Introduction. In the structure of the global burden of diseases, back pain ranks first in the world in the list of causes of disability. Neck pain is one of the most common diseases of the musculoskeletal system. An integral part of the strategy for the treatment of acute neck pain is the use of original nonsteroidal anti-inflammatory drugs (NSAIDs) with high proven effectiveness.Objective. To conduct a comparative analysis of the therapeutic efficacy of the original drug meloxicam (Movalis) and its generic (Amelotex) in the treatment of patients with acute nonspecific musculoskeletal pain of cervical localization.Materials and methods. The article provides an assessment of the therapeutic effectiveness of the original drug meloxicam (Movalis) and its generic (Amelotex) in the treatment of patients with acute neck pain. Comparative analysis of the results of treatment of 108 patients hospitalized in the Branch of the MC JSC “Admiralteyskie Verfi” aged 18 to 60 years (58 men and 50 women) with a diagnosis of acute dorsalgia of cervical localization (M54.2 according to ICD-10). The average age of patients is 42.5 ± 11.1 years.Results. As a result of the clinical and neurological study and statistical processing of the data obtained, it was found that the use of Movalis in the treatment of patients with acute neck pain was more effective compared to the use of generic (Amelotex). In  the  Movalis group, the  level of  pain  intensity and the  index of  disability after treatment was significantly lower than in the Amelotex group at an equivalent dose of 15 mg per day (p < 0.01). The analgesic effect in the group of patients receiving Movalis was longer. There was a significant decrease in sleep disorders associated with pain syndrome in the group (Movalis) compared with patients of the second group (Amelotex). Conclusions. The absence of therapeutic bioequivalence between Movalis and Amelotex revealed in our study determines the relevance of the choice of drugs for the complex therapy of patients of this profile. A multimodal approach to the strategy of treating neck pain, individually patient-oriented, including the use of effective and safe medicines, physical exercises, manual therapy, is optimal.>< 0.01). Introduction: There have been many works devoted to the comparative analysis of high-frequency and low-frequency TENS in the treatment of neuropathic pain syndrome. Meanwhile, the comparative analysis of the methods of labile and stable TENS has not been sufficiently studied to date. Purpose: To make a comparative analysis of labile and stable high-frequency TENS in the treatment of neuropathic pain syndrome in patients with distal polyneuropathy of the lower extremities (DPLE). Materials and methods: 64 patients (F: 34, M: 30) with severe neuropathic pain syndrome on the background of diabetic DPLE were studied. Depending on the method of high-frequency TENS, all patients were divided into 2groups: stable high-frequency TENS (n=31) and labile high-frequency TENS (n=33). TENS was carried out every other day for a month. The severity of pain was determined by the VAS before each procedure. Results: The reduction of pain syndrome with the use of labile stimulation was 76.9%, compared to stable stimulation — 41,6%. With daily registration of pain syndrome, a significant increase in pain syndrome was revealed in patients who underwent stable high-frequency TENS after the 7th procedure to 4,5±0,3points in comparison with the indicators of pain syndrome obtained after the 3rd procedure (3,2±0,14). Conclusions: The analgesic effect of the labile high-frequency TENS significantly exceeds the analgesic effect of the stable high-frequency TENS by 85.1%. Partial tolerance to high-frequency TENS is observed when applying stable stimulation, which develops after the 7th procedure and is not observed after labile TENS.

Introduction: Comorbidities and co-occurring conditions contribute to the overall burden of migraine. Treatment co-occurring condition is an important part in providing optimal care. Aim: To investigate the effectiveness of complex non-pharmacological treatment and impact it on daily activities and concomitant anxiety and depression in patients with episodic migraine and co-occurring neck pain. Materials and Methods: We evaluated 56 patients with episodic migraine (with aura 18 and 38 without aura) and co-existing neck pain, 21 men, 35 women, age from 18 to 55 years. Examination including radiography of the cervical spine, visual analogue scale for the pain, “HIT-6”, Beck Depression Inventory and Spilberger-Hanin’s anxiety scale. The Kolmogorov-Smirnov test was used to compare the data. Results: Patients were divided into two treatment groups: the first group of patients 28 patients received only the specified complex of manual therapy, second group - 28 patients underwent a set of manual therapy and acupuncture. After treatment in first group observed decrease of frequency and severity of migraine attacks and a reduce number of days with a headache, but level of anxiety and depressive manifestations less decreased. In second group was more significant reduce the intensity, duration and frequency of migraine, more significant dynamics of anxiety disorders and general well-being. Conclusions: Non-pharmacological treatment neck-pain syndrome in patients with episodic migraine not only to be effective in symptomatic control of pericranial muscles tension but influence on decrease of frequency and severity migraine attacks. Adding acupuncture to manual techniques more significant reduce pain syndrome and psychological disorders that possible lead to reduce drug intake.

Introduction. Degenerative-dystrophic changes in the spine are the most common chronic human diseases. Pronounced clinical manifestations of vertebrogenic diseases are observed during active labor activity and represent one of the most frequent causes of temporary disability. It is also known that any pain syndromes are accompanied by the development of psychovegetative disorders, which reduces the effectiveness of treatment. In recent years, there has been a reasonable increase in the interest of the medical community to the non-drug treatment methods. It is caused by the polypharmacotherapy side effects, an increased numbers of allergic reactions, problems with polypragmasia, and a low level of compliance. At the same time, the question of the possibility of various non-drug treatment methods combining is quite acute. The goal of research is to substantiate the clinical effectiveness of the combined application of osteopathic correction and reflexotherapy in the complex treatment of patients with dorsopathy at the cervical-thoracic level.Materials and methods. A prospective controlled randomized study was conducted on the basis of the medical clinics of LLC «Family Health» (Kazan) and ANO «Center for SEB assistance» (Kazan) from April 2018 to May 2019. The study involved 52 people with dorsopathy at the cervical-thoracic level, aged from 25 to 45 years. In accordance with the exclusion criteria, 7 people dropped out of the study. The remaining patients (45 people), depending on the used treatment method, were divided by a simple randomization method using a random number generator into three groups of 15 people. The first group (main group) received osteopathic correction and reflexotherapyon the background of standard drug therapy; the second and the third groups (control groups) received only osteopathic correction orreflexotherapy, respectively, on the background of standard drug therapy. The osteopathic examination was performed for all patients regardless of the group, before and after the course of therapy, with the formation of an osteopathic conclusion. Also there was the estimation of the degree of pain syndrome severity, the asthenia, and the degree of accumulated emotional and energy charge that does not get out in a person.Results. It was found in the course of the study that the inclusion of osteopathic correction and reflexotherapy in the complex therapy of patients with dorsopathy at the cervical-thoracic level is clinically more effective than the isolated use of these methods. Such complex therapy allows to achieve a more significant reduction in the severity of the pain syndrome by VAS (p<0,05), relief of internal emotional tension (p<0,05), and increase the effectiveness of correction of dominant somatic dysfunction. Based on the obtained data, it could be assumed that reflexotherapy potentiates the effects of osteopathic correction.Conclusion. Based on the study, it can be concluded that the combination of osteopathic correction and reflexotherapy in the treatment of patients with dorsopathy at the cervical-thoracic level increases the clinical effectiveness of the standard drug therapy. At the same time, it is worth noting the potentiating effect of the combined use of two non-medicinal methods. The question of combining of various non-drug treatment methods is quite acute today, so this study will be continued.
