The case describes the endocarditis of an ASD occluder in a grown-up with congenital heart disease. The patient was at first treated as Adult-onset Still disease because of an infection with unkown focus. Due to the immunosuppressive therapy the infection was at first not related to the endocarditis. After treatment symptoms and signs of Adult-onset Still disease completely vanished.

Cardiac failure and cyanosis are the principal presenting signs in neonates and infants, when most cases are recognized. Others, however, present in childhood, adolescents, or even adulthood with a broader range of symptoms and signs including murmurs, arrhythmia, hypertension and rarely chest pain. Clinical examination, chest X-ray and electrocardiogram are invaluable, but echocardiography has revolutionized precise ‘bedside’ diagnosis of congenital heart disease at all ages.
