Purpose: to analysis the results of microimpulse transscleral diode laser cyclophotocoagulation (DCPC) technique as a method of choice in patients with open-angle subcompensated glaucoma (OAG) with toxic allergic syndrome of the ocular surface (TASOS).Methods. The retrospective analysis of patients’ case reports with subcompensated open-angle glaucoma and TASOS was performed. Transscleral diode laser cyclophotocoagulation (DCPC) in a microimpulse mode was used in their treatment. A total of 19 patients’ case reports were analyzed.Results. In the postoperative period, its course, patient’s complaints, ophthalmological status, visual acuity indicators were determined. The level of intraocular pressure was measured. Almost all patients (84.2 %) reached the target level of IOP. The frequency of side effects was low. In 36.84 % of patients, symptoms of TASOS decreased in response to hypotensive treatment due to reduction of instillation frequency.Conclusion. The use of micro-pulse cyclophotocoagulation in patients with subcompensation form of OAG and toxic-allergic ocular surface syndrome (TASOS) reduces the intensity of the drop regime of antihypertensive drugs. As a result, the manifestations of TASOS are reduced, which increases the patients’ quality of life in the postoperative period. In addition, the use of DCPC in microimpulse mode leads to stabilization of functional and structural indicators, helps to prevent a decrease in visual functions. Thus, the described method is characterized by safety, high efficiency and a wide range of indications for its performance. It allows to compensate the IOP with a minimum percentage of complications and stable postoperative results.

Aim. To conduct a clinical assessment of the incidence of dry eye syndrome (DES) in patients with primary open-angle glaucoma (POAG), depending on the timing of the use of local antihypertensive therapy. Methods. The main group included 45 patients (45 eyes) with POAG. Inclusion criteria - use of local antihypertensive therapy for at least one month; absence of previous laser and microsurgery operations, systemic pathology that can cause DES (thyroid lesions, diabetes mellitus, autoimmune and rheumatoid diseases). The patients’ age averaged 61.2±8.7 years (from 47 to 82 years). There were 29 men and 16 women. The design of the work consisted of a comparative assessment of the frequency of DES formation in the studied groups and subgroups, as well as the degree of its severity. Results. The results of the study showed that with an increase in the duration of the use of local antihypertensive drugs by patients with POAG the incidence of DES development increases. Moderate changes in the studied functional parameters of the ocular surface were recorded as early as 1-3 years from the start of antihypertensive therapy. Especially often DES was formed in the 3rd subgroup and proceeded in more severe clinical forms. Conclusion. The initial statistically insignificant changes in the indicators of the state of the ocular surface in patients with POAG using local antihypertensive therapy took place at the time of their instillation from 1 to 3 years. Most often, according to our data, DES developed in patients with POAG after three years of using antihypertensive drugs (9 out of 15 patients). There are at least 4 reasons for choosing synthetic prostaglandins as the first-line drugs in glaucoma patients: efficacy, stable 24-hour intraocular pressure control, safety and compliance. Such qualities of these antihypertensive drugs are attracting the attention of ophthalmologists around the world. The use of synthetic prostaglandins is also associated with side effects such as eye irritation. One of the ways to solve this problem is to create a preservative-free form of the drug, free of benzalkonium chloride, including latanoprost. Our 10-week studies have convincingly shown that the use of a preservative-free form of latanoprost (monoprost) allows you to be gentle to the ocular surface, to improve the tolerance of the drug by almost 26.7–37.0 % on the Ocular Surface Disease Index. The use of a preservative-free form of latanoprost (monoprost) in patients with primary open-angle glaucoma increases the hypotensive efficacy of the drug by 9.4–15.4 % in comparison with classical latanoprost.

Актуальность. Одним из главных патогенетических звеньев, определяющих характер течения глаукомы, является нарушение гидродинамики глаза. Цель. Сравнительная оценка влияния гипотензивных препаратов на гидродинамику глаза у больных с первичной открытоугольной глаукомой с нестабилизированным уровнем внутриглазного давления (ВГД). Материал и методы. На базе РСНПМЦМГ нами обследованы 60 больных (106 глаз) с ПОУГ I, II и III стадии с нестабилизированным уровнем ВГД и были разделены на 2 группы: 1-группа – 30 больных (52 глаза), которым в качестве гипотензивного средства применялся препарат Офтан-Тимолол 0,5% («Santen») 2 раза в сутки, 30 больным (54 глаза) 2-группы был назначен новый гипотензивный препарат Прессимол («Aseptica») 0,5% 2 раза в день. Динамическое и контрольное исследование гидродинамики глаза проводили до лечения, через 10 дней и через 1 месяц в процессе лечения по методу упрощенной тонографии по А.П. Нестерову. Результаты. В обеих группах отмечалось достоверное снижение коэффициента Беккера (КБ) в среднем в 1,9 раза по сравнению с исходным (р<0,05), а P0 снизилось в среднем на 1,4–1,7 мм.рт. ст. Среднее значение коэффициента легкости оттока достигло 0,49 мм3/мин. Однако, продукция водянистой влаги менялась в значительной степени, на 16-19% от исходного уровня. Выводы. Указанные препараты в обеих исследуемых группах достоверно нормализовали показатели гидродинамики глаз больных глаукомой за счет снижения выработки водянистой влаги.

Patients with primary open-angle glaucoma (POAG) and ophthalmic hypertension have an increased likelihood of developing occlusions (thrombosis) of the central retinal vein. Different groups of antihypertensive drugs differ in their mechanism of action and may affect concomitant ocular pathology, in particular, retinal edema, which occurs, for example, in occlusion of the central retinal vein. Used in most patients with glaucoma, prostaglandin analogs can contribute to the long-term preservation of macular edema due to the effect on the permeability of the vascular wall. Preparations of other pharmacological groups, reducing the production of aqueous humor, on the contrary, may contribute to its regression. Therefore, the question of choosing a drug for antihypertensive therapy in patients with primary open-angle glaucoma and concomitant macular edema is relevant and is for further study.

Оперативное вмешательство - один из основных методов лечения глаукомы. Однако развитие избыточного рубцевания созданных путей оттока определяет результат хирургического лечения в отдаленные сроки. Процессы рубцевания на данный момент недостаточно изучены. Цель исследования - оценка роли матриксной металлопротеиназы-9, ее ингибиторов в процессах рубцевания у больных с первичной открытоугольной глаукомой после оперативного лечения. Методика. Для выявления возможных маркеров избыточного рубцевания методом твердофазного иммуноферментного анализа определяли содержание матриксных металлопротеиназ-9, тканевых ингибиторов металлопротеиназ 2 и -3 в слезной жидкости у 37 пациентов с активной стадией первичной остроугольной глаукомы в динамике послеоперационного периода. Средний возраст пациентов составил 52,8 лет. В зависимости от исхода оперативного вмешательства все пациенты были разделены на 2 группы - с благоприятным исходом (без избыточного рубцевания) и с неблагоприятным исходом (с избыточным рубцеванием) на месте сформированных дополнительных путей оттока внутриглазной жидкости в послеоперационном периоде. Группа контроля включала 20 человек в возрасте от 50 до 66 лет без сопутствующей офтальмологической и соматической патологии в стадии обострения. Результаты. В динамике показано изменение концентрации матриксной металлопротеиназы-9 и ее ингибиторов в послеоперационном периоде. Анализ данных свидетельствует об обратной зависимости уровня матриксной металлопротеиназы-9 и тканевых ингибиторов металлопротеиназы 2 и 3 типов с исходом операции - чем выше концентрация металлопротеиназы-9 и ниже концентрация тканевых ингибиторов металлопротеиназ 2, -3 в слезной жидкости, тем выше вероятность неблагоприятного исхода в виде рубцевания сформированных дополнительных путей оттока внутриглазной жидкости в послеоперационном периоде. Заключение. Мониторинг уровня металлопротеиназ и их тканевых ингибиторов после проведения хирургического лечения пациентов с первичной открытоугольной глаукомой позволяет прогнозировать раннее рубцевание, дает возможность разработки новых методов лечения как в раннем, так и в позднем послеоперационном периоде. Surgery is one of the major treatments for glaucoma; however excessive scarring of created outflow patways affects the long-term outcome. At the present time, scarring processes are not sufficiently studied. Aim. To evaluate the role of matrix metalloproteinase 9 and its inhibitors in scarring after surgical treatment of open-angle glaucoma. Methods. Concentrations of matrix metalloproteinase 9 and tissue inhibitors of metalloproteinases 2 and 3 were measured in tear fluid of 37 patients (mean age, 52.8) with active primary open-angle glaucoma in dynamics during the postoperative period to identify possible markers of excessive scarring. Based on the surgery outcome, all patients were divided into two groups, with a favorable outcome (without excessive scarring) and an unfavorable outcome (with excessive scarring) in the created additional outflow pathways for the intraocular fluid in the postoperative period. The control group included 20 subjects aged 50-66 without eye disease or somatic disease at exacerbation stage. Results. Analysis of changes in concentrations of matrix metalloproteinase 9 and its inhibitors in the postoperative period showed their inverse relationship with the surgery outcome. The higher was the metalloproteinase 9 level and the lower the level of tissue inhibitors of metalloproteinases 2 and 3 the higher was the probability of unfavorable outcome evident as excessive scarring of the formed additional pathways for tear fluid outflow in the postoperative period. Conclusion. Postoperative monitoring of metalloproteinases and their tissue inhibitors allows to predict early scarring and to develop new treatments both in early and late postoperative periods.

Purpose:  To compare the efficacy of the microwave pulse diode laser and argon laser trabeculoplasty in primary open angle glaucoma. Study Design:  Quasi experimental study. Place and Duration of Study:  Shaheed Mohtarma Benazir Bhutto Medical College Lyari and Sindh Government Lyari General Hospital, Karachi, from October, 2017 to March, 2018. Material and Methods:  One hundred and sixty patients, between 42 to 61 years with visual acuity of perception of light to 6/36 were enrolled. Patients diagnosed with POAG were included and patients with intraocular pressure of more than 40 mm Hg, previous glaucoma surgery or laser treatment and narrow angle on gonioscopy were excluded. Ophthalmic examination included visual acuity, slit lamp examination, fundus examination and visual field status using Humphrey perimeter. Patients were divided into two groups. Group A received microwave pulse diode laser (810) and Group B received argon laser trabeculoplasty. Average follow up period was 6 months. Success was assessed objectively by measuring intra ocular pressure and subjectively by visual acuity. Results:  The average time-period for each procedure was 15 ± 5 minutes. In Group A, mean IOP at first week, first month, third month and sixth month was 20.79, 16.34, 16.21and 16.09 mm Hg respectively. While in Group B, IOP at first week, first, third and sixth month was 16.52, 15.76, 13.62, and 12.54 mm Hg at (P < 0.001 in both groups). Conclusion:  Both microwave pulse diode laser and argon laser trabeculoplasty are effective in lowering intra ocular pressures in patients with primary open angle glaucoma.
