Aim      To evaluate the frequency of off-label prescription of medicines in practice of clinical specialists and the awareness of respondents of the procedure of justified off-label prescription.Material and methods  The sample included 542 clinical specialists who worked in definite medical organizations in 26 entities of the Russian Federation. The respondents were proposed to fill in remotely an anonymous questionnaire to evaluate the experience of prescribing medicines off-label to adult patients.Results Prescribing medicines not in consistence with the officially approved instruction for medical use (off-label or “outside instruction”) is a relevant issue of global medical care since convincing scientific evidence for safety of such use is scarce. Analysis of information about off-label prescription is one of current tasks of national medical research centers according to the Federal Project “Development of a network of national medical research centers and implementation of innovative medical technologies”. According to the responses about the frequency of off-label prescriptions 67.5 % of respondents reported of no experience of off-label prescription, 27.7 % said “rarely” or “sometimes”, and 4.8 % said “frequently” and “very frequently”. Specialties of physicians who have more often used medicines off-label (50% and more) included obstetrics and gynecology, pediatrics, rheumatology, hematology, and pulmonology. Cardiologists, neurologists and clinical pharmacologists use medicines off-label relatively rarely (19.6%, 28.6 %, and 22.2 %, respectively). 40 % of medicines used off-label were those designed for the treatment of coronavirus infection SARS-CoV-2. The medicines most frequently used off-label included metformin, rituximab, and thioctic acid. 65 % of respondents assessed their knowledge of off-label prescription as insufficient. In addition, 75 % of respondents consider it useful to receive additional information about risks and benefits of off-label prescription in clinical practice.Conclusion      The survey revealed the need of physicians for information about risks of the off-label use of medicines in clinical practice.

This paper describes the work experience gained at the Federal State Budget Institution «National Medical Research Center of Radiology» (FSBI «NMRC of Radiology») of the RF Health Ministry in 2015-2016. Information about 94 grants received by researchers by means of the use of domestic and international information resources stored on electronic and print media is presented. The results of processing and analysis of databases enabled to characterize the personnel activity in branches to receive financial support for research in the areas of medical radiology, radiobiology, radiation epidemiology and oncology as well as to make a decision and highlight problems in funding of projects.

The basis for constructing a vertically integrated management system in the healthcare sector in the Russian Federation is the model of functioning of national centers. One of the key components in achieving success is the interaction of the national center with a leading national society. The aim of the article is to present the experience of joint work of the National Society of Pediatric Hematologists and Oncologists (NSPHO) with the Dmitry Rogachev National Medical Research Center of Pediatric Hematology, Oncology and Immunology in the field of development of pediatric oncology-hematology in the Russian Federation. In order to accomplish the task of building a system for organizing medical care for children in pediatric oncology-hematology, a comprehensive development program was implemented, which included collecting data on the structure of the service through questionnaires in the regions, obtaining expert opinion and the subsequent creation of a service passport using data visualization. In addition, the experience of creating professional standards and the concept of a unified approach in the field of training in the specialties “Pediatric Oncology” and “Hematology” using the technology of continuing medical education are reflected. The result of the work was the formation of a single plan for the organization of the service of pediatric oncology-hematology, developed by the NSPHO in conjunction with the Dmitry Rogachev National Medical Research Center of Pediatric Hematology, Oncology and Immunology with the support of the National Medical House and under the leadership of the Ministry of Health of Russia.

Novel coronavirus infection, causing COVID-19, is not fully controlled for today. In periodicals, there are often reports of a high risk of severe complications in cancer patients during the development of COVID-19. The review presents information from literary sources based on the experience gained in overcoming this formidable disease and its consequences using the examples of foreign colleagues of clinicians, as well as domestic experience and recommendations for the treatment of cancer during the pandemic COVID-19 of the National Medical Research Center for Radiology of the Ministry of Health of Russia with the participation and support of the Association of Oncologists of Russia and the Association of Health Organizers in Oncology. The information is based on actual data collected through the search of sources in abstract and sciencemetric electronic databases, open access resources in Russian and English. The review presents recommendations from international and domestic professional communities regarding the routing and multidisciplinary treatment of cancer patients during the pandemic period.
