The article presents the results of a study of legal regulation of medicines in accordance with the statement on January 1, 2019 of the Federal Project Creation of the Unified Digital Circuit of Health Care on the basis of the Unified State Health Information System. The project involves pharmacy organizations, which are going to issuance prescription drugs in the form of an electronic document. The development of the Unified State Health Information System will require a lot of organizational and methodological work both on the part of the regional health authorities and pharmacy organizations. An analysis of the activities of pharmacy organizations in Saint Petersburg and the Northwestern Federal District that issuance prescription drugs made it possible to assess the level of preparedness to work in the new conditions. The developed measures necessary for pharmacies to work in the Electronic Prescription System cover infrastructural, functional and educational aspects and can be used as an appropriate action plan.

Legislation in the field of e-health, adopted in 2017, opens up fundamentally new opportunities in the development of medical care using telemedicine technologies. The article provides an analysis of regulatory legal documents that establish the legal framework for the provision of medical care using telemedicine technologies. An assessment is given of the current state of the legal regulation of telemedicine in Russia. The main legal risks are identified in the practical implementation of telemedicine technologies, including the risks of identifying participants in telemedicine consultations. The procedure for transferring and entering information into the federal register of electronic medical documents and the integrated electronic medical record is not defined. The legal and organizational problems in regulating the interactions of medical organizations with the unified state health information system and other information systems are identified. The law on telemedicine requires adjustment and refinement of existing regulatory legal acts, procedures, standards for the provision of medical care with detailed regulation of tools and situations of their application. For the further development of digital health care, in particular telemedicine, electronic document management, electronic prescriptions and a unified state health information system, it is necessary to develop comprehensive rules for all telemedicine service providers. Due to the current situation in healthcare, now is the perfect time to create telemedicine technologies and other e-health products that will help in solving public health problems.

Background: As of today, there is no unified health information system that registers and systematizes the results of periodic medical examinations of workers exposed to occupational hazards in the Russian Federation. Our objective was to examine the results of periodic medical examinations in the Sverdlovsk Region over a five-year period. Materials and methods: We reviewed medical reports and conclusions issued by regional health facilities in 2015–2019 and submitted to the Regional Occupational Health Center of the Yekaterinburg Medical Research Center for Prophylaxis and Health Protection in Industrial Workers, and analyzed completeness and timeliness of their submission. Results: We established that occupational health assessment based on the results of periodic medical examinations is complicated by late and incomplete data reporting by healthcare facilities of the Sverdlovsk Region. Conclusions: Our findings prove the importance of creating a “Periodic Medical Examination” subsystem of the unified state health information system. Providing access to standardised results of periodic medical examinations based on a single digital platform to all professional participants of the national occupational health system will enable data recording, analysis and assessment at the interregional and intersectoral levels, as well as at the level of business entities, and facilitate their use for the purposes of workers’ health risk management. Timeliness of reporting and completeness of date entry in the “Periodic Medical Examination” subsystem (or a Federal Registry of Periodic Medical Examinations) may be monitored and verified by local Rospotrebnadzor bodies to be connected to the unified state health information systems of constituent entities of the Russian Federation.

Aim. The aim of the study was to consolidate the principles and rules for building a regulatory and reference space based on the analysis of previous phases of implementation to prioritize the development and to create a single digital contour in health care.Information sources. Data were obtained from the Regulatory Service at the Federal Registry of Regulatory and Reference Information (RRI), regulations of the Russian Ministry of Health in the field of information technology development, and the methodical and conceptual documents of the federal project “Creation of the integrated digital contour in healthcare based on the uniform state health information system”.Results. The stages of creating a single system of regulatory and reference information in the health sector over the past decade were analyzed taking into account the continuity of goals and objectives consistently presented in the conceptual and strategic documents that define the process of creating unique state health information system, as well as the experience of their solution. The analysis allowed to formulate the basic principles of development, namely: central management, structural unity, functional unity, information unity, and prioritization of tasks. Consolidation of principles and rules provided the basis for a scientific and methodical approach to further development of a single RRI system at the stage of creating a single digital contour in health care and shaping priorities of the development.Conclusion. At the stage of creating a single digital contour in health care, which has been approached by many countries, including Russia, the importance of a ubiquitous language of communication increases dramatically and, as the events associated with the COVID-19 pandemic have shown, the readiness of information systems directly determines the readiness of society to meet challenges and to protect the population.

Introduction: Tachydysrhythmias (TDS) frequently occur after complete repair of tetralogy of Fallot (TOF). However, not much is known about the effect of TDS on morbidity and mortality after TOF repair. We sought to assess the associations between TDS and mortality and morbidity after repair of TOF using a multicentre database. Materials and Methods: We identified all children aged 0–5 years in the Pediatric Health Information System who underwent TOF repair between 2004 and 2015. Codes for TDS were used to identify cases. Outcome variables were inpatient mortality and total length of stay (LOS). Univariate and multiple logistic and linear regression analyses were used to identify the effects of multiple risk factors, including TDS, on mortality and LOS. Results: A total of 7,749 patients met inclusion criteria, of which 1,493 (19%) had codes for TDS. There was no association between TDS and inpatient mortality. However, TDS were associated with 1.1 days longer LOS and accounted for 2% of the variation observed in LOS. Conclusion: After complete repair of TOF, TDS were not associated with mortality and appeared to have only a modest effect on LOS.

Objective To determine the incidence of auditory neuropathy spectrum disorder (ANSD) and its risk factors among the neonatal intensive care unit (NICU) population from 2009 to 2018 in the Pediatric Health Information System database. Study Design Retrospective national database review. Setting Population-based study. Methods The Pediatric Health Information System database was queried to identify patients ≤18 years old with NICU admission and ANSD diagnosis. Patient demographics, jaundice diagnosis, use of mechanical ventilation, extracorporeal membrane oxygenation, furosemide, and/or aminoglycosides were extracted. Multivariable linear regression was used to assess trends in incidence. Chi-square analysis was used to assess differences between patients with and without ANSD. Logistic regression was used to assess factors associated with ANSD. Results From 2009 to 2018, there was an increase in (1) NICU admissions from 14,079 to 24,851 ( P < .001), (2) total ANSD diagnoses from 92 to 1847 ( P = .001), and (3) annual total number of patients with ANSD and NICU admission increased from 4 to 16 ( P = .005). There was strong correlation between the increases in total number of NICU admissions and total ANSD diagnoses over time ( R = 0.76). The average ANSD incidence was 0.052% with no statistically significant change over 10 years. When compared with all NICU admissions, children with ANSD had a higher association with use of furosemide ( P < .001) and ventilator ( P < .001). Conclusion Despite a statistically significant increase in NICU admissions and total ANSD diagnosis, the incidence of ANSD in the NICU population has not increased from 2009 to 2018. Furosemide and mechanical ventilator use were associated with increased likelihood of ANSD.
