This case study documents a 13-year-old female who presented to our intensive inpatient chronic pain rehabilitation program with complex regional pain syndrome (CRPS) of her left leg, which was significantly interfering with her normal daily functioning. She participated in a full day of traditional interdisciplinary therapies, including physical and occupational therapy for 3 hours daily. As assistive equipment was altered or weaned her physical mobility, balance, and tremors worsened and/or increased. As she began advancing her legs more independently (versus requiring physical assist), she demonstrated more variable functional strength and stability, inconsistent balance reactions, and a more unsteady gait pattern. The team was treating her according to her incoming CRPS diagnosis; however, as treatment progressed, her physical and psychological presentation seemed more aligned with diagnostic criteria of functional neurologic symptom disorder (FND). Staff then treated according to the FND diagnosis resulting in successful long-term outcomes. The clinical impact from this case study includes highlighting the commonalities between CPRS and FND clinically, discussing differentiating treatment suggestions depending on the diagnosis, and emphasizing key components of family/patient education.

Stroke causes disability with high morbidity and mortality in the world, causes a variety of disabilities and symptoms including disturbances in motor function, sensory, and cognitive. Sensory disability in post stroke patient can be categorized into two group; the one is stimulated by peripheral mechanism which we often called post stroke pain, and another one is stimulated by central mechanism which we called central post stroke pain (CPSP). Pain after stroke is usually underdiagnosed and poorly understood. In 1906, CPSP was called the thalamus pain syndrome by Dejerine and Roussy, but then it is known that CPSP can also developed in extra-thalamic stroke lesion. The prevalence of CPSP is approximately 1-12% in all around the world. CPSP occurs in one-third of overall post-stroke pain cases. In most cases, the onset of CPSP is within 1 month and then the incidence decreases with time. The most common manifestations are allodynia and dysesthesia. The pathophysiology itself remains clearly unknown in detail. Various theories such as central sensitization, disinhibition of medial thalamus, and central imbalance theory thought to be contribute in CPSP pathophysiology. This complexity make CPSP very difficult to manage. Some pharmacotherapies and non-pharmacotherapies have been studied to relieve pain in order to improve the quality life of CPSP sufferers. The aim of this article is to discuss the general view of central post stroke pain to increase the understanding and awareness of health giver in order to give sooner and better management for patient that can affect prognosis of the patient.

AbstractStroke is a brain functional disorder characterized by nerve paralysis due to obstruction of blood flow to the brain. The frequent impact if a post-stroke client is not carried out with rehabilitation immediately will result in mobility disorder that can impede daily physical activity. One pratice in the rehabilitation process is to prevent disability in a client with post-stroke physical mobility disorder that is, by performing a Range Of Motion rope. The case study is intended to illustrate application of roma exercises to boost muscle strength. The method of writing the case study USES a descriptive design and case studies reported in narration. The instrumrnts used are observation sheets and structured observation sheets covering the assessment of muscle strength. The resulth of the case study have been after the six-meeting application of roma that the roma can increase muscle strength in subject. I the average muscle strength 3.31 to 3.50 and subject II average muscle strength 3.68 to 3,93. The conclusion of the case study increased muscle strength after the practice of roma. Suggestions for the family in order to train ROM for family members who had suffered stroke. Saalah satu bentuk latihan dalam proses rehabilitasi untuk mencegah terjadinya kecacatan pada klien dengan gangguan mobilitas fisik pasca stroke yaitu dengan melakukan latihan Range Of Motion ROM. Studi kasus ini bertujuan untuk menggambarkan penerapan latihan ROM untuk meningkatkan kekuatan otot. Metode penulisan studi kasus ini menggunakan rancangan deskriptif dan hasil studi kasus dilaporkan dalam bentuk narasi. Instrumen yang digunakan yaitu lembar pengkajian dan lembar observasi terstruktur meliputi penilaian kekuatan otot. Hasil studi kasus setelah dilakukan penerapan ROM selama 6 pertemuan bahwa ROM dapat meningkatkan kekuatan otot pada Subyek I nilai rata-rata kekuatan otot 3,31 menjadi 3,50 dan Subyek II nilai rata-rata kekuatan otot 3,68 menjadi 3,93. Simpulan studi kasus ini terjadi peningkatan kekuatan otot setelah dilakukan latihan ROM. Saran bagi keluarga agar dapat melatih ROM pada anggota keluarga yang mengalmi stroke.
