure responded favorably to operative treatment. Because of persistent hydrocephalus, a shunt was inserted at 2 months of age. At 9 months of age, the child remained without signs of cardiac failure. Cerebral damage was manifested by a mild left hemiparesis. Successful surgical treatment of this unusual lesion in a neonate is exceptional.
