This study measured objectively the postural physical activity of 4 groups of older adults (≥65 yr). The participants (N= 70) comprised 3 patient groups—2 from rehabilitation wards (cityn= 20, 81.8 ± 6.7 yr; ruraln= 10, 79.4 ± 4.7 yr) and the third from a city day hospital (n= 20, 74.7 ± 7.9 yr)—and a healthy group to provide context (n= 20, 73.7 ± 5.5 yr). The participants wore an activity monitor (activPAL) for a week. A restricted maximum-likelihood-estimation analysis of hourly upright time (standing and walking) revealed significant differences between day, hour, and location and the interaction between location and hour (p< .001). Differences in the manner in which groups accumulated upright and sedentary time (sitting and lying) were found, with the ward-based groups sedentary for prolonged periods and upright for short episodes. This information may be used by clinicians to design appropriate rehabilitation interventions and monitor patient progress.

Background: Older adults with cognitive impairment may have difficulty understanding and complying with medical or medication instructions provided during hospitalization which may adversely impact patient outcomes. Objective: To evaluate the prevalence of cognitive impairment among patients aged 65 years and older within 24 hours of hospital admission using Mini-Cog™ assessments performed by advanced pharmacy practice experience (APPE) students. Methods: Students on APPE rotations were trained to perform Mini-Cog™ assessments during routine medication education sessions from February 2017 to April 2017. The primary end point was the prevalence of cognitive impairment indicated by a Mini-Cog™ score of ≤3. Secondary end points were the average number of observed Mini-Cog™ practice assessments required for APPE students to meet competency requirements, caregiver identification, and 30-day hospital readmissions. Results: Twelve APPE students completed the training program after an average of 4.4 (standard deviation [SD] = 1.0) graded Mini-Cog™ assessments. Of the 1159 admissions screened, 273 were included in the analysis. The prevalence of cognitive impairment was 55% (n = 149, 95% confidence interval [CI]: 48%-61%). A caregiver was identified for 41% (n = 113, 95% CI: 35%-47%) of patients, and 79 patients had a caregiver present at bedside during the visit. Hospital readmission within 30 days of discharge was 15% (n = 41, 95% CI: 11%-20%). Conclusion: Cognitive impairment could substantially impair a patient’s ability to comprehend education provided during hospitalization. Pharmacy students can feasibly perform Mini-Cog™ assessments to evaluate cognitive function, thereby allowing them to tailor education content and involve caregivers when necessary.

Objective: To evaluate the impact of a geriatric day hospital program on older adults’ functional independence and on caregiver stress. Methods: We used a single group pre- and posttest design. The data were collected through chart reviews and follow-up phone calls. Outcomes included fear of falling, balance, functional exercise capacity (walking distance), and caregiver stress. Descriptive statistics were used for sociodemographic data, dependent t test for paired samples of normally distributed data, and the Wilcoxon signed-rank test for determining differences between nonnormally distributed data sets. Results: We found a statistically significant difference in pre (33.54) and post (27.47) mean rank scores for fear of falling ( Z = −3.895, P < .001), pre (49.5) and post (59.42) scores for balance ( Z = −8.725, P < .001), and pre (250.07 m) and post (291.20 m) for functional exercise capacity ( P < .001). No statistically significant difference was found with respect to caregiver stress pre (22.05) and post (19.90) scores ( Z = −0.422, P = .673). Discussion: Future research may consider approaching evaluative studies of a similar type using not only quantitative but also qualitative methods to obtain a more comprehensive understanding of older adults’ functional ability and caregiver stress before and after participating in a geriatric day hospital program.

Day Hospital treatment offers the patient a wide range of services. The number of patients who take advantage of this structure is growing every day and this fact creates some problems in the management of such a service. For this reason we have created a computerised programme which has made the management of the Day Hospital service easier and allows us to carry out research and analyses on the activities done.

BACKGROUND Mental health is a prevalent condition amongst Canadians. Mental health conditions are the leading cause of disability, as each year 1 in 5 Canadians experiences a mental health issue. Five percent of people aged 65 years and older perceive their mental health as fair or poor, and 6.3% of them have mood disorders. Regarding older adults with cognitive impairments such as dementias, up to 40 to 50 per cent of them experience depression at some point. We believe that older adults can benefit significantly from information and telecommunication technologies (ICTs) as a strategy for improving mental health conditions such as depression and anxiety, while at the same time improving their quality of life. 3Scape Systems Inc. is an Albertan-based private company that has produced a series of specialized 3D videos that have been designed to simulate real-life events and engage individuals living with mental health disorders and cognitive impairments such as dementia. OBJECTIVE The purpose of this study is to explore the trial design and effects of 3Scape videos on older adults’ symptoms of depression, anxiety, and quality of life, as well as the efficacy of this technology in reducing the caregiver burden on the START Psychiatry Day Hospital program at Glenrose Rehabilitation Hospital (GRH), and to provide data in order to estimate the parameters required to design a definitive Randomized Controlled Trial (RCT). METHODS The trial will use a randomized controlled design comprising 15 intervention participants and 15 control group participants. The participants will be adults aged 65 years and older who are cognitively intact or who have a minimal cognitive impairment, i.e. Montreal Cognitive Assessment (MOCA) ≥18), and clients from the START Psychiatry Day Hospital program at GRH. This study’s primary outcome variables are related to the clients’ depressive and anxiety symptoms, and their quality of life. The control group will receive the standard of care (i.e. the START Psychiatry Day Hospital program at GRH). The intervention group will receive the same standard of care as the control group at each facility and will use the 3Scape Systems videos for the therapeutic activities. RESULTS Our study is currently on hold due to the Covid-19 global pandemic. The recruitment process is expected to resume by November 2020 and the primary impact analysis is expected to be conducted by February 2021. CONCLUSIONS This study will provide valuable information such as the measurement of comparative intervention effects, perception of older adults and hand therapists about the 3Scape Systems, the associated costs of treatment, and product costings. This will contribute to the evidence planning process, which will be crucial for the future adoption of the 3Scape Systems.
