ABSTRACT OBJECTIVE To assess the construct validity and reliability of the Portuguese version of the European Task Force on Patient Evaluation of General Practice Care questionnaire. METHODS We applied the Portuguese version of the European Task Force on Patient Evaluation of General Practice Care to 392 users of 20 Family Health Units from the North of Portugal. The validity of the construct was evaluated by exploratory factor analysis, with the Principal Axis Factoring method, by orthogonal rotation (varimax procedure), by the Kaiser normalization criteria (eigenvalue ≥ 1). The factorability of the data matrix was verified by the Kaiser-Meyer-Olkin and Bartlett’s sphericity test. We estimated the reliability by the indicator of internal consistency Cronbach’s alpha. To analyze the correlations between satisfaction and loyalty, we used the Pearson correlations. The predictor effect of satisfaction on loyalty was analyzed by simple linear regression. RESULTS Satisfaction presented five robust and well individualized dimensions – medical care, nursing care, clinical secretariat services, accessibility, and organization of services – with alpha values between 0.86 and 0.97, good levels of internal consistency. The loyalty showed alpha value of 0.72, considered a reasonable internal consistency. The satisfaction was predictive of loyalty. CONCLUSIONS The Portuguese European Task Force on Patient Evaluation of General Practice Care questionnaire is a robust and reliable instrument to measure the satisfaction and loyalty of users of the Family Health Units.

This issue of the Australian Journal of Primary Health features a selection of papers about general practice in Australia. These papers reflect a range of perspectives important in general practice, including disease management in the general practice context, innovative approaches to prevention, consumer perspectives on conditions and services, future development of general practice in the primary health care system, and more. Publication of these papers occurs at a time when development of the National Primary Health Care Strategy is well under way. The focus of the National Primary Health Care Strategy is on related themes of prevention, chronic disease, evidence-based practice and better integration of general practice into the primary health care system.

INTRODUCTION: It is recognised that patients who are culturally and linguistically diverse (CALD) have challenges in accessing health care and understanding health advice or instructions. Those with multimorbidity (MM) are likely to have additional difficulties. In New Zealand, little is known about how this patient group view their health and general practice health care. This study examined the views of multimorbid CALD patients about MM and the health care available in a Very Low Cost Access general practice. METHODS: This qualitative study recruited Samoan, Cook Island Maori, and Cambodian patients with diabetes and more than three other long-term conditions. Two individual interviews and two language-specific focus groups were undertaken to yield themes representing the experience of these CALD patients with MM. FINDINGS: Participants described MM as having considerable impact on their life. They reported feeling responsible for supporting their own health and many detailed self-management techniques. However, they also expressed confusion, lack of information and limited understanding of MM, in particular about managing medication. Not all patients were aware of the range of available general practice services and some described difficulties in accessing general practice care. CONCLUSION: Despite being motivated to self-manage, this patient group report challenges in understanding their conditions and how to manage them, which may also be influenced by health beliefs. Available general practice services are not well known by CALD patients with MM. There is likely to be value in developing a specific structured, yet patient-centred, model of care for this group of patients. A satisfação com cuidados de saúde é conceito multidimensional que considera aspectos como acesso, organização, interação usuário-profissional.O objetivo do estudo foi adaptar e aplicar na Estratégia de Saúde da Família (ESF) um instrumento de avaliação da satisfação do usuário em Atenção Primária à Saúde (APS), tendo por base o European Task Force on Patient Evaluation of General Practice Care (EUROPEP), que refere-se à satisfação dos usuários de serviços de medicina geral e familiar (composto por cinco dimensões de satisfação: relação e comunicação, cuidados médicos, informação e apoio, continuidade e cooperação, e organização dos serviços). O estudo foi composto por fases: revisão, adaptação do instrumento e aplicação a uma amostra representativa dos usuários da ESF no município do Rio de Janeiro. Para análise dos resultados foram calculadas médias das proporções de respostas para cada indicador, sendo Relação e Comunicação entre profissional e usuário o que mais se destacou e o de Organização dos Serviços recebeu a pior avaliação. Em relação à escolaridade, boa autopercepção de saúde e mais idosos mostraram-se mais satisfeitos. O instrumento mostrou ser de fácil aplicação, podendo ser utilizado rotineiramente para monitoramento da ESF, sendo importante ferramenta para a institucionalização da avaliação.
