CORRELATION BETWEEN LOW BACK PAIN DUE TO FATTY DEGENERATION AND SEX AND AGE: STUDY BY MRI

ABSTRACT Objective: To correlate low back pain caused by fatty degeneration, visualized by magnetic resonance imaging (MRI), with sex and age. Methods: With a timeframe of 2015 to 2020, data on sex and age of fifty symptomatic patients with low back pain were collected from medical records and correlated with MRI analyses showing the occurrence of fatty infiltration in the paraspinal and erector muscles. Three trained observers, using the classification of Goutallier et al. (1994), analyzed five lumbar levels and the degree of injury. The Kolmogorov-Smirnov, Shapiro Wilk, Student's t, Mann-Whitney and Spearman correlation tests were used, all with a 5% significance level. Results: Among the cases, the L5-S1 level was found in all the images, with a prevalence of 46% of grades 1 and 2 and 24% in the five lumbar levels. Severity was present in a small portion (8%) of the injuries. There was a predominance of 78% women, with significantly more severe injuries in relation to the number of levels (p < 0.001) and injuries of a higher degree (p < 0.001). The age range was 14 to 38 years (mean = 26.70 ± 5.70 years), showing a significant and moderate correlation between the number of levels reached and a greater degree of degeneration (p < 0.001). Conclusion: Fatty degeneration as a cause of low back pain was significantly more severe in women in terms to the number of levels and the higher degree of injury. The number of levels and the severity of fatty degeneration were not correlated with the patient's age. Level of evidence II; Retrospective Study.

Introduction: Patients with non-specific low back pain (NSLBP) and movement control dysfunction demonstrate alternation in hip muscles flexibility and spinal movement patterns. Therapeutic modalities that augment hip muscles flexibility could help these patients. The aim of this study was to investigate the effect of global postural reeducation (GPR) on pain and hip muscle flexibility in patients with NSLBP and movement control dysfunction. Materials and Methods: A total of 27 men with a mean age of 31.21±2.5147 years, height of 166.44±6.11 cm, and weight of 64.21±5.25 kg participated in this study. The visual analogue scale (VAS) was used to evaluate pain. The flexibility of hip muscles (rectus femoris, tensor fasciae latae, external rotators and hamstring) was measured using universal goniometer. All data were assessed at baseline and after the intervention. The Shapiro-Wilk test and paired t test were used for statistical analysis at significance level of P=0.05. Results: Our results revealed a decline in pain (P<0.004) and an increase in the flexibility of the hamstring muscles in the right (P<0.003) and left (P<0.003) legs. There were no statistically significant differences in the flexibility of rectus femoris muscle, external rotators, and tensor fasciae latae. Conclusion: The results suggest that GPR had a significant effect on the level of pain. Further, it affected the flexibility of hamstring muscles in legs. Using GPR is recommended for pain relief and improving the flexibility of hamstring muscles in patients with NSLBP.

ABSTRACT Objective To evaluate whether vertebral endplate signal changes (VESCs) influence the prognosis of patients submitted to conservative or surgical treatment for low back pain and lumbosciatica. Methods Study with 241 patients who underwent conservative treatment, infiltration or surgery with 12 months of follow-up. They were evaluated for pain by the Visual Analog Scale for Pain (VAS), for function by the Roland Morris questionnaire and for quality of life by the EuroQoI5 (EQ-5D). Results The VESCs did not have a significant effect on the treatment responses for the VAS (F = 0.03; P = 0.97), Roland Morris (F = 0.51; P = 0.60) and EQ-5D (F = 2.67; P = 0.07) variables, nor was there any interaction between VESC and treatment for VAS (F = 2.15; P = 0.08), Roland Morris (F = 1.55; P = 0.19) and EQ-5D (F = 2.15; P = 0.08). There was a significant effect for all treatments, however, the effect of the surgical procedure was superior when compared to the others (P <0.001). The VESC frequency was 48.33% for type 0, 29.17% for type I and 22.50% for type II. Conclusions The presence of VESC and its different types is not associated with a worse prognosis, nor was a higher prevalence of VESC observed in the patients with low back pain and lumbosciatica. Objective: to analyze the prevalence of psychological disorders (anxiety, depression, stress) and the relationship of these factors with functional disability in patients with chronic low back pain. Study design: cross-sectional descriptive and observational study. Setting: the physiotherapy outpatient clinic of the Northen University of Paraná, Londrina, Paraná, Brazil. Methods: 84 individuals were recruited, being 43 elderly (20 with low back pain and 23 control subjects) and 41 adults (21 with low back pain and 20 control subjects). In order to assess the psychological aspects, Beck Inventories of Depression and anxiety as well as Lipp Stress Inventory were used. To evaluate the funcionality, Roland Morris Questionnaire was chosen. The evaluation of the pressure pain threshold was performed using algometer emg systems®. To analyse was considered a significance level of 5% (p<0.05). Results: It was observed that people with low back pain have higher scores of depression, anxiety, stress and functional disability compared to the control group (p<0.05). Among adults, an inverse relationship between anxiety and disability (p<0.05) was observed. In subjects with low back pain positive correlation was observed between anxiety and functional disability and, between the depression and functional disability (p<0.05). In addition, no association was found between stress and functional disability in patients with low back pain (p<0.05). Conclusion: suggest that in patients with low back pain worsening of functionality is anxiety and stress and elderly people with low back pain has higher levels of stress, depression, anxiety and function disability.

ABSTRACT Objective Low back pain is an extremely common condition that can affect up to 84% of people at some point in life. It affects patients of both sexes, mainly between 30 and 50 years of age, and has a great impact on the quality of life, in addition to placing a significant demand on health services. Obesity is considered as an important risk factor for the development of low back pain since the change in body fat distribution causes changes in the locomotor system and especially in the spine. Methods The study makes a comparative, retrospective analysis of patients who underwent bariatric surgery through questionnaires to assess the improvement in their quality of life and low back pain. The data were submitted to a statistical analysis and the results compared to the literature data. Results As a result, there was a statistically significant improvement in quality of life in relation to weight loss; the greater the weight reduction, the better the quality of life and the younger the patient, the better their quality of life. Conclusions When asked about low back pain, 70.7% of patients classified it as moderate to severe. Level of evidence III; Comparative retrospective.

ABSTRACT The aim of this study was to evaluate the evidence of application of short-wave diathermy (SWD) in individuals with chronic low back pain (CLBP) and its prescription parameters. The data sources (MEDLINE, PubMed, LILACS, DARE, PsycINFO, AusportMed, SciELO, PEDro and the Cochrane Central Register of Controlled Trials) were systematically searched for articles published up to December 2017. Randomized and non-randomized clinical trial studies that investigated the effect of the application of SWD on CLBP were selected. Two independent reviewers assessed the risk of bias in studies using the Jadad and the Downs & Black scales. Five studies (731 patients) were included, all of whom presented improvements in CLBP. The majority used continuous mode SWD, with only one using pulsed mode (82 and 200 Hz). SWD produced improvements in CLBP, but there is limited evidence for its application, and a lack of standardization of the parameters used. Furthermore, the results that address this topic should be interpreted with caution due to their low methodological quality and limited number. Level of evidence II; Systematic review.

ABSTRACT Objective: To evaluate the effect of lateral lumbar interbody fusion (LLIF) or oblique interbody fusion (OLIF) on low back pain due to degenerative intervertebral disc disease. Methods: We systematically reviewed articles on surgical treatment of low back pain by LLIF and OLIF, according to the Cochrane Handbook for Systematic Reviews of Interventions. We searched through electronic databases, including Medline via PubMed, Lilacs through the Virtual Health Library (VHL), Cochrane Collaboration/Central and Cohrane/Dare Cochrane Controlled Trials Register, without language or publication date restrictions and with design for prospective cohorts and randomized clinical trials. Results: We have selected and presented three studies. Conclusion: The literature review showed great relevance in the improvement of the new surgical approach (LLIF) in relation to TLIF, but greater content availability in the databases is necessary to reach a satisfactory conclusion regarding the efficiency of the lateral approach and its advantages over other traditional procedures. ABSTRACT Objective: To correlate the results of the STarT Back Screening Tool and DRAM questionnaires, applied simultaneously, in a population with low back pain. Methods: Comparative cross-sectional study with 84 participants with low back pain assessed by both STarT Back Screening Tool (SBST) and DRAM questionnaires. The degree of correlation between the two questionnaires was analyzed through the evaluation of individualized data and using the Spearman correlation coefficient. Results: According to the DRAM, 19% of the patients were classified as “normal”, 32.1% as “at risk” and 48.8% as “distressed”. According to SBST, 59.5% of patients were classified as “low risk”, 31% as “medium risk” and 9.5% as “high risk”. Applying the Spearman's coefficient to evaluate the degree of correlation between the two questionnaires, a value of 0.4 was obtained. This shows that there is a positive, but weak, correlation (p <0.001) between the two questionnaires. Conclusion: There is a positive correlation between the two questionnaires, but the DRAM showed a greater tendency to classify patients with some degree of psychological distress when compared to the SBST. Both questionnaires are effective in identifying these factors, but the data suggest that the DRAM may be more effective as a screening tool in patients with low back pain, in view of the higher number of patients identified. This narrative review investigated imaging parameters of the paraspinal muscles and their association with spinal degenerative features and low back pain (LBP) found in the literature. Three principal signs of muscle degeneration were detected on imaging: decreased muscle size, decreased radiographic density, and increased fat deposits. Men have a higher density of paraspinal muscles than women, younger individuals have a higher density than older ones, and lean individuals have a higher density than those with an increased body mass index. Fatty infiltration appears to be a late stage of muscular degeneration and can be measured noninvasively by an MRI scan. Fatty infiltration in the lumbar multifidus is common in adults and is strongly associated with LBP, especially in women, independent of body composition. Fatty infiltration develops in areas where most degenerative changes are found. MR spectroscopy studies have corroborated that the lumbar multifidus in LBP subjects has a significantly higher fat content than asymptomatic controls. There is a strong need for establishing uniform methods of evaluating normal parameters and degenerative changes of the paraspinal muscles. Additional imaging studies are needed to improve the understanding of the association and causal relationships between LBP, spinal degeneration, and changes in the paraspinal muscles.
