Background: Smoking is the leading cause of preventable deaths in the United States. The rates of smoking remain elevated in rural, low income populations in comparison with the rest of the United States. Thus, prompting the process improvement project of implementing the Ask–Advise–Connect (AAC) method to the national quitline in a nurse practitioner–managed clinic for an automotive manufacturing plant in rural Tennessee. Methods: Ask–Advise–Connect method was added to the current smoking cessation program. The employees who utilized the clinic were assessed for smoking status at each visit and subsequently counseled on cessation. Individuals interested in cessation were connected to the national quitline with the AAC method. Pharmaceutical options and nicotine replacement therapy was also offered at no cost to the employee. Findings: In the 4-month period, the clinic provided 102 tobacco cessation counseling visits to workers who smoke. Twenty-four employees enrolled in the cessation program. The participants reported a cessation rate of 12.5% and 21% had a significant decrease in the number of cigarettes smoked. Of the participants, 12.5% ( n = 3) engaged in behavioral counseling with the quitline. Conclusion/application to practice: The addition of the AAC method as part of the smoking cessation program had limited success. As smoking cessation is difficult to achieve, any success greater than 7% can be considered an achievement. The 12.5% cessation rate of the participants was above the national average. Thus, demonstrating the benefit of having a workplace cessation program and incorporating the AAC method to the current smoking cessation program.

To design strategies for provider education and implementation of clinical guidelines, this study investigated how physicians (1) approach tobacco cessation, including barriers to screening and treatment, (2) prioritize tobacco cessation, and (3) perceive the role of public health. Semi-structured focus groups were conducted with 30 New York City physicians across specialties. Physicians reported that they: (1) understand risks of smoking, as well as basic counseling and medications for smoking cessation; (2) do not always follow clinical guidelines for treatment of smoking cessation; (3) prioritize treatment of patients based upon a number of criteria; and (4) see the role of public health and the city health department as separate from the clinical environment, despite population-level interventions to reduce smoking. Physicians understand the importance of treating tobacco dependence, but identified barriers to treatment, some of which are health system-related. Further, patients who do not yet present with smoking-related illness may receive less intense interventions.

This study represented the largest statewide demonstration (n = 346) of the teen smoking cessation program Not On Tobacco (N-O-T) to date and one of the few systematically controlled teen smoking cessation trials reported in the literature. Results showed that N-O-T female teens were 4 times more likely to quit smoking almost 6 months after the program ended than female teens who received a brief intervention (BI). The quit rate for the N-O-T female groups was significantly higher than that for female brief intervention comparison groups. The study demonstrated that 2 times more N-O-T than BI teens quit smoking overall. Differences in the biochemically validated quit rate between the N-O-T groups and the brief intervention groups overall and for male participants were not statistically different, however. Furthermore, findings showed that N-O-T was more effective than the brief intervention in assisting youth with cigarette reduction. There was a significant difference in the reduction rate between the N-O-T and the BI groups on weekdays and weekends 6 months after the program ended. Overall, approximately 84% of N-O-T teens either quit or reduced smoking, compared with approximately 55% of BI teens. This study is 1 phase of an ongoing multiphase evaluation of N-O-T This study resulted in several important findings that will help guide future teen cessation studies and tobacco cessation efforts of school health professionals.

Abstract School has an important role in preventing students’ smoking behavior. This research aims to analyze non- smoking area policies and prevention programs for smoking behavior, as well as of the role of students, parents, teacher and staff in preventing students’ smoking behavior. This qualitative research was conducted in six Muhammadiyah Schools in Depok City. Data collection was carried out in January 2018 to April 2019. The informants of the study were 15 selected through a purposive sampling technique. Data collection is done through in-depth interviews, observation and document analysis. The research instrument was developed from North Carolina Tobacco Free School Assessment Tool to determine smoking behavior, smoking cessation programs and the role of various parties in these policies and programs. Data analysis was carried out by thematic content analysis technique. Schools have implemented non-smoking area policies throughout the school area for student, school teachers, staff and visitors. However, there are no detailed regulations regarding the prevention policy of smoking behavior that is pointed out to staff, teachers and visitors. Educational programs related to the dangers of smoking are carried out in the hidden curriculum. The school has not organized a smoking cessation program. The involvement of parents and students in order to prevent smoking behavior in school’s prevention program has not yet been done optimally. The challenges in preventing smoking behavior is lack of optimal prevention smoking behavior outside of school and the absence of smoking cessation program conducted in schools. Abstrak Sekolah memiliki peranan penting dalam mencegah perilaku merokok siswa. Penelitian ini bertujuan untuk menganalisis kebijakan Kawasan Tanpa Rokok (KTR) dan program pendidikan pencegahan perilaku merokok, serta peran siswa, staf, dan orang tua dalam pencegahan perilaku merokok siswa. Penelitian kualitatif ini dilakukan di enam Sekolah Muhammadiyah Tingkat Menengah Atas di Kota Depok. Pengambilan data dilakukan pada bulan Januari 2018 hingga April 2019. Informan penelitian berjumlah 15 informan yang dipilih melalui teknik purposive sampling. Pengumpulan data dilakukan melalui wawancara mendalam, observasi, dan analisis dokumen. Instrumen penelitian dikembangkan dari North Carolina Tobacco Free School Assessment Tool untuk mengetahui kebijakan pencegahan perilaku merokok, program pendidikan untuk mencegah perilaku merokok, program upaya berhenti merokok dan peran berbagai pihak dalam kebijakan dan program tersebut. Analisis data dilakukan dengan teknik konten tematik analisis. Sekolah telah menerapkan kebijakan KTR di seluruh area sekolah bagi siswa, staf, guru dan pengunjung. Namun tidak ada peraturan mendetil terkait kebijakan pencegahan perilaku merokok yang ditujukkan kepada staf, guru dan pengunjung. Program pendidikan terkait bahaya merokok dilakukan secara hidden curriculum. Sekolah belum menyelenggarakan program berhenti merokok. Pelibatan orang tua dan siswa dalam rangka mencegah perilaku merokok di sekolah belum dilakukan secara optimal. Tantangan dalam pencegahan perilaku merokok ialah kurang optimalnya pencegahan perilaku merokok di luar sekolah serta belum adanya program berhenti merokok yang dilakukan di sekolah.

Das Zigarettenrauchen stellt für viele Menschen ein gesundheitliches Problem mit weitreichenden Folgen dar. Da das Rauchen für Patienten und Patientinnen der HIV-Sprechstunde am Universitätsspital Basel, Schweiz, ebenfalls ein großes Problem ist, konnte ein auf Evidenz basierendes Rauchstopp-Programm entwickelt werden. Dieses Programm stützt sich auf die zwei in der Literatur beschriebenen, erfolgreichen Interventionen; die Pharmakotherapie und die Beratung. Bei der Beratung steht der Dialog mit den Betroffenen und deren Empowerment im Zentrum. Das Rauchstopp-Programm beinhaltet eine Kurzintervention, eine allgemeine Raucherberatung und eine mehrteilige Einzelberatung. Da Raucherberatungen in der Schweiz bislang beinahe ausschließlich von Ärztinnen und Ärzten durchgeführt wurden und keine deutschsprachigen Publikationen von Pflegenden in diesem Gebiet existieren, beabsichtigt dieser Artikel, Pflegenden ein Konzept zur Raucherberatung anzubieten. Unsere ersten Praxiserfahrungen zeigen, dass insbesondere die Kombination bestehend aus pflegerischer und ärztlicher Langzeitbetreuung und Nikotinsubstitution eine wirksame interdisziplinäre Intervention darstellen könnte.
