A complex therapy of patients with degenerative-dystrophic diseases of the spine with herniated intervertebral discs with neurological manifestations was carried out with the addition of phonophoresis Kariflex gel followed by segmental massage with Kariflex cream. The comparative evaluation of the method was carried out in 89 patients. Comparative analysis revealed the reliable efficiency of the method used. Against the background of the proposed therapy, there was a significant decrease in pain and muscle-tonic syndromes, an increase in the range of motion in the spine

Aim: to evaluate the efficacy of conservative treatment in patients with degenerative meniscal tear (DMT) in the setting of knee osteoarthritis (OA) as an alternative to combined treatment. Patients and Methods: a prospective comparative study included 236 patients aged 45–65 years with DMT in the setting of stage I–III OA according to the Kellgren-Lawrence classification (70 of them — men). Group 1 consisted of 122 patients (86 of them — women) who underwent combined treatment: arthroscopy for DMT of the knee joint and complex therapy in the postoperative period: NSAIDs (celecoxib), a combined drug from the group of symptom-modifying slow-acting drugs SYSADOA (chondroitin sulfate + glucosamine sulfate), exercise therapy, physiotherapy. Group 2 (n=114) received only complex therapy. Control tests (pain assessment on the visual analog scale (VAS), WOMAC scale, life quality assessment on the EQ-5D scale, treatment satisfaction) were performed 10 and 20 weeks after the therapy initiation. Results: there was a statistically significant (p<0.05) improvement in VAS and WOMAC indicators, which continued to decline by week 20 after 10 weeks of both combined and conservative treatment. Also, it did not depend on gender, age, concomitant pathology, but on the body mass index and OA stage according to the Kellgren-Lawrence classification, which was confirmed by the results of the correlation analysis. Patients’ treatment satisfaction and improvement in their life quality were noted in 50% of patients in both groups. Conclusion: arthroscopic meniscectomy should not be indicated as the first treatment stage in patients with DMT during OA. Complex therapy, including the use of drugs from the NSAID and SYSADOA groups at the recommended doses, has demonstrated high treatment efficacy and safety of patients with DMT during OA.      Low back pain (lumbar pain) due to injury of or damage to intervertebral discs is common in all societies.  The loss of work time as a result of this problem is massive.  Recent research suggests that melatonin may prevent or counteract intervertebral disc damage. This may be especially relevant in aging populations given that endogenous melatonin, in most individuals, dwindles with increasing age. The publications related to melatonin and its protection of the intervertebral disc are reviewed herein, including definition of some molecular mechanisms that account for melatonin’s protective actions.

Study Design: Retrospective observational study. Objective: To investigate the relationship between the extent of ligament ossification and the range of motion (ROM) of the lumbar spine and develop a new scoring system. Methods: Forty-three patients (30 men and 13 women) with lumbar spinal canal stenosis who underwent decompression from January to December 2018. Ligament ossification at L1/2 to L5/S was assessed on plain X-ray (Xp) and computed tomography (CT) using a modified Mata scoring system (0 point: no ossification, 1 point: ossification of less than half of the intervertebral disc height, 2 points: ossification of half or more of the intervertebral disc height, 3 points: complete bridging), and the intra-rater and inter-rater reliability of the scoring was assessed. The relationship of the scores with postoperative lumbar ROM was investigated. Result: Intra-rater reliability was high (Cronbach’s α was 0.74 for L5/S on Xp but 0.8 or above for other sections), as was inter-rater reliability (Cronbach’s α was 0.8 or above for all the segments). ROM significantly decreased as the score increased (scores 1 to 2, and 2 to 3). A significant moderate negative correlation was found between the sum of the scores at L1/2-L5/S and the ROM at L1-S (ρ = − 0.4493, P = 0.025). Conclusion: Our scoring system reflects lumbar mobility and is reproducible. It is effective for assessing DISH in fractures and spinal conditions, and monitoring effects on treatment outcomes and changes over time.

✓ Thirty-three intervertebral disc patients who otherwise would have been subjected to surgery were treated by injection with chymopapain. The pharmacology, toxicology, physiology, immunology, and chemical action of this enzyme, which specifically reacts with chondromucoproteins, are discussed. Results indicate that this method, although still experimental and unapproved by the U. S. Food and Drug Administration, may become a useful addition to the surgeon's armamentarium in the treatment of intervertebral disc disease.

Purpose. Analysis of the accommodative ability in children with hyperopic anisometropia, amblyopia of operated amblyopic and paired leading eyes after femtolaser-assisted laser intrastromal keratomileusis (FS-LASIK). Material and methods. In the 1st group were 20 children with hyperopic anisometropia and amblyopia after FS-LASIK. In the 2nd were children receiving conservative treatment. Results. After 6 months, when analyzing the accommodation, the coefficient of the accommodative response (CAR) of amblyopic eye in the 1st group was 0.05±0.07 conv. units, in the 2nd group was -0.1±0.19 conv. units (рm-u=0.03). The microfluctuation coefficient (CMF) of the amblyopic eye was increased, but the CMF of the paired leading eye in children after FS-LASIK approached the norm and amounted to 58.4±5.4 µF / min. In the conservative treatment group CMF remained high and was 60.8±4.44 µF / min (рm-u= 0.08). The objective accommodative response of the amblyopic eye in the 1st group increased by -1.13±0.03 diopters, in the 2nd group – by -0.82±0.48 diopters (рm-u=0.05). Conclusion. FS-LASIK helps to improve the accommodative ability in children with hyperopia, anisometropia and amblyopia. Key words: accommodation, hyperopia, anisometropia, amblyopia, FS-LASIK.

В статье представлены результаты сравнительного анализа 50 больных тромбоэмболии легочной артерии (ТЭЛА), которые по способу лечения были разделены на 3 группы: I группа - с антикоагулянтной терапией (гепарин). II группа - с селективной катетерной тромболитической (альтеплаза) и антикоагулянтной терапией. III группа - с катетерной аспирационной тромбоэкстракцией; Установлено, что на фоне комплексной терапии, включающую тромболитическую и антикоагулянтную терапии наблюдалась лучшая выживаемость пациентов с острой ТЭЛА. The article presents results of a comparative analysis of 50 patients with pulmonary embolism (PE), which were divided into 3 groups according to the method of treatment: Group I - with anticoagulant therapy (heparin); Group II - with catheter thrombolytic (alteplase) and anticoagulant therapy. Group III - with catheter embolectomy (aspiration thromboextraction); It was found against the background of complex therapy, including thrombolytic and anticoagulant therapy, there was a better survival rate for patients with acute PE.

Introduction. In the vast literature concerning the development of the intervertebral discs controversies exist as to the period of differentiation and structure of the nucleus pulposus and annulus fibrosus. These controversies result from different determination of age of the investigated embryos. Aim. Using embryos from departmental collection age of which was established according to international Carnegie staging and expressed in postfertilizational days, the differentiation of the intervertebral discs was traced. Material and methods. Study was performed on 34 embryos at developmental stages 13–23 (32–56 days). Embryos were serially sectioned in sagittal, frontal and horizontal planes. Sections were stained with various histological methods and impregnated with silver.Results. Division of sclerotomes into loose cranial and dense caudal zones (sclerotomites) was observed in embryos aged 32 days (stage 13). The intervertebral disc developed from the dense zone of sclerotome and was well recognized in embryos aged 33 days (stage 14). At the end of fifth week (embryos at stage 15, 36 days) the annulus fibrosus and the nucleus pulposus were seen. The annulus fibrosus differentiated into lateral and medial zones. Within the lateral zone cells were arranged into circular rows. These rows were considered as the first stage of laminar structure. In further developmental stages the laminae occupied both zones of the annulus fibrosus.Conclusions. The intervertebral discs develop from the dense zone of the sclerotome which is evident in embryos at stage 13 (32 days). Discs differentiate in embryos aged 33 days, when the nucleus pulposus and annulus fibrosus are recognized. In embryos aged 36 days in the annulus fibrosus circular rows forming laminar arrangement are seen.

The activation of the sympathetic nervous system plays an important pathophysiological role in the development of heart failure, in particular, in the development of left ventricular insufficiency. Although high blood pressure is considered as the main determinant of structural changes in the left ventricle, sex, salt intake, obesity, diabetes, as well as neurohumoral and genetic factors can affect the mass and left ventricular geometry. The usual concept of hypertonic re-modeling. In the comparative analysis of clinical and neurological manifestations in patients with chronic cerebral ischemia and chronic heart failure with a preserved and reduced release fraction, changes in the nervous system that showed a tendency to increase the disturbances and deviations from the norm with increasing heart failure, the fraction of release and the presence of hypertensive encephalopathy In the correlation analysis, a direct correlation between the quality of life indicator and the degree of heart failure (r = 0.56), the presence of myocardial infarction in the history (r = 0.42), arterial hypertension (r = 0.33) and the presence of valvular pathology the heart (r = 0.31) and the inverse correlation dependence on the indicator of the left ventricular ejection fraction (r = -0.69). A comparative analysis of correlation relationships indicates a reliable clinical and social significance of the left ventricular ejection fraction in patients with chronic cerebral ischemia and chronic heart failure.
