Cognitive-Behavioral Treatment of Schizophrenia: A Case Study

Cognitive-behavioral treatment (CBT) has rarely been applied as the primary treatment for the multiple, severe and persistent problems that characterize schizophrenia. This case study describes the process of CBT in the long-term outpatient care of a young woman with schizophrenia. The study highlights the adaptation of cognitive-behavioral strategies to the unique needs of schizophrenia and presents data regarding clinical outcomes in this case. Measures of psychosocial functioning, severity of symptoms, attainment of treatment goals and hospitalization data were used to assess change over the 3- year treatment period and at 1-year follow-up. Results indicate considerable improvement in all outcome variables and maintenance of treatment gains. These findings suggest the potential usefulness of cognitive-behavioral interventions in the treatment of schizophrenia.

Studies of the outcome of cognitive-behavioral treatment of clients diagnosed with personality disorders are few and need further replication. This case study examines how Beck’s (Beck, Freeman, & Associates, 1990) and Young’s (Young, 1990; Young & Lindemann, 1992) schema-focused approach offers a helpful framework to use with an Avoidant Personality Disorder (APD) client. The findings point to the value of cognitive/behavioral strategies in restructuring Early Maladaptive Schema (Young, 1990) historically associated with APD, and in modifying many of the behavior patterns characteristic of APD.

In behavioral medicine, there is adequate support for behavior modification strategies in reducing depression and anxiety in medical patients. There is comparably less support for cognitive interventions with these patients. Treatment outcome studies with cancer patients generally support the efficacy of cognitive–behavioral interventions, but studies have significant methodological limitations. Additionally, cognitive–behavioral therapy for depression incorporates numerous treatment components, and when examining data with nomothetic statistics, important individual differences may go unrecognized. With reference to the sudden gain literature on significant session-by-session treatment gains, this paper highlights the cognitive–behavioral treatment of two breast cancer patients with clinical depression. In addition to positive treatment outcome in which depression was significantly reduced and quality of life and medical outcomes improved, session-based sudden change data suggest that for these cancer patients, cognitive interventions were most significant in accounting for treatment gains. Findings are reviewed in the context of mechanism of change issues in the cognitive–behavioral treatment of depression.

RAINBOW: A Child- and Family-Focused Cognitive-Behavioral Treatment for Pediatric Bipolar Disorder is a comprehensive, evidence-based treatment manual designed specifically for children ages 7–13 with bipolar spectrum disorders and their families. Intended for qualified child-focused mental health professionals, this manual includes the conceptual background of the treatment and user-friendly step-by-step instruction in delivering RAINBOW with families, including handy session outlines and engaging worksheets for the child and caregiver(s).

Chapter 3, “Cognitive Behavioral Therapy: Theory and Practice,” presents the history, examines the theoretical underpinnings, and explains the essential skills and techniques of cognitive behavioral therapy (CBT). Theoretical principles, such as cognitive distortions, underlying assumptions and schema, and their presentation in older adults, are discussed. The treatment approach of CBT is outlined, including the nature of the therapeutic relationship, changing cognitions, behavioral strategies, the use of homework in treatment, and special considerations and adaptations for practice with older clients. Various contexts and settings where CBT is implemented are summarized, such as individual and group settings within community-based, acute-care, and long-term-care facilities.
