ABSTRACT Objectives To systematically evaluate the effect of the surgery-first approach (SFA) on oral health-related quality of life (OHRQoL) in patients with dentofacial deformities. Materials and Methods An electronic database search and hand search of selected journals and references were carried out. Studies investigating the OHRQoL of patients receiving SFA with or without a control group were included. The risk of bias was assessed by the Cochrane risk of bias tool in randomized clinical trials (RCTs) and the Newcastle-Ottawa Scale in non-RCTs. Results A total of seven articles met the eligible criteria and were included, of which six were cohort studies and one was an RCT, and six assessed the OHRQoL of the SFA with conventional orthodontic–surgical treatment (COST) as a control and one without. A total of 214 patients were examined, with sample sizes in studies ranging from 9 to 50. A total of 3 articles successfully measured the OHRQoL both before and after treatment in both the SFA and conventional orthodontic–surgical treatment groups. A total of six cohort studies were classified as low to moderate risk of bias, and the RCT was classified as high. Conclusions The SFA could improve the OHRQoL of patients with dentofacial deformities similar to conventional orthodontic–surgical treatment at the end of complete treatment. In addition, it increases OHRQoL immediately at the beginning of treatment without a deterioration.

Individuals with dentofacial deformities have a greater negative impact on oral health and in quality of life. The aim of this study was identified the main factors involved in the perception of the oral health related quality of life in individuals with dentofacial deformities. Material and methods: In a crosssectional study, were evaluated 72 individuals in preoperative period of orthognathic surgery at the Oral and Maxillofacial Surgery Service at Federal University of Paraná and at Positivo University. The age, gender, facial profile, presence or absence of asymmetry and temporomandibular disorders (TMD) symptoms were accessed. Through the Oral Health Impact Profile-14 (OHIP-14) the perceptions about oral health related quality of life of the patients was accessed. The TMD symptoms were evaluated with Fonseca anamnestic index. Data were submitted to descriptive and inferential statistical analysis with a significance level of 0.05. Results: The OHIP-14 mean score was higher with increasing age (p= 0.007), in females (p= 0.076), asymmetric patients (p= 0.030). and according to the increase in reported TMD symptoms (p= 0.001). Individuals with no TMD symptoms had an OHIP-14 mean of 8.86±8.06; individuals with mild TMD had a mean of 15±6.28, moderate TMD had a mean of 20.6 ± 9.44 and severe TMD had a mean of 26.42±7.66. There were no differences between OHIP-14 and type of facial profile (p= 0.725). Conclusion: It is concluded that increasing age, female gender, presence of asymmetry and the severity of TMD symptoms are important factors related to poor oral health related quality of life in individuals with dentofacial deformities.

Objective. The treatment protocol in the modern health care paradigm has shifted considerably towards enhancing the quality of life in the last decade. This is particularly important in cosmetic and elective treatments, and hence the interest in oral health-related quality of life (OHRQoL) also has increased. OHRQoL always been measured by endogenous, functional, social, or psychological determinants. Self-esteem (SE) is one of the internal factors that affect the perception of malocclusion and hence OHRQoL before and after treatment. The purpose of this review is to assess whether there exists any correlation between the Oral Health-Related Quality of Life, Self-esteem (SE) in patients following orthodontic treatment. Methods. A literature search was confined to the English language using Medical Subject Heading terms (MeSH) in PubMed, Cochrane Library, and Ovid® covering the period from January 1, 1951 to May 15, 2020. Search in Google Scholar, grey literature, and hand search on cross-references was performed to find additional data. The studies found to be suitable were selected based on the predefined inclusion and exclusion criteria. The quality of assessment and risk of bias for the included studies were evaluated independently by two invigilators utilizing “The Cochrane Collaboration’s tool for assessing the risk of bias” and “Modified version of the Newcastle Ottawa scale” for Randomized Controlled Trials (RCTs) and non-randomized trials respectively. Results. A total of 7688 studies were retrieved from all the sources. After screening all the titles and excluding the duplicates, 28 studies were finally included for text review, and all of them were fit for quality appraisal. The design of the final studies included comprised of 3 RCTs, 14 cohort studies, 9 cross-sectional studies, and 2 case-control studies. Conclusion. There is moderate evidence to show that fixed orthodontic treatment improves OHRQoL and SE in children. OHRQoL also increased in adolescents and adults. However, there is a weak correlation between SE and OHRQoL. More evidence-based studies are needed to analyze the relationship.
