Bladder exstrophy is a rare and complex malformation. Often associated with diverse deformations, an accurate diagnosis is a must for adequate management. In the African setting, especially in a remote area, delivery rarely occurs within a healthcare structure thus reducing the chance of an early diagnosis.

This study describes a rare case of a 19 year old girl who presented with features of subcutaneous emphysema and pneumomediastinum to the emergency department in whom further investigation revealed pulmonary and neurotuberculosis. Tuberculosis presenting as pneumomediastinum and subcutaneous emphysema is a rare, but an important entity and a better knowledge of this condition can help in the early diagnosis and adequate management of tuberculosis.

The prognosis of gastric cancer depends principally upon an early diagnosis. An early and accurate diagnosis of gastric cancer needs some basic knowledges about the endoscopic characteristics of white light endoscopy, chromoendoscopy, magnified endoscopy, FICE and NBI…A strategy of screening is also a key factor for early diagnosis. The treatment of early gastric cancer by endoscopy techniques have showed more and more advantages. Beside of EMR, the technique of ESD is now applied more widely and lead to a very good prognosis and nearly a curative treatment for the patients with early gastric cancer. Background: Supernumerary kidney is an accessory organ with its own encapsulated parenchyma, blood vessels and ureters, either separated from the normal kidney or connected to it via fibrous tissue and ectopic kidney is a migration abnormality of the kidney. Here, we have evaluated a rare case of the supernumerary and ectopic kidney with DMSA, MAG3 and also CT fusion of the images. Methods: The absolute divided renal function was calculated for each kidney by DMSA. The MAG3 scintigraphy showed no obstruction in the ureteropelvic junction. Furthermore, the renogram curve and Tmax and time to ½ values were assessed. Two months after the conventional scintigraphies, the patient was referred to a CT scan and the fusion of DMSA SPECT and CT data was generated on a workstation. Results: The ectopic supernumerary kidney was functioning very well except a small hypoactive area, visible on DMSA, which was possibly a minimal pelvicalyceal dilatation. However, consequent CT scan did not show any pathology. Conclusion: It is important to evaluate particularly complicated or rare cases with multimodality systems with 3D or fusion techniques for the accurate diagnosis.

Solitary fibrous tumours (SFTs) are rare mesenchymal tumours that are mostly seen in the pleura. Lately, they have also been described in other locations. Recent discovery of the NAB2-STAT6 fusion gene which is specific for SFTs has led to an accurate diagnosis of SFTs. The occurrence of SFTs in the mesentery is very rarely reported in the literature. We report a case of a 63-year-old female who presented with abdominal pain, rectal bleeding and Fusobacterium bacteraemia, who was ultimately found to have a mesenteric SFT.

Cor triatriatum dextrum is an exceptionally rare congenital heart disease, in which the right atrium is partitioned into two chambers by a membrane to form a triatrial heart. It is caused by persistence of the right valve of sinus venosus. The aim of presenting this case is to develop awareness regarding cor triatriatum dextrum , though a rare case, can be present and may contribute to right heart failure and 2D-echocardiography is an important tool in making early and accurate diagnosis. We are reporting a case of an elderly Bangladeshi male presented with the features of mitral stenosis with pulmonary hypertension with CCF with respiratory tract infection, where cor triatriatum dextrum with an atrial septal defect was an incidental finding on routine echocardiographic assessment.Bangladesh Heart Journal 2016; 31(1) : 37-40

Pertusis affects all the age groups but is most severe in neonates and early infancy and may even cause mortality. Clinical presentation of neonatal pertusis is varied and thus knowing the spectrum of clinical presentation is vital for early diagnosis. Unlike older children, most of the times neonatal pertusis has an atypical presentation and classical presentation is very rare. Here we present such a rare case of neonatal pertusis who presented with classical symptoms of pertusis.

The children presenting with acute scrotum have many diseases in differential diagnosis and testicular torsion in undescended testis and normal testis is an important consideration. Torsion of the testis, or more correctly, torsion of the spermatic cord, is a surgical emergency because it causes strangulation of gonadal blood supply with subsequent testicular necrosis and atrophy. In most of cases history and physical examination are sufficient to make an accurate diagnosis. While other causes may require simple treatment but torsion require immediate exploration to save the testis. Early diagnosis and prompt treatment is essential to help such children.
