Mucinous rectal carcinoma is morphologic subtype of Colorectal carcinoma. Both colloid ( mucin lakes ) and intracellular mucin ( signet ring cells ) are relatively rare. Various studies have been done to know whether palliative or radical treatment beneficial in treatment of advanced stage colorectal carcinoma. In our institute, we studied the prognostic factors in a case of 26 yr female in which abdominoperineal resection with endcolostomy with B/L salpingoohrectomy was done for stage 4 rectal carcinoma with synchronous secondaries in B/L ovaries.

We present an uncommon case of mucinous adenocarcinoma of the bladder (MAB) with signet-ring cells extensively infiltrating prostate gland and pelvic/retroperitoneal lymph node stations and not responsive to usual systemic chemotherapy regimens. This case highlights the important features of MAB including the pattern of tumor spread, the tendency for initial misdiagnosis, and the importance of immunohistochemical study in order to define its primary origin from the bladder and choose the most appropriate treatment since the beginning.

CONTEXT: Primary adenocarcinomas of the bladder are uncommon and usually occur by contiguity with or hematogenic dissemination of other adenocarcinomas such as colorectal, prostate and gynecological tract carcinomas. Mucinous and signet-ring cell histological patterns are even rarer and it is often difficult to morphologically distinguish them from metastatic colorectal adenocarcinoma. CASE REPORT: We present and discuss a rare case of primary mucinous adenocarcinoma of the bladder with signet-ring cells in a 57-year-old male patient. Other primary sites for the tumor had been excluded and, in the absence of digestive tract tumor and for confirmation that it was a primary bladder tumor, an immunohistochemistry study was performed.
