Introduction: Due to the fundamental role of esthetics in the outcomes of dental treatments, especially in the anterior region (esthetic zone), the necessity of considering the matter of esthetics in clinical studies has become into focus in the current era. The aim of this study was the evaluation of esthetic outcomes of two treatment protocols in the treatment of congenital uni-lateral missing of maxillary lateral incisors as well as patient satisfaction from the treatment outcomes. Methods: in this study the sample size was 24 people (16 women and 8 men), These individuals sought dental treatment for replacement of the congenitally missing maxillary lateral incisor. Convenience sampling method was used and patients were divided into two groups regarding the kind of treatment they received. The two treatment protocols included: 1. Space closure by means of orthodontic treatment and then reshaping the canines; and 2. Space regaining by means of orthodontic treatment and replacing the lateral incisor with dental implants. Photographs of patients were acquired from the frontal view with retraction of the lips using digital cameras. Photographs were evaluated for Pink esthetic score. Results: No significant difference was detected between the two study groups in the evaluated factors in this study. Conclusion: The results of this study indicated that there is no significant difference in esthetic results in the two groups. Furthermore, both groups lead to similar results in patient satisfaction from treatment outcomes.

Keberadaan kista dapat menghambat perawatan ortodontik konvensional yang diberikan pada pasien. Tujuan artikel ini adalah menyajikan laporan kasus perawatan ortodontik alternatif seorang pasien perempuan berusia 17 tahun dengan kondisi gigi atas berjejal berat dan kista jinak di antara gigi insisivus lateral dan kaninus kiri atas. Setelah menetapkan diagnosis ortodontik, perawatan pertama yang dilakukan adalah pengangkatan kista. Kista dikirim untuk biopsi dan didiagnosis sebagai kista jinak. Orang tua dan pasien memutuskan untuk mencabut gigi insisivus lateral atas setelah mengetahui bahwa gigi insisivus lateral kiri atas nekrosis. Setelah gigi dicabut, bracket ortodontik dipasang di gigi bawah pasien. Tiga bulan kemudian, bracket ortodontik dipasang di gigi atas pasien. Kondisi gigi atas yang berjejal terkoreksi dan kaninus atas menggantikan posisi insisivus lateral. Saat ini, perawatan aktif masih dilanjutkan dan terpasang bracket ortodontik dengan elastik kelas III pada pasien. Kesimpulan artikel ini adalah pendekatan perawatan ortodontik inkonvensional dapat menjadi pertimbangan ketika ditemukan hambatan seperti adanya kista. Pasien dan orang tua harus diberi informasi mengenai konsekuensi perawatan sebelum perawatan dimulai.Unconventional Orthodontic Treatment for Upper Teeth Crowding with Benign Cyst. The presence of a cyst can provide an obstacle to the conventional orthodontic treatment given to the patients. The purpose of this article is to describe a case report of a seventeen year-old girl with a severe crowding of maxilla and presence of benign cyst between lateral incisor teeth and upper left canine treated with an alternative orthodontic treatment plan. After orthodontic diagnosis, the first treatment to be conducted was the removal of the cyst. The cyst was sent for biopsy and diagnosed as non-malignant cyst. The patient and her parents decided to extract upper laterals incisor tooth after knowing that the upper left lateral incisor tooth was necrotic. After the extractions, orthodontic brackets were bonded on the mandible. Then three months later the maxilla was bonded. The upper crowding of teeth condition was resolved very promptly and the upper canines were used to replace the laterals incisor position. Currently, active treatment is still in progress, and the patient has full upper and lower orthodontic brackets with class III elastics. After the treatment so far, it can be concluded that unconventional orthodontic treatment is worth considering when an obstacle such as a cyst is present. Patients and parents must be informed about the consequences of the treatment before active treatment is started.

BACKGROUND: Teeth impaction has become a common problem faced by orthodontic clinicians with the greatest incidence reported among third molars and maxillary canines. The great challenge lies in successfully treating these cases without deleteriously affecting the impacted as well as adjacent teeth while achieving acceptable functional and esthetic results. Several etiological factors have been associated with impactions including the presence of an odontome which is an asymptomatic odontogenic hamartomatous lesion. CASE REPORT: This article presents a detailed orthodontic assessment and treatment of a 16 years old female having impacted right maxillary lateral incisor and canine caused by complex odontome. CONCLUSION: Successful orthodontic treatment of multiple impactions can be achieved with minimal side effects even when odontomes are associated, through 3D radiographic examination, detailed evaluation as well as proper biomechanical control.

Impacted teeth are one of the reasons for orthodontic appointments. Two conditions must be met for the tooth to be considered impacted, i.e. a fully developed tooth that has not erupted on time. The presence of an odontoma in the maxillary bone when the impacted teeth are observed is a particular situation. We present a case of a patient with a compound odontoma located in the anterior part of the mandible and associated with an impacted right lateral incisor, who received interdisciplinary (surgical and orthodontic) treatment. The aim of the study was to present the advances in interdisciplinary (surgical and orthodontic) treatment of the compound odontoma associated with the impacted permanent lateral incisor of the mandible and a review of relevant literature. A literature review covering years 1990-2016 was performed. Medical databases, such as PubMed and Medline, were used. The following keywords were used: “impacted tooth”, “odontoma”, “orthodontic treatment”. Additionally, a manual search of specialist literature on orthodontics and maxillofacial surgery was conducted. Written consent from the subject/subject’s parents/subject’s legal guardians was obtained. Polish- and English-language publications were included. Early diagnosis of odontoma enables less complicated and less expensive treatment. The treatment plan should be tailored to each patient. More research is needed on predicting the behaviour of impacted teeth related to odontomas.

Orthodontic treatment is thoroughly planned considering the patient’s facial and dental characteristics, the main complaint, treatment time and the orthodontist’s experience. Transposition is a form of ectopia, in which two adjacent teeth exchange positions in the dental arch. Transposition can be partial or complete. This article reports the treatment of a female patient with two kinds of tooth transposition managed in the mixed and permanent dentitions. A girl, aged eight years and three months, came to routine paediatric consultation with an ectopic permanent mandibular left lateral incisor in the mixed dentition. Radiographic analysis indicated partial transposition of the permanent mandibular left lateral incisor and canine (Md.L2.C), and development of a complete tooth transposition between the permanent maxillary right first pre-molar and canine (Mx.C.P1). The patient was treated in two phases. The first, in the mixed dentition, and the second, in the permanent dentition with a three-year follow-up between them. These challenging treatment approaches are described in detail, including the mechanics used. The key points of this treatment were early diagnosis of the ectopic mandibular lateral incisor, use of light forces and interphase patient follow-up. These determined the best time to start the second treatment phase, enabling achievement of aesthetic and functional outcomes, and the results remained stable one year after the end of orthodontic treatment.

Impaksi palatal kaninus sering mengakibatkan keluhan secara estetis. Faktor genetik merupakan faktor yang dominan serta beberapa faktor yang lain yaitu diskrepansi lengkung gigi, ukuran gigi, retensi gigi desidui, kerusakan dini, pencabutan dini , posisi yang abnormal benih gigi, agenese incisivus lateral dan kista. Tujuan laporan kasus ini adalah untuk memberikan informasi tentang perawatan kasus impaksi palatal kaninus rahang atas pada maloklusi kelas I skeletal menggunakan alat cekat teknik Begg. Pasien perempuan umur 15 tahun, datang ke klinik ortodonsia RSGM Prof. Soedomo FKG UGM. Pemeriksaan subyektif, pasien terganggu dengan keadaan gigi depan yang maju dan bercelah. Pemeriksaan obyektif menunjukkan adanya rudimenter, agenese gigi incisivus lateral kanan dan kiri rahang atas, impaksi palatal kaninus kiri atas. Transposisi kaninus kanan atas ke ruang incisivus lateral kanan atas, pergeseran midline. Maloklusi angle kelas I dengan relasi skeletal kelas I dengan bimaksiler retrusif disertai protrusif incisivus maksila dan retrusif incisivus mandibula. Perawatan diawali dengan pencabutan gigi rudimenter. Tahap I menggunakan multiloop archwire untuk leveling dan unraveling gigi anterior, koreksi pergeseran midline. Tahap berikutnya adalah pemasangan button pada kaninus impaksi untuk mengaitkan kawat ligatur pada archwire yang berfungsi untuk menarik kaninus impaksi palatal pada lengkung gigi. Perawatan ortodontik pada kasus dengan impaksi palatal kaninus rahang atas pada maloklusi kelas I skeletal menggunakan alat cekat teknik Begg dapat dilakukan dengan hasil perawatan yang baik. ABSTRACT: Repositioning of Palatally Impacted Canine in Orthodontic Treatment Using Begg Fixed Appliance. Palatally impacted canine often leads to esthetic complaints. Genetic factor is dominant followed by such other factors as dental arch discrepancy, tooth size, retention of deciduous teeth, early decay, premature extraction, abnormal position of tooth germ, lateral incisor agenesis, and cysts. This article provides information about the treatment of palatally impacted maxillary canine case in a skeletal class I malocclusion using Begg fixed appliance technique. A 15-year-old female patient came to the orthodontia clinic of RSGM Prof. Soedomo FKG UGM. The subjective examination found that the patient was disturbed by her protrusive, gapped front teeth. Then, the objective examination indicated the presence of rudimentary, lateral incisor agenesis of right and left upper jaw, and upper left palatally impacted canine. In addition, there was a transposition of upper right canine to lateral incisor area as well as a midline shift. Angle class I malocclusion with class I skeletal relationship and bimaxillary retrusion along with maxillary incisor protrusion and mandibular incisor retrusion also occurred. The treatment began with rudimentary tooth extractions. The first stage used a multiloop archwire for leveling and unraveling of anterior teeth as well as correction of midline shift. The button attached to the impacted canine could tie the ligature wire to the archwire that served to attract the palatally impacted canine in the dental arch. The orthodontic treatment in cases of palatally impacted maxillary canine with skeletal class I malocclusion using Begg fixed appliance technique can be applied with a good treatment result.
