The occurrence of post-operative fever after myomectomy without an apparent infectious cause has been reported in various studies. We here in report a case of an 18-year girl with abnormal uterine bleeding and severe anemia resulting from a large 5 cm submucosal fibroid. She underwent laparoscopic myomectomy. Surgery was uneventful. Post-operative she had fever of 100.4°F which lasted for 48 hours. Antibiotic coverage was continued for 48 hours. Her urine culture and hemogram were negative for infection. Early postoperative fever is most commonly caused by inflammatory changes rather than infectious causes. Large submucous fibroids can be managed by laparoscopy with excellent clinical outcomes and minimal morbidity.

<p class="abstract"><strong>Background:</strong> Abnormal uterine bleeding is the cause of concern, inconvenience and discomfort to many ladies, this affects millions of women in both developed and developing world.  Until recent times, dilatation and curettage was the usual method of evaluation .This detects the cause of bleeding in less than 50% of the cases, but hysteroscopy is a better diagnostic modality and prompt treatment can be provided in the same setting.</p><p class="abstract"><strong>Methods:</strong> <span lang="EN-IN">A prospective study was carried out in the Department of Obstetrics and Gynecology at Dhulikhel hospital, Kathmandu University Hospital, Kavre, Nepal from January 2010 to January 2016. Two hundred and eighteen cases were enrolled in this study. Patients with abnormal uterine bleeding above the age of 18 years were enrolled. Hysteroscopic examination was done in all patients after the pregnancy was ruled out with urine pregnancy test and ultrasonology. The patients then underwent hysteroscopic guided endometrial sampling and endometrium was sent for histopathologic examination. The correlation between findings on hysteroscopy and histopathologic examination was tabulated. </span><span lang="EN-IN"> </span></p><p class="abstract"><strong>Results:</strong> Following were the findings on hysteroscopy: proliferative endometrium was in 59%, secretary   endometrium was in 14%,endometrial hyperplasia in 12%, atrophic in 2 %, endometrial polyp in 7%, submucous myoma in  5% and carcinoma of endometrium in 1%.</p><p class="abstract"><strong>Conclusions:</strong> In patients with abnormal uterine bleeding, hysteroscopy provides more accurate diagnosis than dilatation and curettage<span lang="EN-IN">. </span></p>

Background: Aim of the study was to analyze retrospectively the efficacy of hysteroscopy in the diagnosis of abnormal uterine bleeding (AUB).Methods: Eighty-six women in the reproductive and perimenopausal age group (20-50 years) visiting the gynaecology OPD from March 2018 to February 2019 with the complaints of abnormal uterine bleeding were enrolled in the study. All the patients who underwent hysteroscopic examination were subjected to endometrial curettage; which was sent for histopathological examination (HPE). The hysteroscopic findings were then corelated and compared with HPE.Results: Mean age of the patients was 37.2 years. Around 60.46% patients presented within six months of complaints. Clinically, 54.65% were diagnosed as menorrhagia, 37.2% as polymenorrhoea and 8.13% as intermenstrual bleeding. On hysteroscopy, 44.1% showed abnormal pathology. The positive findings including polyps (8.13%), calcification (3.48%), submucous myoma (12.79%), necrotic mass and forgotten IUCD (2.32%) and adhesions in one case. On the other hand, the findings of histopathology; 56 patients (65.11%) had normal / proliferative / atrophic endometrium, 17 (19.76%) had hyperplasia, 10 (11.62%) had polyps and 3 (3.48%) had calcified endometrium. There was no significant difference between two modalities for normal / proliferative / atrophic endometrium. The HPE diagnosed slightly higher patients of hyperplasia as compared to hysteroscopy. Hysteroscopy diagnosed a higher number of patients with submucous myoma and necrotic mass.Conclusions: Hysteroscopy provided additional visual information for some pathologies which otherwise would remain undiagnosed by HPE.

30 girls aged from 11 to 17 years were surveyed in order to research the use of gels for the intimate hygiene containing xylitol, alginates and allantoin combined with chamomile extract or quince that have a neutral pH and do not contain perfumes. The average age of pa-tients was 15.2 ± 2.8 years (11; 17). Surveyed patients used to have the following clinical diagnosis: ovarian cysts - 8 patients , ovarian apoplexy - 8, acute salpingo-oophoritis - 1, abnormal uterine bleeding - 13. Patients were asked to evaluate the following quality criteria according to the scale from 1 to 5 scores: simplicity of use, comfort after hygienic procedures, the feeling of freshness, the perfume. Mean score for simplicity was 4.5 ± 0.7 (2; 5), comfort after the procedure - 4.5 ± 0.7 (3; 5), feeling of freshness - 4.6 ± 0.5 (3; 5) and perfume - 4.1 ± 0.9 (2; 5). 7 of 30 patients (23.3 %) estimated the perfume as scores 2 and 3. Lowest score 3.7 ± 1.2 was given by youngest girls aged 11-13 years (n = 6). Girls aged 14-15 years (n = 9) gave the higher score that equaled 4.1 ± 0.8 and girls aged 16-17 years (n = 15) gave the highest mean score of 4.3 ± 0,7. 22 (73.3 %) of 30 girls almost over 15 years old said they intend to use this hygienic agent in the future. 8 (26.7 %) younger girls found this difficult to assess. Thus, attractiveness of this agent for personal hygiene was lower in girls from younger age group (11-13 years) due to the absence of perfume and in spite of numerous assets.
