Cutaneous horn is a conical, circumscribed, dense hyperkeratotic protrusion from skin with epithelial cornification. It is also known by the Latin name ‘Cornu cutaneum’. This rare medical entity resembles animal horn but histological disparity is present between both. They are more commonly present in sun exposed sites or areas that are prone for actinic radiation, burns and hence frequently seen in forearm and upper part of face. Only few cases have been reported with cutaneous horns in unusual sites. Cutaneous horns occurring in oral cavity or perioral regions are extremely rare. The significance of knowing about this dead keratinous cutaneous horn is that it may occur as a part of or in association with a wide range of underlying pathologies, either malignant, premalignant or benign. Majority are due to benign pathologies. We report an unusual presentation of cutaneous horn in left oral commissure of a 45-year-old gentleman which is an extremely rare perioral location for such an ailment.

Background. A case report of a nonhealing ulcer of the tongue histologically proven to be adenocarcinoma.Method. A 92-year-old man underwent clinical, immunochemistry, and imaging investigation.Results. Tests confirmed a distant metastasis of a primary colorectal carcinoma.Conclusion. Metastasis from colorectal carcinoma to the oral cavity is primarily to bone, but non-healing ulcers of the oral cavity should be considered in differential diagnoses.

Urticaria multiforme is a condition which manifests as acute, polycyclic, annular oedematous pink plaques with an ecchymosis hue that is associated with acral edema. The condition is often misdiagnosed as erythema multiforme, serum-sickness-like reactions, or urticarial vasculitis. Author present a case of acute annular urticaria in a 3-year-old girl who presented with unusual clinical manifestations of the condition. Through this case report, Author aim to emphasize the wide range of morphologic manifestations that can be seen in urticaria multiforme. This can assist pediatric physicians to differentiate urticaria multiforme from other clinical dermatologic conditions and prevent misdiagnosis. A detailed case history and physical examination, along with relevant diagnostic tests can enable prompt and effective management of the condition.

Cutaneous horn is a conical, dense, and hyperkeratotic protrusion that often appears similar to the horn of an animal. Giant cutaneous horns are rare; no incidence or prevalence has been reported. The significance of cutaneous horns is that they occur in association with, or as a response to, a wide variety of underlying benign, premalignant, and malignant cutaneous diseases. A case of giant cutaneous horn of left oral commissure along with carcinoma left buccal mucosa is reported here as an extremely rare oral/perioral pathology.

Fusion and gemination are not entirely uncommon dental anomalies that may also be named ‘double teeth’. This is due to their unusual presentation in a twin-like fashion. Teeth with these irregularities may appear clinically similar, presenting as large and bulbous in shape with unusual anatomy and fissure patterns. The exact aetiology of these anomalies is uncertain, but many potential local or systemic disturbances during the morphodifferentiation stage of odontogenesis have been considered. Gemination occurs rarely in the permanent dentition and even less frequently in the posterior region of the oral cavity. This report presents a case of a geminated mandibular second molar and discusses the importance of early recognition and diagnosis of such teeth so that adequate preventative care and treatment can be provided.

Besides respiratory symptoms, COVID-19 disease has a wide range of clinical, subclinical, and atypical presentations reported previously. Here, we report the case report of a middle-aged man, with no previous known medical illness, who presented with a 5-day-history of anxiety, fever, confusion, and hallucinations. Patient’s SARS-CoV-2 polymerase chain reaction test was positive, and he underwent daily vital signs and respiratory, cardiovascular, and abdominal examinations. Chest radiography, electrocardiogram, microbial culture, biochemistry, and toxicology tests were also investigated. In this report, a case of COVID-19 is described with an unusual presentation of confusion and hallucinations in the absence of severe upper respiratory or constitutional symptoms. The earlier recognition of atypical manifestation, the safer the practice, with optimal timely diagnosis, and less anticipated outbreaks in healthcare facilities. Further studies are needed to establish the underlying pathophysiological mechanisms involved.

Aerodigestive injury due to a fishbone foreign body is a widespread emergency department case. It presents with a wide range of manifestation; however, occasionally, we come across rare complaints. The Difference

There is a wide range of hymenal variants which exist and usually become apparent to women during puberty. Our case describes a 16-year-old woman with a previously undiagnosed hymenal septum who presented with a retained tampon within the urinary bladder. This case report discusses a rare presentation of a hymenal variant and highlights the importance of understanding the difference in pelvic examinations in children and adolescents when compared with adults and awareness of the different hymenal variants.
