Most pancreatic neuroendocrine tumors (NETs) are detected incidentally and arise in the endocrine tissues. NETs can secrete hormones and result in clinical syndromes. However, between 50 and 75 percent of pancreatic NETs are nonfunctioning. Ultrasonography shows a well-circumscribed mass with a smooth margin and round or oval hypoechoic shape. A 38-year-old woman visited our hospital with mild upper abdominal discomfort. We visualized an oval hypoechoic mass with inner hyperechogenicity after water intake in the stomach and position change. The patient underwent surgery, and the mass was diagnosed as pancreatic NET.

Oesophagogastric invagination is a relatively rare disease that is primarily caused by a sliding hiatal hernia. We report a successfully treated case of oesophagogastric invagination caused by achalasia. Oesophagogastric invagination should be considered in patients complaining of upper abdominal discomfort.

Clonorchis (syn. Opisthorchis) sinensis is a fluke (flatworm) acquired by ingestion of undercooked freshwater fish in eastern Asia. Larvae in the duodenum enter the biliary tree through the sphincter of Oddi and mature. Most patients are asymptomatic, but there may be right upper abdominal discomfort, and infection can be complicated by bacterial cholangitis and there is an increased risk of cholangiocarcinoma. Diagnosis is suggested by finding eggs in faeces or in duodenal aspirates, but can only be confirmed by examination of adult flukes. Treatment is with praziquantel....

Temporal and spatial tumor heterogeneity can be observed in pancreatic neuroendocrine tumor. We report the case of a young woman with long term stabilization of a G2 metastatic pancreatic NET that, after pregnancy, suddenly progressed into one single liver metastasis corresponding to a transformation into G3 large-cell neuroendocrine cancer. The patient underwent liver resection (the progressive and one dormant metastasis). With a 45 months follow-up the patient is without evolutive disease. Exome sequencing of the two metastases revealed completely different genomic signatures and gene alterations: the dormant metastasis was MSS without any gene alteration; the poorly differentiated tumor was MSI, with gain of many mutations including MEN1, BCL2, MLH1 and TP53 corresponding to a mutational signature 11. Could temozolomide play a role in this transformation?

Papillotubular adenoma of the ampullary region in a rare tumor which has the potential for malignant transformation. Although reported more frequently nowadays, ampullary tumors are difficult to diagnose before operation unless they cause obstructive jaundice. Occasionally they are detected by ultrasonography, computed tomography or ERCP in patients who complain of nonspecific upper abdominal discomfort.We report a case of a periampullary papilotubular adenoma causing generalized pruritus and weight loss but no jaundice. The tumor was extirpated by pancreaticoduodenectomy, and the patient has remained healthy for over five years.

Carcinoma stomach- Captain of men of death, is the first diagnosis we have across any patient presenting with pallor, epigastric lump and loss of appetite. We present the case of a 47 year old lady with upper abdominal discomfort, anemia and a lump in the upper abdomen and family history of carcinoma stomach, suspected even intra operatively to have carcinoma or gastro intestinal stromal tumour(GIST) of stomach, to be surprised by the histopathology of a benign schwannoma. GIST, a more common pathology of stomach, has malignant behaviour upto 30%. On the other hand, schwannomas are more benign and have excellent prognosis. There lies the importance of differentiating the two.

Objective : Pancreatic NET and GIST represent rare neoplasms. This is a case report on a patient operated for PNET with incidentally found jejunal GIST in our unit. Results : Patient underwent enucleation for pancreatic NET and jejuno-jejunal resection and anastomosis for GIST. Postoperatively patient was prescribed imatinib. Post operative period and followup were uneventful. Conclusions : Sporadic non-functioning PNET was an incidental diagnosis, while jejuna GIST was intraoperative incidental diagnosis. Prognosis predominantly depends upon type, size and stage of presentation of PNET, as GIST was asymptomatic.

Mesothelial cells constitute the lining of the three serous sacs of the body i.e. the pleura, pericardium and peritoneum. The more common type of malignant neoplasia of the serous sacs is seeding by metastatic tumors and primary malignancy of the mesothelium is unusual. Of the three sacs, the pleura is the most common site of malignant mesothelioma. Involvement of the peritoneum is extremely rare.We report 3 cases of malignant mesothelioma of the peritoneum. All of them were female. Their ages were 57, 67 and 72 years, respectively. The patients presented with abdominal discomfort and/or ascites. The extent of the tumors ranged from a peritoneal implant to widespread infiltration of the peritoneum and omentum. Histologic examination in Case 1 showed the presence of a diffusely infiltrating papillary mesothelioma without a sarcomatoid component. A mesodermal element was present in the other two cases. In order to establish a morphological diagnosis of malignant mesothelioma, the possibility has to be excluded of a metastatic adenocarcinoma.
