This integrative review investigates how nurses plan the hospital discharge of patients with Congestive Heart Failure (CHF) since an inadequate discharge plan and patients’ subsequent non-adherence to instruction provided upon discharge are indicated as potential factors for re-hospitalization. A total of 24 papers were found in a search carried out in the LILACS and MEDLINE databases between 2004 and 2008, which given the inclusion criteria, were reduced to 14 papers. The papers were analyzed and categorized into ‘Health Education’, and ‘Nursing Care’. The synthesis of results indicates that the discharge plan devised by nurses is based on two categories. The actions of nurses to promote health education can enable patients with CHF to improve self-care.

Purpose: This scholarly project was to determine if an advanced practice registered nurse (APRN) – led transition of care (TOC) would positively influence the outcome of quality of life in congestive heart failure (CHF) patients. The purpose was to meet Medicare-mandated target goals in reducing readmission to 12%. The project investigates the gap between the time of discharge and transition of care. The improvement plan objectives are threefold: to increase retention of self-care activities utilizing the teach-back method while monitoring emergency room (ER) utilization and measuring benchmark assessment. This project aligns with one of the Institute of Medicine [IOM] six dimensions of patient-centered care. Findings: In the period of February 22 – March 22, 2018, ten subjects were enrolled in the project (N=10). Even though chi square analysis revealed no significant difference (x2 (1, N= 10) = 2.3746, p = .1233) in readmissions post intervention (See Appendix -T); APRN – led TOC demonstrated positive changes in teach back and reteach via leadership competences and clinical experience revealing overall improvement in trends. Discussion: There were ten participants in the pilot study (N=10). Out of these ten CHF patients one was readmitted in the 30-day period of the study. All subjects were carefully followed and monitored. Field assessment benchmark revealed that at one week 100% of all patients were retaining 90% or more of the self-care activities and maintaining appropriate priority tasks. There was no ER utilization and patients in the “yellow” zone knew what to do with a three-pound weight gain.

Background: Heart failure is a cardiovascular disease that has an increased incidence and prevalence. The risk of death from congestive hearth failure ranges from 5-10% per year in mild heart failure which will increase to 30-40% in severe heart failure. In addition, heart failure is a disease that most often requires re-treatment in a hospital, and outpatient treatment must be performed optimally.one of the factors that influence the incidence of hospitalization for heart failure patients is the independence of patients in self-care. Objectives: The purpose of this study was to find out the description of self care in congestive heart failure patients at Kraton Hospital Pekalongan District. Methods: The research model used in this study was descriptive qualitative with sampling using purposive sampling. The inclusion criteria in this study were patients with heart failure with NYHA I-III with stable conditions, patients hospitalized, who were hospitalized with rehospitalization, heart failure patients who were willing to become participants, not having cognitive impairment., patients with NYHA I-III with stable conditions. For exclusion criteria in this study were heart failure patients with comorbidities such as stroke, kidney failure and heart failure patients who were treated in the ICU. The sample in this study was five respondents. This research was conducted by interviewing heart failure patients about their experience in self care. Results: The results of this study obtained 4 themes are knowledge about self care, the implementation of self care, barriers in carrying out self care and social support in doing self care. Conclusion: patients consider that self care is very important but they have not been able to do self care optimally because they feel unsure of themselves. Thereforeweneedaninterventionaimedatimproveself-carebehaviorinpatientswith congestive heart failure.

Congestive Heart Failure menjadi masalah kesehatan masyarakat yang utama pada beberapa negara berkembang seperti Indonesia. Angka kejadian Congestive Heart Faliure yang meningkat menimbulkan penurunan kualitas hidup penderita salah satunya kurangnya perawatan diri (self care). Penelitian bertujuan untuk mengetahui hubungan self care dengan kualitas hidup pasien Congestive Heart Failure di Rumah Sakit Mitra Medika Medan. Penelitian ini menggunakan desain survey analitik dengan pendekatan cross sectional dan menggunakan uji chi-square. Jumlah responden 62 pasien Congestive Heart Failure di Rumah Sakit Mitra Medika Medan yang diambil menggunakan teknik total sampling. Penelitian ini menggunakan analisa univariat dan analisa bivariat dengan menggunakan uji chi-square. Hasil analisa univariat menunjukkan bahwa mayoritas responden memiliki self care baik (30,6%) dan kualitas hidup baik (32,3%). Hasil analisa bivariat menggunakan uji chi-square terlihat nilai p value sebesar 0,035. Karena nilai p value (0,035) < α (0,05), maka dapat disimpulkan bahwa terdapat hubungan antara self care dengan kualitas hidup pasien Congestive Heart Failure di Rumah Sakit Mitra Medika Medan.

Depresi merupakan kondisi yang rentan dialami oleh pasien Congestive heart failure, dimana keadaan patologis gagal jantung seperti kerusakan struktur dan fungsi jantung secara tidak langsung mempengaruhi kualitas hidup pasien, serta tingginya angka rehospitalisasi dan mortalitas dapat menyebabkan timbulnya keterbatasan fungsional dan distress psikologis bagi pasien. Penelitian ini bertujuan untuk mengetahui Hubungan Self Care Activity Dengan Tingkat Depresi Pada Pasien CHF. Jenis penelitian ini adalah kuantitatif dengan desain deskriptif korelasional dengan pendekatan cross sectional. Penelitian ini dilakukan di Poliklinik Jantung RSU Famili Husada. Pengambilan sampel dengan menggunakan nonprobability sampling dengan teknik proposive sampling, diperoleh sampel 50 orang. Hasil penelitian menunjukkan dari 50 responden sebagian besar  memiliki tingkat depresi sedang yaitu 30 responden (60%). Sedangkan self care activity sebagian besar memiliki self care activity yang cukup yaitu 36 responden (72%).  Hasil uji rank spearman  test menunjukan nilai p-value 0,000  α = 0,05 yang artinya ada hubungan self care activity dengan tingkat depresi pada pasien CHF di poliklinik Jantung RSU Famili Husada.
