Abstract Purpose: Allergen sensitization are common allergic diseases worldwide, with widely varying estimates of prevalence in the population. The characteristics and prevalence of allergens and diseases in Shenzhen. Patients and methods: Allergen-specific IgE results were investigated among 20483 subjects from January 2016 to June 2020 based on by Western blotting, and statistical analysis was performed on sIgE and related diseases in patients with different ages, genders, months, mixed allergens and grade. Results: Among 20483 subjects, 13149 (64.19%) were clinically positive for at least single allergen. Single allergen positive rate (35.42%), double allergen positive rate (23.96%), triple allergen positive rate (16.04%). The positive rate of sIgE in male was higher than that in female (69.15% and 60. 02% respectively, P<0.01). Allergen detection rate varies with age (P<0.05). The cumulative positive results were 35083, grade 1: 16705 items, 47.61%; grade 2: 8003 items, 22.81%; grade 5: 3840 items, 10.94%. The positive rate of sIgE in March was the highest (82.20%), which was statistically significant compared with other months (P<0.05). The probability of cockroach allergen causing atopic dermatitis/urticaria was 67%, and asthma in just 4%. Conclusion: Our findings reveal that the positive rate of allergens was high in Shenzhen. Dust mite was the main allergen. There were differences in age, gender and month, and the probability of allergic diseases such as atopic dermatitis/urticaria aznd rhinitis/sinusitis is high.

Background. Malformations in epidermal barrier in children with atopic dermatitis (AD) can cause transcutaneous sensitization with further development of allergic diseases that can worsen the AD course and significantly reduces patients’ quality of life.Objective. The aim of the study was to determine the effect of topical treatment and maintenance therapy with pimecrolimus 1% cream (PIM) and topical glucocorticosteroids (tGCS) in infants with AD on reducing the risk of developing transcutaneous sensitization (due to the levels of specific IgE to the cow milk protein over time) and on reducing the disease severity (by the EASI scale).Methods. The study included children aged from 1 to 4 months with early manifestations of moderate and severe AD. The severity of AD was estimated via the EASI scale at start of observation, then at 6, 9 and 12 months of life. The class and level of specific IgE to cow milk proteins (CMP) were determined by the ImmunoCAP method at the point of enrolment and at the ages of 6 and 12 months. Statistical analysis of studied indicators dynamics and their comparison in research groups was carried out using multifactorial dispersion analysis.Results. The study included 36 patients. All patients have received standard tGCS therapy in combination with emollients (wet wrap) for 10 days. The maintenance therapy was prescribed in postacute period. It included topical calcineurin inhibitor PIM 2 times/day for 3 months, then double application (morning/evening) 3 times/week up to the age of 1 year old (group 1). Other group had maintenance therapy — tGCS2 times/week for 3 months, and then at AD aggravation (group 2). Group 1 has shown lower level of sensitization to CMP at the age of 6 and 12 months and more significant decrease in AD severity according to EASI scale compared to group 2.Conclusion. The treatment with PIM is effective in therapy of AD and prevention of transcutaneous sensitization in infants.

Thirty children from infancy to 12 years suffering from atopic dermatitis were evaluated for food hypersensitivity by means of history, skin prick test, total eosinophils count, and elimination of suspected food. Sixteen (53%) patients had history of allergy to suspected food, the other 16 (53%) had ether allergic diseases. Of the 30 patients, 15 (50%) had one of the parents with allergic diseases, and in 3 patients both parents suffered from a1Iergic diseases. Nineteen (6:3-1.) children had atopic dermatitis triggered by food; egg accounted for 400/o, fish for 53-lo and shrimp for 40% for the allergic manifestations. Skin prick test consisted of 20 food allergens was done to all children above 2 years of age, 12 (40%) of the pa1ients showed positive results. This study demonstrated that food hypersensitivity may play a pathogenic role in some children with atopic dermatitis. Appropriate diagnosis and restriction of diet can improve their skin symptoms.

AbstractIgE sensitisation in tropical areas is under-reported. A 2 year retrospective cohort study of allergy data specific to aero and food allergens in Brunei demonstrated that specific IgE levels to house dust mite (Dermatophagoides pteronyssinus, D.farinae, Blomia tropicalis) were highest in this population and correlated with atopic dermatitis (p < 0.001). Shrimp and peanut were the most common food allergens. A dominance of house/storage mite sensitization is seen in Brunei which is consistent with other tropical countries.

Background Skin prick test (SPT) or Phadiatop, a multi-allergen IgE screening test, was used as a tool for detecting aeroallergen sensitization. Objective To compare SPT and Phadiatop as a tool for diagnosis allergic rhinitis (AR) using the nasal provocation test (NPT) as a comparative standard. Methods Children aged 5-18 years with rhinitis symptoms more than 6 times in the past year were enrolled. SPT to 13 common aeroallergens, serum for Phadiatop, and NPT to Dermatophagoides pteronyssinus (Der p) were performed. NPT to mixed cockroach (CR) were performed in children who had CR sensitization and negative NPT to Der p. Children who had a disagreement between the result of SPT and Phadiatop or having negative results were evaluated for specific IgE (sIgE) to common aeroallergens. Results One hundred-forty children were enrolled with the mean age of 9.8 ± 3 years, 56% were male. Of 92 children (65.7%) with positive SPT to any aeroallergens, 88 children (95.6%) were sensitized to house dust mite (HDM). NPT showed positive results in 97 children (69.3%). Of 48 children who showed negative SPT, 4 children (8.3%) had sIgE to aeroallergens but NPT was positive in 1 child. Eighty-eight children (62.9%) had positive tests for Phadiatop and 4 (4.5%) of them had negative results for NPT to Der p. Among 52 children who had negative results for Phadiatop, 4 children (7.6%) had sIgE to aeroallergens but NPT was positive in 2 children (3.8%). SPT and Phadiatop showed 94.2% agreement: with Kappa 0.876, p < 0.001. Using NPT as a comparative standard for diagnosis for AR, SPT showed a sensitivity of 89.6% and specificity of 88.3% and Phadiatop provided the sensitivity of 88.6% and specificity of 95.3%. Conclusions SPT to aeroallergen and Phadiatop have good and comparable sensitivity and specificity for the diagnosis of AR in children.

The growth of allergic diseases dictates the need to develop new forms of therapeutic allergens with high immunogenic and low allergenic activity. For many years, our laboratory has been developing drugs for the diagnosis and treatment of house dust mites (HDM) allergies. The purpose of this study is to summarize the results of the five-year development of therapeutic preparations of HDM allergens. During this period, we obtained the following forms of therapeutic allergens: a granular sublingual dosage form of a mixed allergen from Dermatophagoides pteronyssinus (Der.p) and Dermatophagoides farinaе (Der.f) and a succinylated monomeric HDM allergoid Der.p. The physicochemical and immunobiological properties of the obtained preparations were studied by methods: electrophoresis in PAGE in the presence of SDS-sodium, micropoint immunoblot, ELISA, inhibition of the binding reaction of allergen-specific IgE in the sera of patients. The research results showed that the obtained preparations have a reduced allergenic and increased immunogenic activity in comparison with native extracts. The created forms of mite allergens can be further used to treat patients sensitized to HDM of the genus Dermatophagoides.

Background/Aim. Atopic diseases such as atopic dermatitis, allergic rhinitis and asthma have had increased prevalence during the past decade and nowadays occur in every third child in developed countries. The aim of the study was to determine frequency and type of atopic diseases at the age of two, as well as the importance the total IgE antibodies concentrations have in diagnosis and prognosis of the disease. Methods. The study involved 175 children up to two years of age. Allergy-like symptoms were found after surveying their parents and pediatric medical records. Using the fluorescence immunossay (FIA) method, total IgE antibodies concentrations and specific IgE antibodies (Phadiatop infant) were determined on an Immunocap 100 Dyagnostic System. Results. One or more allergy-like symptoms accounted for 57.7% of findings in children under the age of two, whilst in 19.4% the existence of IgE-related allergic diseases was found. Atopic diseases usually have clinical manifestations of atopic dermatitis (11.4%), IgE-bound wheezing/asthma (10.8%) and food allergies (7.4%), and to much lesser extent those of allergic rhinitis (3.4%) and urticaria (1.7%). The significantly higher total IgE antibodies concentrations were found in children with allergy-like symptoms (p < 0.0005) (cut-off 15.15 kU/L, sensitivity 76.5% specificity 71.6%). Conclusion. Almost 20% of two-year-old children have any of clinically manifested allergic diseases, with atopic dermatitis and IgE wheeze/asthma being predominant. The higher total IgE antibodies concentration is a good marker for sensitization in children with allergy-like symptoms.

Abstract Background Worldwide, allergy affects more than one billion people, with particularly rising prevalence in industrialised areas. Specifically, young adults appear to be predominantly targeted for an allergy diagnosis. Allergic diseases in pregnancy are mainly pre-existing but could also occur de novo. The immunological changes while pregnant, with increased Th2 lymphocyte activity, can facilitate allergen sensitisation. Objective The aim of this study was to evaluate the pattern of specific IgE (sIgE) sensitisation to common inhalant and food allergens in pregnancy, and assess its relationship to self-reported allergic disease. Methods We assessed 200 pregnant women, aged 20–38 years (mean age = 29 years), participant of ELMA (Epigenetic Hallmark of Maternal Atopy and Diet) study, living in a metropolitan area, with no pregnancy associated metabolic complications, for total IgE and allergen specific IgE to 20 allergens. Results 48% of pregnant women were sensitised to at least one allergen, at a cut-off point of 0.35 kU/L and they were assigned as atopic. However 42% in atopic group were not reporting any allergic disease. The most common inhalant allergens were: pollen (24.5%) and animal dander (23.5%). The most common food allergens were: cow’s milk (5.5%) and apples (4.5%). 7.5% of women reported asthma, 21.5% allergic rhinitis, 11.5% atopic dermatitis and 18.5% food allergy. 8.5% of were taking medication for asthma or allergies. Atopic dermatitis had the highest tendency to become more severe during pregnancy. Total IgE values were significantly higher in atopic women. Conclusions Allergic sensitisation is a common phenomenon in pregnancy. Some sensitisations could be asymptomatic. Further studies should investigate if sensitisation in mothers confers risks for immune alterations in their children.

Background. One of the key aspects in the development of atopic dermatitis (AtD) is epidermal barrier dysfunction leading to the penetration of pathogens and allergens through the skin with further body sensibilisation to them. Such pathological interaction can later on lead to the development of various allergic diseases in the child which not only worsen the course of atopic dermatitis itself, but also significantly reduce the quality of life of these patients. Objective. Aim of the study is to estimate the efficacy of therapeutic approaches for treatment of atopic dermatitis in reducing the transcutaneous sensibilisation risk in infants. Methods. The study included children aged 1 to 4 months with established AtD from moderate to severe forms. The severity of AtD was estimated via the EASI index. The level of specific IgE (sIgE) to food and domestic allergens was measured by the ImmunoCAP method using special reagents’ sets. The sensibilisation class was established depending on the sIgE index. Statistical analysis of the studied indexes shift and their comparison between the study groups was performed via multivariate analysis of variance (ANOVA). Results. The study included 81 patients. All patients were divided into two groups after basic AtD therapy with topical glucocorticosteroids (tGCS). Patients from study group № 1 received maintenance therapy with topical calcineurin inhibitor (TCI) (pimecrolimus 1%; PIM) for a long time, while patients from group № 2 continued to apply tGCS as proactive therapy. We have revealed that the level of sensibilisation to chicken protein and to the mixture of domestic allergens “domestic dust” was lower to the 12th month of life in group № 1 compared to group № 2 as a result of the data analysis. Children in group № 1 had faster and more significant decrease in the severity of AtD in comparison to group № 2 according to EASI index. Conclusion. Maintenance therapy including PIM is more efficient in reducing AtD severity and in prevention of transcutaneous sensibilisation in infants.

Background. To identify differences in sensitization to widespread allergens in patients with common allergic diseases: allergic rhinitis, bronchial asthma, angioedema, allergic urticaria and atopic dermatitis. Materials and methods. Determination of sIgE in serum using commercial and domestic reagent kits («AllergoI-FA-specific IgE» and biotinylated allergens of «Alcor Bio», St. Petersburg) was conducted in patients with allergic rhinitis, bronchial asthma, angioedema, allergic urticaria and atopic dermatitis. Results. There was a significant similarity between the spectrum of sensitization in patients with allergic rhinitis and asthma In atopic dermatitis patients the increasing of frequency of food and especially mold sensitization was estimated. The high degree of correlation between the moulds, plants and food allergens in atopic dermatitis was observed. Conclusion. The results obtained in the study can be interesting for physicians in allergological examination and for the researchers to identify new cross-reactive allergens.
