The whirlpool sign predicts cases of testicular torsion with the preserved flow: a case series

Abstract Background To investigate the diagnostic characteristics of high-frequency ultrasound with color doppler flow signal in pediatric testicular torsion to increase the diagnostic accuracy.Methods Seven pediatric patients from October 2017 to August 2019 with preserved blood flow signal but surgically diagnosed as testicular torsion were retrospectively included into the study. The imaging manifestations of high-frequency ultrasonography were evaluated.Results Seven cases of testicular torsion with preserved blood flow aging from 49 days to 15 years old were included in the study. All the cases had preserved blood flow in testis, but the surgical findings showed various twist degrees from 90° to 540°. Preoperative ultrasound showed spermatic cord distortion in all cases, and the testicular long axis tilted in four cases.Conclusion In some testicular torsion cases, color doppler may show normal or increased blood flow signals in the testis. The “whirlpool sign” in the spermatic cord is an important indicator suggestive of testicular torsion.

Abstract Background: Different treatments with varying clinical efficacies have been applied to treat knee osteoarthritis (KOA). This study observes the effectiveness of deep hyperthermia combined with dynamic traction in KOA treatment. Methods: Deep hyperthermia and dynamic traction was administered to 60 patients with KOA. The pretreatment and 20- and 40-day post-treatment visual analog scale (VAS) and Lysholm score, knee joint cavity effusion, synovium thickness, and synovial blood flow signal (by high-frequency ultrasound) of the patients were recorded. The pretreatment and 20- and 40-day post-treatment Lysholm scores were 47.17 ± 5.07, 63.22 ± 3.44, and 81.98 ± 3.30 (P < 0.001), respectively. Results: The pretreatment and 20- and 40-day post-treatment VAS scores were 5.67 ± 0.91, 2.92 ± 0.81, and 0.60 ± 0.72 (P < 0.001), respectively. The pretreatment and 20- and 40-day post-treatment articular effusion was 5.41 ± 0.46 mm, 4.34 ± 0.34 mm, and 2.32 ± 0.32 mm (P < 0.001), respectively. The pretreatment and 20- and 40-day post-treatment synovial thickness was 3.44 ± 0.31 mm, 2.79 ± 0.15 mm, and 2.32 ± 0.32 mm (P < 0.001), respectively. The pretreatment and 20- and 40-day post-treatment grades of synovial blood flow signal exhibited a significant difference (P < 0.001). Conclusion: Deep hyperthermia combined with dynamic traction can significantly improve the clinical symptoms and pain degree in KOA, reduce joint cavity effusion, synovium thickness, and internal blood flow, and the effect improves with time. The changes in soft tissues observed by high-frequency ultrasound are reliable and consistent with clinical signs.

In the period between 1994–96, 16 patients with suspected testicular torsion confirmed during operation, underwent color-Doppler sonography of the spermatic cord after local objective examination. The sonography showed a reduced or absent arterial flow in 15 patients and an increase in flow in 1 patient. In our study, color-Doppler sonography proved to have a sensitivity of 93.7%.

In order to investigate the characteristics of high-frequency ultrasound for the patients with thyroid nodules (TN), and we use the Thyroid Imaging Reporting And Data System (TI-RADS) to classify all the TN. Combined TIRADS classification with the features of color Doppler ultrasound and Doppler blood flow parameters for each nodule, we tried to diagnose TN between benign and malignant nodules so as to provide clinical physicians a more appropriate method and decision-making strategy for patients with of TN by ultrasound findings. The pulse Doppler technique was used to measure the peak systolic velocity (PSV) and resistance index (RI) of the blood flow within the nodule or closely around. The final pathology results were compared with all the ultrasound test. The diagnostic coincidence rates for B&M-TN were statistically compared by single two-dimensional ultrasound or by single color Doppler flow imaging or by combined technique of both. It can be seen that the combination strategy can optimize the coincidence rate of clinical diagnosis of B&M-TN. Therefore, the differential diagnosis of TN by high-frequency ultrasound TI-RADS classification combined with color Doppler blood flow parameters deserves our attention and further study.

AbstractAcute testicular pain in childhood can be caused by testicular torsion, torsion of the appendix testis, or epididymo-orchitis. Quick and reliable diagnosis is essential for determining the further course of action (surgery or conservative approach). The diagnostic tool of choice is high-resolution sonography with a linear transducer (> 10 MHz) combined with color and spectral Doppler sonography. The Doppler device settings should include a low pulse repetition frequency (< 4 cm/s), a low wall filter (< 100 Hz), and adequate gain. Comparison with the unaffected healthy testis is essential. The most important of the three diseases is torsion of the spermatic cord because it requires immediate surgical intervention and detorsion. The affected testis is enlarged and has an inhomogeneous echotexture with hypoechoic and hyperechoic areas as well as an associated hydrocele. In testicular torsion, color Doppler shows reduced or absent intratesticular vessels in comparison with the healthy contralateral testis. Spectral Doppler shows decreased flow velocities especially during diastole in intratesticular arteries and an increased resistance index. The investigation should always include imaging of the spermatic cord from the outer inguinal ring to the upper pole of the testis. In contrast to a normal finding, the vessels and the ductus deferens are not displayed as linear tubular structures but in form of a spiral twist. Ultrasound shows a target-like structure with multiple concentric rings. Color Doppler sonography shows the typical whirlpool sign. In torsion of the appendix testis, the appendix testis is enlarged in the groove between the testis and epididymis. The longitudinal diameter of the appendix testis can be greater than 5 mm. The echogenicity of the torsed appendage can vary between hypoechoic (acute torsion) and hyperechoic (prior torsion). An associated hydrocele of varying size is usually seen. Color Doppler sonography reveals a lack of perfusion of the enlarged appendix testis and increased vascularity of the testis and primarily the epididymis. Epididymo-orchitis is characterized by an enlarged epididymis and/or testis with inhomogeneous echogenicity (hypoechoic – hyperechoic). Color Doppler sonography shows increased vascularity in comparison with the unaffected testis. Spectral Doppler reveals increased diastolic flow velocities and a decreased resistance index. Idiopathic scrotal edema and an incarcerated inguinal hernia must be ruled out in the differential diagnosis.

The diagnostic significance of the color Doppler study of testicular blood flow in testicular torsion has been studied for about 40 years, however, the prognostic value of the results obtained is ambiguous, as are the actual imaging capabilities of the technique. Our own experience of 110 observations of testicular torsion in children older than the neonatal period allows us to draw statistically reliable conclusions. Made on equipment premium-class research made it possible to evaluate how visualization intratesticular vascular pattern, and whirpool-sign and submit the primitive model of the actual inversion, explaining the impossibility of rendering whirpool-sign with the greatest tight inversion. The dynamics of testicular blood flow was studied immediately after successful and unsuccessful manual detorsion and in the long term after operative detorsion and orchopexy. The publication contains a brief analysis of the literature and is extensively illustrated.

Aim: To evaluate the morphostructural aspects and nail vascularity in the nail unit of patients with psoriasis, and to evaluate whether there are differences among psoriatic patients with and without nail involvement. Material and methods: Nail plates and nail bed changes, nailfold vessel resistance index (NVRI), power and color Doppler blood flow appearances were investigated in 23 patients with moderate-to-severe psoriasis, with and without nail involvement, and compared to those of 11 healthy participants. Results: Ventral nail plate deposits were present only in psoriasis patients. Irregular or totally fused nail plates and increased nail plate thickness was frequently observed in psoriasis patients compared to controls. NVRI was increased in psoriatic patients’ nails compared to controls (0.62 vs. 0.57, p<0.0001). In the psoriasis patient group there was significant statistical difference in NVRI in patients with nail involvement compared to those without (0.66 vs. 0.55, p<0.0001). Conclusions: High-frequency gray scale sonography provides valuable information regarding morphostructural changes in nail unit structure in patients with psoriasis. Power Doppler imaging enables blood flow assessment in psoriasis nail induced changes.
