Bruxism merupakan suatu kebiasaan parafungsional berupa gerakan menggertakan dan menggerus gigi. Tidak jarang pasien yang mempunyai kebiasaan bruxism memerlukan perawatan ortodontik. Perawatan bruxism dapat dilakukan bersamaan dengan perawatan ortodontik cekat. Studi kasus ini bertujuan untuk menganalisa efek Trainer for Braces (T4B) pada pasien bruxism yang memerlukan perawatan ortodontik cekat. Pasien perempuan usia 21 tahun, mengeluhkan gigi berjejal dan tidak rapi. Pemeriksaan objektif menunjukkan bidental protrusi, crowding rahang atas dan rahang bawah, deep overbite, konstriksi berat pada regio gigi premolar dan molar rahang atas dan rahang bawah, 47 linguoversi dan disertai bruxism. Maloklusi Angle Kelas I dengan hubungan skeletal Kelas I, bidental protrusi, overjet 3,7 mm, overbite 4 mm, crowding, edge to edge bite, cross bite dan bruxism. Pasien dirawat menggunakan alat ortodontik cekat teknik Edgewise dengan alat tambahan Lingual Arch Bar untuk ekspansi rahang dan koreksi 47 yang linguoversi dan Trainer for Braces (T4B) untuk bruxism. Setelah 8 bulan perawatan, crowding rahang atas dan rahang bawah terkoreksi, ekspansi rahang dapat tercapai, 47 yang linguoversi terkoreksi, overjet dan overbite berkurang menjadi 3,5 mm, perawatan pada pasien masih berlangsung hingga saat ini. Kombinasi perawatan ortodontik cekat dengan penggunaan alat tambahan seperti Trainer for Braces (T4B) efektif untuk membantu koreksi maloklusi pada pasien bruxism. ABSTRACT: Edgewise Technique Combined with Trainer for Braces for Bruxism Patient. Bruxism is a parafunctional habit of grinding and clenching the teeth. It is common for patients with fixed orthodontic treatment to experience bruxism. When dealing with these patients, clinicians could initiate the bruxism treatment in conjunction with the orthodontic treatment. This case report will analyze the effects of Trainer for Braces (T4B) in a patient with malocclusion and bruxism habit. A 21 year old female patient complained of her crowding in upper and lower anterior teeth. The objective examination shows protrusion and crowding in upper and lower teeth, deep overbite, severe maxillary and mandibulary constriction, 47 linguoversion and bruxism habit. Angle Class I with skeletal Class I malocclusion, bidental protrusion, overjet 3.7 mm, overbite 4 mm, crowding, edge to edge bite, crossbite and bruxism habit. The orthodontic treatment used fixed preadjusted Edgewise appliance with Lingual Arch Bar for expansion and lingoversion molar correction. Trainer for Braces (T4B) was also prescribed to treat her bruxism. After 8 months of treatment, the crowding in upper and lower teeth was corrected, dental arch expansion was achieved, linguoversion molar was corrected, and overjet and overbite became 3.5 mm and the treatment was still on going. The combination of fixed preadjusted Edgewise appliance with Trainer for Braces (T4B) can be considered as an effective therapy for correcting malocclusion in bruxism patient.

Pencabutan dini pada gigi desidui seringkali menyebabkan perpendekan lengkung sehingga ruang yang dibutuhkan untuk erupsi gigi permanen kurang. Kaninus ektopik, impaksi premolar dan asimetri rahang adalah kasus yang sering terjadi akibat kurangnya ruang untuk erupsi gigi. Pasien perempuan usia 20 tahun, mengeluhkan gigi gingsul dan tidak rapi. Pemeriksaan objektif menunjukkan crowding rahang atas dan rahang bawah, kaninus maksila kanan atas ektopik, premolar mandibula kanan impaksi, disertai pergeseran midline. Maloklusi Angle kelas I dengan hubungan skeletal kelas I, overbite 1 mm, overjet  1,3 mm, crowding mandibula dan maksila, ektopik kaninus unilateral, impaksi premolar unilateral, asimetri rahang dan pergeseran midline. Pencabutan dilakukan pada 2 gigi premolar pertama rahang atas dan 2 gigi molar pertama rahang bawah yang mengalami nekrosis. Perawatan menggunakan alat cekat teknik Edgewise dengan multiloop. L loop digunakan untuk ekstrusi gigi kaninus ektopik dan menarik impaksi premolar. Asimetri rahang dikoreksi dengan ekspansi lengkung gigi menggunakan mainarchwire. Setelah 7 bulan perawatan, gigi kaninus yang ektopik, impaksi premolar dan crowding terkoreksi. Overjet dan overbite menjadi 2,5 mm. Perawatan masih berlangsung hingga saat ini. Penggunaan multiloop pada teknik Edgewise efektif untuk mengoreksi crowding gigi dan L loop efektif untuk mengekstrusi gigi kaninus ektopik dan impaksi premolar. ABSTRACT: Management of Impacted Premolar, Ectopic Canine and Jaw Asymmetry Case Using Edgewise Technique. The premature loss of primary teeth may reduce the arch length required for permanent teeth eruption. Ectopic canine, premolar impaction and jaw asymmetry are often encountered due to the lack of space for permanent teeth eruption. A twenty-year-old female patient complained an ectopic canine and crowding of upper and lower teeth. The objective examination found maxilla and mandibula crowding, ectopic right maxilla canine, mandibular premolar impaction, and midline shift of maxilla and mandibula. Angle class I malocclusion, overbite 1 mm, overjet 1.3 mm, maxilla and mandibula crowding, unilateral ectopic canine, unilateral premolar impaction, jaw asymmetry and midline shift occurred. Extraction of 2 maxillary first premolars and 2 necrose mandibular first molars was conducted followed by a treatment using fixed orthodontic appliance Edgewise technique with multiloop. The ectopic canine and premolar impaction were extruded using L loop. Jaw asymmetry was corrected by expanding the dental arch using the expanded main archwire. Ectopic canine, premolar impaction, maxilla and mandibula crowding were corrected after 7 months of treatment. The overjet and overbite reached 2.5 mm. The treatment is still ongoing. The multiloop edgewise archwire technique is found effective to overcome dental crowding, and L loop is effective to extrude the ectopic canine and premolar impaction.

Impaksi palatal kaninus sering mengakibatkan keluhan secara estetis. Faktor genetik merupakan faktor yang dominan serta beberapa faktor yang lain yaitu diskrepansi lengkung gigi, ukuran gigi, retensi gigi desidui, kerusakan dini, pencabutan dini , posisi yang abnormal benih gigi, agenese incisivus lateral dan kista. Tujuan laporan kasus ini adalah untuk memberikan informasi tentang perawatan kasus impaksi palatal kaninus rahang atas pada maloklusi kelas I skeletal menggunakan alat cekat teknik Begg. Pasien perempuan umur 15 tahun, datang ke klinik ortodonsia RSGM Prof. Soedomo FKG UGM. Pemeriksaan subyektif, pasien terganggu dengan keadaan gigi depan yang maju dan bercelah. Pemeriksaan obyektif menunjukkan adanya rudimenter, agenese gigi incisivus lateral kanan dan kiri rahang atas, impaksi palatal kaninus kiri atas. Transposisi kaninus kanan atas ke ruang incisivus lateral kanan atas, pergeseran midline. Maloklusi angle kelas I dengan relasi skeletal kelas I dengan bimaksiler retrusif disertai protrusif incisivus maksila dan retrusif incisivus mandibula. Perawatan diawali dengan pencabutan gigi rudimenter. Tahap I menggunakan multiloop archwire untuk leveling dan unraveling gigi anterior, koreksi pergeseran midline. Tahap berikutnya adalah pemasangan button pada kaninus impaksi untuk mengaitkan kawat ligatur pada archwire yang berfungsi untuk menarik kaninus impaksi palatal pada lengkung gigi. Perawatan ortodontik pada kasus dengan impaksi palatal kaninus rahang atas pada maloklusi kelas I skeletal menggunakan alat cekat teknik Begg dapat dilakukan dengan hasil perawatan yang baik. ABSTRACT: Repositioning of Palatally Impacted Canine in Orthodontic Treatment Using Begg Fixed Appliance. Palatally impacted canine often leads to esthetic complaints. Genetic factor is dominant followed by such other factors as dental arch discrepancy, tooth size, retention of deciduous teeth, early decay, premature extraction, abnormal position of tooth germ, lateral incisor agenesis, and cysts. This article provides information about the treatment of palatally impacted maxillary canine case in a skeletal class I malocclusion using Begg fixed appliance technique. A 15-year-old female patient came to the orthodontia clinic of RSGM Prof. Soedomo FKG UGM. The subjective examination found that the patient was disturbed by her protrusive, gapped front teeth. Then, the objective examination indicated the presence of rudimentary, lateral incisor agenesis of right and left upper jaw, and upper left palatally impacted canine. In addition, there was a transposition of upper right canine to lateral incisor area as well as a midline shift. Angle class I malocclusion with class I skeletal relationship and bimaxillary retrusion along with maxillary incisor protrusion and mandibular incisor retrusion also occurred. The treatment began with rudimentary tooth extractions. The first stage used a multiloop archwire for leveling and unraveling of anterior teeth as well as correction of midline shift. The button attached to the impacted canine could tie the ligature wire to the archwire that served to attract the palatally impacted canine in the dental arch. The orthodontic treatment in cases of palatally impacted maxillary canine with skeletal class I malocclusion using Begg fixed appliance technique can be applied with a good treatment result.

Case report of an adult Bangladeshi male aged 26 years who complained about ineffective chewing of food and unpleasant aesthetic look. The patient was diagnosed to have a 4 mms of anterior dental open bite on skeletal Class I with normal mandibular plane and palatal plane angles. Treatment was orthodontics alone with non-extraction. The main mechanics used to close the anterior open bite, were by the conventional aligning, leveling and arch contraction by standard edgewise technique. The result showed that it was possible to correct the open bite within a short period; however, the appliance was difficult to clean. The treatment was complete after 2 years with positive overbite of 3 mms and acceptable occlusion. The patient was happy with his new appearance and function. ABSTRACT This clinical case report describes the orthodontic treatment of an 8-year and 9-month old female patient with Angle Class I malocclusion, anterior crossbite and canine Class III relationship. Orthodontic treatment was carried out in two stages. The first one was orthopedic, while the second one included the use of a fixed appliance and the need for space gain for reshaping of maxillary lateral incisors. The two-stage treatment combined with multidisciplinary Restorative Cosmetic Dentistry allowed excellent esthetic and functional outcomes to be achieved. This case was presented to the Brazilian Board of Orthodontics and Dentofacial Orthopedics (BBO) as a requirement for the title of certified by the BBO.

This article reports the orthodontic diagnosis and treatment planning carried out with a 14-year and 5-month-old female patient with esthetic and functional complaints. She presented an Angle Class I malocclusion, anterior crossbite and severe crowding in both maxillary and mandibular arches, in addition to a lightly concave straight facial profile. Orthodontic treatment did not require extraction. Crossbite was corrected by protrusion of upper teeth, which contributed to alignment and leveling of teeth, in addition to improving the patient's facial profile. The case was presented to the Brazilian Board of Orthodontics and Dentofacial Orthopedics (BBO) as a requirement for the BBO certification.

Background.This case report describes the orthodontic treatment of a woman, aged 17 years, with a permanent dentition, brachyfacial typology, Angle Class I, with full impaction of two canines (13,33), and a severe ectopy of the maxillary left canine. Her main compliant was the position of the ectopic teeth.Methods.Straightwire fixed appliances, together with cantilever mechanics, were used to correct the impaired occlusion and to obtain an ideal torque control.Results and Conclusion.The treatment objectives were achieved in 26 months of treatment. The impactions were fully corrected with an optimal torque. The cantilever mechanics succeeded in obtaining tooth repositioning in a short lapse of time. After treatment, the dental alignment was stable.

Crowding is one of the most common findings in orthodontic patients. This case report describes the correction of anterior crowding with interproximal reduction (IPR). The case is that of a 24-year-old female patient who consulted for irregularly placed anterior teeth. Fixed appliances were placed followed by IPR. Treatment time was 18 months. Class I molar relation was maintained, with normal overjet and overbite. IPR is effective for treatment of patients with Class I malocclusion with anterior moderate crowding.

BACKGROUND: After levelling and alignment in skeletal Class III patients with upper anterior crowding, the upper incisors usually have excessive proclination. In these cases, the upper incisors’ axial proclination need to be reduced to improve esthetics. CASE REPORT: This case report presents an invisible orthodontic treatment of a 24-year-old adult female patient with skeletal Class III relationship, anterior crossbite, proclined upper incisors, and reduced incisor showing. Patients denied extraction and interproximal reduction. With multi-slotted lingual brackets and straight archwires, we applied lingual crown torque to upper anterior teeth to reduce axial proclination. The resulting uprighted position of upper incisors led to increased incisor showing. A good smile and stable occlusion were obtained after 15 months of active treatment. CONCLUSION: The use of lingual brackets to apply lingual crown torque helps to reduce axial proclination and increasing upper incisor showing without interproximal reduction nor extraction in skeletal Class III patients with upper anterior crowding.

Two cases of anterior crowding, both Skeletal Class I and Angle Class I maloclussion, one being low angle and the other high angle respectively, respectively, were treated and evaluated to ascertain whether or not there is a relationship among disproportionate mesial axial angulation of the maxillary lateral teeth and the Frankfurt Horizontal- Functional Occlusal plane, therefore generating maxillary anterior crowding. Both cases were Japanese boys, the first one aged 9 years 10 months with chief complaint being anterior crowding and the second case aged 7 years and 8 months complaining of inadequate space for satisfying canine eruption. During and after the second stage of orthodontic treatment on both cases, several radiographic analysis were performed to assess treatment progress and retention; from these radiographs, it was noticed among other findings that in the high-angle case, the axial angulations of the maxillary lateral incisors were markedly smaller than in the low-angle case, thus indicating mesial tipping in the upper dental arch. This decreased mesial axial angulation of the lateral teeth observed at high angles may potentially cause maxillary space deficiency.
